    Abstract Sedation is common during digestive endoscopy to provide comfort and pain relief for patients. However, the use of sedation in endoscopy also poses potential risks, and recent issues have been raised regarding its safety and administration. This literature review paper will discuss the most recent developments in the field of sedation in digestive endoscopy, including the adverse events that might be associated with sedation and how to manage it, the legal issues associated with administration, the impact of COVID-19 on sedation practices, and sedation in special situations. It will also touch upon the current guidelines and recommendations for sedation, including the importance of patient selection and monitoring and the need for training and certification for endoscopists administering sedation. The review will also analyse studies evaluating the safety and efficacy of various sedation techniques, including propofol, midazolam, and others. It will examine the benefits and drawbacks of these agents.

    Abstract Apple trees face various challenges during cultivation. Apple leaves, as the key part of the apple tree for photosynthesis, occupy most of the area of the tree. Diseases of the leaves can hinder the healthy growth of trees and cause huge economic losses to fruit growers. The prerequisite for precise control of apple leaf diseases is the timely and accurate detection of different diseases on apple leaves. Traditional methods relying on manual detection have problems such as limited accuracy and slow speed. In this study, both the attention mechanism and the module containing the transformer encoder were innovatively introduced into YOLOV5, resulting in YOLOV5-CBAM-C3TR for apple leaf disease detection. The datasets used in this experiment were uniformly RGB images. To better evaluate the effectiveness of YOLOV5-CBAM-C3TR, the model was compared with different target detection models such as SSD, YOLOV3, YOLOV4, and YOLOV5. The results showed that YOLOV5-CBAM-C3TR achieved mAP@0.5, precision, and recall of 73.4%, 70.9%, and 69.5% for three apple leaf diseases including Alternaria blotch, Grey spot, and Rust. Compared with the original model YOLOV5, the mAP 0.5increased by 8.25% with a small change in the number of parameters. In addition, YOLOV5-CBAM-C3TR can achieve an average accuracy of 92.4% in detecting 208 randomly selected apple leaf disease samples. Notably, YOLOV5-CBAM-C3TR achieved 93.1% and 89.6% accuracy in detecting two very similar diseases including Alternaria Blotch and Grey Spot, respectively. The YOLOV5-CBAM-C3TR model proposed in this paper has been applied to the detection of apple leaf diseases for the first time, and also showed strong recognition ability in identifying similar diseases, which is expected to promote the further development of disease detection technology.

    Abstract Background: The primary health care (PHC) system plays an important role in China's health care system, but there are challenges such as irrational allocation of health resources and inefficient operation, which need to be improved. The purpose of this study was to explore the impact of resource allocation on the efficiency of the PHC system in China.
    Methods: The data in 31 provinces were collected from the China Statistical Yearbook 2017-2021 and the China Health Statistical Yearbook 2017-2021. The comprehensive health resource density index (CHRDI) was constructed based on the entropy method and the health resource density index (HRDI), which was used to analyze the allocation of primary health resources in each province. The adjusted efficiency of the PHC system in each province was calculated by the bootstrap data envelopment analysis (DEA). Finally, the spatial Dubin model was used to explore the effect of the CHRDI on efficiency.
    Results: From 2016 to 2020, the allocation of primary health resources in 31 provinces showed an increasing trend, and the average efficiency after correction showed a decreasing state year by year. The spatial direct effect and spatial spillover effect coefficients of CHRDI were 0.820 and 1.471, which positively affect the efficiency. Per capita Gross Domestic Product (GDP), urbanization rate, and the proportion of the elderly were the factors affecting the efficiency of the PHC system.
    Conclusions: The allocation of primary health resources in all provinces in China has improved each year, but there are still great differences, and efficiency must be further improved. Pay attention to the spatial spillover effect of the level of resource allocation and formulate differentiated measures for different regions. Attention should also be paid to the impact of population aging and economic development on the utilization of primary health resources by increasing health needs and choices.

    Abstract Objective: To evaluate the potential impact of consistent use of similar treatments over a long period; it is essential to investigate the potential correlation between genetic variations that influence the expression or function of pharmacological targets for reducing lipid levels and the risk of developing rheumatoid arthritis.
    Methods: We used variants in the following genes to conduct Mendelian randomization analyses: HMGCR (encoding the target for statins), PCSK9 (encoding the target for PCSK9 inhibitors, such as evolocumab and alirocumab), and NPC1L1 (encoding the target for ezetimibe). Data from lipid genetics consortia (173,082 sample size) were used to weight variations according to their correlations with low-density lipoprotein cholesterol (LDL-C). In two large datasets (total n = 19,562 cases, 501,655 controls). We conducted a meta-analysis of Mendelian randomization estimates, weighted by LDL-C levels, on the regional differences in the risk of rheumatoid arthritis using data from two large databases.
    Results: We approached SMR and IVW-MR analyses to examine the relationship between target gene expression (including HMGCR, PCSK9, and NPC1L1) and LDL-C levels mediated by these genes with RA. The IVW-MR analysis revealed no significant association between genetically predicted LDL-C concentration and the risk of RA (OR = 0.88, 95% CI = 0.59-1.29; OR = 0.91, 95% CI = 0.67-1.23; OR = 0.81, 95% CI = 0.49-1.36; all p > 0.05). Similarly, our findings from the SMR approach provided no evidence to suggest that gene expression of HMGCR, PCSK9, and NPC1L1 was associated with the risk of RA (OR = 0.91, 95% CI = 0.79-1.05, p = 0.207; OR = 0.96, 95% CI = 0.85-1.09, p = 0.493).
    Conclusions: Our results do not provide evidence to support the hypothesis that reducing LDL-C levels with statins, alirocumab, or ezetimibe effectively prevents the risk of developing RA. However, our study provides valuable insights into the assessment of lipid-lowering agents in RA, which can enhance our understanding of the condition and assist in clinical practice by aiding in the determination and monitoring of RA status to clinical response.

    Abstract Immune checkpoint inhibitors (ICIs) offer significant advantages for the treatment of urologic tumors, enhancing the immune function of anti-tumor T cells by inhibiting PD-1 and PDL1 binding. They have been shown to be well tolerated and remarkably effective in clinical practice, offering hope to many patients who are not well treated with conventional drugs. Clinical trials in recent years have shown that anti-PD-1 and PD-L1 antibodies have good efficacy and safety in the treatment of urologic tumors. These antibodies can be applied to a variety of urologic tumors, such as bladder cancer, renal cell carcinoma, and prostate cancer. They have been approved for the first-line treatment or as an option for follow-up therapy. By blocking the PD-1/PD-L1 signaling pathway, ICIs can release immune functions that are suppressed by tumor cells and enhance T-cell killing, thereby inhibiting tumor growth and metastasis. This therapeutic approach has achieved encouraging efficacy and improved survival for many patients. Although ICIs have shown remarkable results in the treatment of urologic tumors, some problems remain, such as drug resistance and adverse effects in some patients. Therefore, further studies remain important to optimize treatment strategies and improve clinical response in patients. In conclusion, PD-1/PD-L1 signaling pathway blockers have important research advances for the treatment of urologic tumors. Their emergence brings new hope for patients who have poor outcomes with traditional drug therapy and provides new options for immunotherapy of urologic tumors. The purpose of this article is to review the research progress of PD-1 and PD-L1 signaling pathway blockers in urologic tumors in recent years.

    Abstract Objectives: To investigate the effect of Rain Classroom and Presentation-Assimilation-Discussion (PAD) class blended learning mode on a surgical nursing course.
    Methods: In this retrospective study, a total of 212 nursing undergraduates of Youjiang Medical University for Nationalities were selected as the research objects. There were 102 participants taking the traditional teaching model, assigned into the control group. The remaining 110 participants taking Rain Classroom and PAD class blended learning mode were assigned into the observation group. A questionnaire survey was conducted after the intervention.
    Results: After the intervention, the comprehensive assessment score of the observation group was higher than the control group ((83.8 ± 2.64) vs. (81.71 ± 3.74), P = 0.01). The independent learning ability ((81.61 ± 12.04) vs. (77.46 ± 4.23), P = 0.001), and self-efficacy ((27.78 ± 4.18) vs. (26.39 ± 4.67), P = 0.023) were higher in the observation group than those in the control group. The course satisfaction of the observation group was higher than that in the control group (79.09% and 65.69%, P = 0.029).
    Conclusions: The blended mode of Rain Classroom with PAD class effectively improves teaching quality, academic performance, self-learning ability, self-efficacy of students, and increased students' satisfaction with teaching methods.

    Abstract Numerous studies have examined the transfer of carbon emissions in China's trade. However, few papers have established a link between China's domestic value chains (DVCs) and the global value chains (GVCs) to comprehensively trace the flows of carbon emissions within a unified framework. To address this research gap, our paper aims to connect China's DVCs with the GVCs and map the carbon emissions flows of China's eight regions in both domestic and international trade. Using structural decomposition analysis (SDA), it studies the driving factors behind change in carbon emissions transfer (CET). Our findings are as follows: Firstly, in 2018, the total carbon emissions transfer of China's eight regions amounted to 5122.0mt, a significant increase of 31.1 % compared to 2012. Notably, the carbon emissions transfer through intermediate product trade is more than four times that of the final product trade. Secondly, significant variations exist in the sources and destinations under different trade patterns, which have been largely overlooked in the existing literature. Over 70.0 % of carbon emissions resulting from final product trade are directed towards regions outside of Chinese mainland, while approximately two-thirds of carbon emissions resulting from intermediate product trade are allocated to Chinese mainland's internal regions. Thirdly, the reduction of carbon emissions transfer increment is primarily driven by the carbon intensity, while the indirect demand scale effect of Chinese mainland's internal regions emerges as the most significant driving factor, playing a substantial role in the increase of carbon emissions transfer. Additionally, the unreasonable input-output structure between regions within Chinese mainland has contributed to the rise in carbon emissions transfer. The research findings offer valuable insights and policy recommendations for the formulation of regional carbon reduction policies in China.

    Abstract Purpose: Due to the existence of information opacity, there is a common problem of adverse selection in the process of screening alternative technology start-ups (TSs) and determining investment targets by venture capital institutions, which does not reveal the true value of enterprises and makes the market inefficient. The aim of this paper is to design an evaluation and screening system help venture capital institutions to select the qualified TSs as their investment objective.
    Design: A research framework of four dimensions that include conception, technical innovation, business model and team structure, was built based on previous studies. Based on the research framework, 15 second-level indicators and 33 third-level indicators were extracted with literature research method. This paper proposes an evaluation model with back propagation neural network (BPNN) optimized by genetic algorithm (GA) to improve the rate of selecting and investing in qualified start-ups.
    Findings: The results show that the evaluation accuracy of the evaluation model for qualified and unqualified enterprises can reach 80.33% and 93.67% respectively, which has verified the effectiveness of the model and algorithm.
    Originality/value: This paper established an effective evaluation system based on PCA and GA-BPNN to help venture capital institutions preliminarily screen potential technology start-ups, which provides the possibility for venture capital institutions to greatly reduce the screening time and cost, improve the screening efficiency of TSs, and scientifically assess the risk of investee projects or investee enterprises to obtain sustainable and stable excess profits.
