    Abstract Advances in site-selective molecular editing have enabled structural modification on complex molecules. However, thus far, their applications have been restricted to C-H functionalization chemistry. The modification of the underlying molecular skeleton remains limited. Here, we describe a skeletal editing approach that provides access to benzazepine structures through direct nitrogen atom insertion into arenols. Using widely available arenols as benzazepine precursors, this alternative approach allowed the streamlined assembly of benzazepines with broad functional group tolerance. Experimental mechanistic studies support a reaction pathway involving dearomatizative azidation and then aryl migration. This study further highlights the potential for carbon-nitrogen transmutation sequences through combinations with oxidative carbon atom deletion, providing an alternative for the development of N-heteroarenes and demonstrating significant potential in materials chemistry.

    Abstract Tumor-associated macrophages (TAMs) are present in almost all solid tumor tissues. 16They play critical roles in immune regulation, tumor angiogenesis, tumor stem cell activation, tumor invasion and metastasis, and resistance to therapy.     Abstract Unknown domain shift caused by the unavailability of target domain during training phase degrades the performance of intelligent fault diagnosis models in practical applications. Domain generalization (DG)-based methods have recently emerged to alleviate the influence of domain shift and improve the generalization ability of models toward invisible working conditions. However, most existing studies are conducted on multiple fully labeled source domains. Meanwhile, domain-specific information related to the variations of working conditions is often neglected during model training. Therefore, in order to realize reliable generalization fault diagnosis based on partially labeled source domains, this article proposes a contrast-assisted domain-specificity-removal network (CDSRN) to extract transferable features from domain-specificity-removal perspective. Concretely, a domain-specific feature removal branch is designed to disentangle domain-invariant features and domain-specific features, thus excavating generalized information only in domain-invariance dimension. Simultaneously, proxy-contrastive representation enhancement module is embedded to facilitate the fault class-discriminative and domain-discriminative feature learning, thereby assisting the model in further improvement of generalization capability. Experimental studies confirm the effectiveness and competitiveness of the proposed CDSRN in semi-supervised generalization fault diagnosis.

    Abstract Background: Post-operative diarrhoea is a common adverse event after pancreatic surgery. While the risk factors for this condition have been identified, the increasing trend of administering chemotherapy before surgery might change these factors. This study aimed to identify the risk factors of post-operative diarrhoea in patients with pancreatic ductal adenocarcinoma (PDAC) who underwent neoadjuvant chemotherapy.
    Design: A retrospective cohort study.
    Methods: Patients who underwent neoadjuvant chemotherapy and pancreatectomy because of PDAC between 2021 and 2023 were included. The preoperative characteristics of, operative details of and post-operative outcomes in patients with and without post-operative diarrhoea were collected and compared. The independent risk factors of post-operative diarrhoea were identified using logistic regression analysis. STROBE checklist was used.
    Results: Post-operative diarrhoea occurred in 65 out of 145 (44.8%) patients during hospitalization. Elevated white blood cell count, advanced tumour stage, and late abdominal drain removal were independent risk factors for post-operative diarrhoea (p < .001, p = .006 and p = .009, respectively).
    Conclusions: Some perioperative factors influence post-operative diarrhoea in patients who undergo neoadjuvant chemotherapy. More attention should be paid to patients at a higher risk of post-operative diarrhoea, with an emphasis on high-quality management for these patients.

    Abstract Objectives: In this study, we try to investigate the risk factors of postoperative surgical site infection (SSI) in closed pilon fractures and establish a nomogram prediction model.
    Methods: From January 2012 to June 2021, 516 closed pilon fracture patients were included in this study. Of these, 387 patients were randomly assigned to the training group and 129 patients were assigned to the validation group (3:1). By univariate and multivariate Cox analysis, we identified independent risk factors for postoperative SSI after Pilon fracture. We established a nomogram model and used receiver operating characteristic (ROC) and calibration chart to evaluate its discriminant and calibration.
    Results: SSI occurred in 71 patients in the training group and 23 patients in the validation group. Ultimately, age, preoperative blood sugar, operative time, Tscherne classification and fracture classification were identified as independent risk factors for SSI. The AUC values for SSI of the training and validation group were 0.898 and 0.880, and the P value of the Hosmer-Lemeshow test was 0.125. We established a nomogram prediction model based on age, preoperative blood sugar, operative time, Tscherne classification and fracture classification.
    Conclusion: Our nomogram model had good discrimination and calibration power, so it could be used to predict SSI risk in patients with pilon fracture.

    Abstract Tumor cells can evade immune surveillance by expressing immune checkpoint molecule ligands, resulting in effective immune cell inactivation. Immune checkpoint blockades (ICBs) have dramatically improved survival of patients with multiple types of cancers. However, responses to ICB immunotherapy are heterogeneous with lower patient response rates. The advances have established that the gut microbiota can be as a promising target to overcome resistance to ICB immunotherapy. Furthermore, some bacterial species have shown to promote improved responses to ICBs. However, gut microbiota is critical in maintaining gut and systemic immune homeostasis. It not only promotes differentiation and function of immunosuppressive immune cells but also inhibits inflammatory cells via gut microbiota derived products such as short chain fatty acids (SCFAs), tryptophan (Trp) and bile acid (BA) metabolites, which play an important role in tumor immunity. Since the gut microbiota can either inhibit or enhance immune against tumor, it should be a double-edged sword in ICBs against tumor. In this review, we discuss the effects of gut microbiota on immune cells and also tumor cells, especially enhances of gut microbiota on ICB immunotherapy. These discussions can hopefully promote the development of ICB immunotherapy.

    Abstract ObjectiveTo investigate the cognition of e-cigarette compositions and hazards and its influencing factors among secondary school personnel (junior, senior and vocational high school) in Shanghai, so as to provide ideas for improving the cognition level of e-cigarettes among the school personnel and tobacco control efforts on campus in the future.MethodsA multi-stage stratified cluster random sampling method was used to conduct an anonymous online questionnaire survey among 1 767 secondary school personnel in Shanghai.ResultsThe proportion of school staff who had used e-cigarettes was 3.79%. The participants who were not sure whether the e-cigarette smoke was “mainly water vapor (incorrect)” and whether the e-cigarette smoke “contained nicotine (correct)” and “contained formaldehyde (correct)” accounted for 62.71%, 54.44% and 61.91%, respectively. Among them, 68.48%, 55.46% and 50.37%, respectively, agreed that e-cigarettes would be “addictive”, “harmful” and “that second-hand smoke was harmful”. Binary logistic regression analysis showed the following risk factors for the low level of school personnel’s awareness of e-cigarette compositions: age ≥40 years (OR=1.65, 95%CI: 1.32‒2.06), working in junior high school (OR=1.37, 95%CI: 1.09‒1.73), educational attainment level as junior college or below (OR=1.67, 95%CI: 1.02‒2.74), and not having participated in tobacco control education activities (OR=1.37, 95%CI: 1.08‒1.73). Meanwhile, working in junior (OR=1.43, 95%CI: 1.12‒1.83) or senior (OR=1.44, 95%CI: 1.08‒1.92) high school, educational attainment level as junior college or below(OR=2.10, 95%CI: 1.24‒3.56), having used e-cigarettes (OR=2.98, 95%CI: 1.63‒5.42), not having participated in tobacco control education activities (OR=1.49, 95%CI: 1.16‒1.92) and low level of awareness of e-cigarette compositions (OR=4.24, 95%CI: 3.44‒5.23) were risk factors for the low level of school personnel’s awareness of e-cigarette hazards.ConclusionsThe level of e-cigarette awareness among school personnel in Shanghai is low, ...
