    Abstract In recent years, the concept of spread through air spaces (STAS) has been discussed as an adverse prognostic factor for lung cancer. The aim of our study is to clarify the prognostic role of STAS in relation to the main recognized prognostic factors in a retrospective cohort of 330 European patients who underwent stages I to III lung adenocarcinoma resection. On univariate analysis, the presence of STAS was related to progression-free survival (PFS; hazard ratio [HR]: 1.48; 95% CI: 1.02-2.19; P = 0.038) and overall survival (OS; HR: 1.61; 95% CI: 1.03-2.52; P = 0.50). On multivariate analysis, STAS was related to PFS (HR: 1.51; 95% CI: 1.00-2.17; P = 0.050) and to OS (HR: 1.67; 95% CI: 1.00-2.81; P = 0.050). We showed that the presence of STAS was associated with lower PFS, equivalent to the next pathologic T stage, especially the median PFS of T3 stages without STAS was at 62.8 months while the median PFS of T3 stages with STAS was at 15.7 months, closer to the median PFS of 17.4 months in T4 stages. To conclude, STAS is an independent prognostic factor of PFS in this European cohort and is close to significance for OS. We suggest that the presence of STAS might lead to an upstaging of lung adenocarcinoma.

    Abstract Characterisation of antioxidant fraction of the fruit of Garcinia gummi-gutta was done using liquid chromatography mass spectroscopic (LC/MS) analyses. Total poly phenolics and radical scavenging activity of various extracts such as acetone, methanol and hydroalcohol were estimated spectrophotometrically. The active extract was analysed by LC/MS in order to identify the molecular mass and tentative structures of major compounds. Phenolic compounds such as luteolin 7-O-glucuronide, kaempferol 3-O-(6-O-acetyl) glycoside, dicaffeoylquinic acid, apigenein-6-C-pentosyl-8-C-hexoside and p-coumarylquinic acid were identified from hydro alcoholic extract of G. gummi-gutta.

    Abstract Cyanthillium cinereum is a medicinal plant commonly known as little ironweed is used in various traditional systems of medicines. In Ayurveda, it is known as Sahadevi and is used as remedy for many ailments. It has been reported to possess numerous biological properties including diaphoretic, anthelmintic, diuretic, lithontriptic, anti-inflammatory and anticancer activities. Metabolite profiling of aqueous extract of C. cinereum (CCE) and its herbal formulation KGS tablet has been carried out by liquid chromatography tandem mass spectroscopic analysis. ESI-MS/MS based characterisation led to the identification of several bioactive phytochemicals belonging to various classes of metabolites like phenolic acids, phenolic glycosides, flavonoids and anthocyanin. Most of the identified compounds are known for their various biological properties, such as antioxidant, anti-inflammatory and anticancer activities.

    Abstract This article presents a clinical technique based on a case report for restoring the contours and shape of the upper teeth involved in the smile display of a young patient. After planning the treatment for the patient using digital tools (Digital pictures, Digital Smile Design, digital waxup, three-dimensional [3D] printed models, mockup), the upper teeth were restored using an improved injection technique. This improved technique involves the direct injection of flowable composite resin using clear polyvinyl siloxane molds made from 3D-printed models of the patient. The details and benefits of this new technique are described in the article.

    Abstract Background: Obtaining informed consent (IC) before a surgical procedure is the cornerstone of medical practice. The practice of IC continues to evolve as litigations increase. Most studies on patients' perspectives of IC are either old or were done in southern Nigeria. This study assessed the surgical patients' IC experience in a tertiary hospital in northwest Nigeria.
    Methods: This cross-sectional study assessed 244 consecutive patients who had elective surgeries in surgical departments of a tertiary hospital. Pretested questionnaires were used to collect data regarding their perception of the meaning of IC, the process of obtaining it, satisfaction with how it was obtained, and factors associated with satisfaction on how consent was obtained.
    Results: Most were females (61.9%); their mean age was 34.8±14.3 years; 52.9% and 61.9% of respondents did not believe that IC enables patient-clinician shared decision-making or patient's self-decision making, respectively. Most were allowed to ask questions (83.2%), received information on the surgical procedure (91.4%), diagnosis (97.9%); however, 38.5% and 48.8% did not receive information about surgical procedures' immediate and long-term complications, respectively. Surgical procedure explanation was mostly provided by Resident Doctors (53.7%). Most (88.9%) were satisfied with how IC was obtained; satisfaction was associated with being allowed to ask questions, receiving explanations on diagnosis, surgical-procedure, complications of surgery, available alternative treatments, and when the resident/ consultants gave the explanation (all P<0.05).
    Conclusion: Deficiencies exist in the process of getting IC. Satisfaction with this process was high though associated with following the recommended strategies. Improving the IC process will require appropriate interventions in this and similar settings.
