    Abstract Background & aims: Pretreatment predictors of finite nucleo(s)tide analogue (NUC) therapy remain elusive. We studied the association between pretreatment HBV DNA levels and outcomes after therapy cessation.
    Methods: Patients with chronic hepatitis B who were HBeAg negative at the start of NUC treatment were enrolled from sites in Asia and Europe. We studied the association between pretreatment HBV DNA levels and (1) clinical relapse (defined as HBV DNA >2,000 IU/ml + alanine aminotransferase >2 × the upper limit of normal or retreatment) and (2) HBsAg loss after NUC withdrawal.
    Results: We enrolled 757 patients, 88% Asian, 57% treated with entecavir, with a median duration of treatment of 159 (IQR 156-262) weeks. Mean pretreatment HBV DNA levels were 5.70 (SD 1.5) log IU/ml and were low (<20,000 IU/ml) in 150 (20%) and high (>20,000 IU/ml) in 607 (80%). The cumulative risk of clinical relapse at 144 weeks after therapy cessation was 22% among patients with pretreatment HBV DNA levels <20,000 IU/ml
    Conclusions: Lower pretreatment HBV DNA levels are associated with a lower risk of clinical relapse and a higher chance of HBsAg loss after cessation of NUC therapy, independent of end-of-treatment viral antigen levels. Further studies are needed to confirm these findings in non-Asian populations.
    Impact and implications: A subgroup of patients with chronic hepatitis B may not require retreatment after stopping antiviral therapy. In this study, comprising 757 patients with chronic hepatitis B from Europe and Asia, we found that higher viral load before initiation of treatment was a risk factor for relapse after stopping treatment. Patients with a low HBV DNA level before starting antiviral therapy had the lowest risk of relapse, and a high chance of HBsAg loss, after stopping treatment. These findings can help select patients for treatment withdrawal and guide intensity of off-treatment monitoring.

    Abstract Background: A considerable proportion of recipients of liver transplantations who are presented hepatitis B immunoglobulin (HBIG) monotherapy for hepatitis B virus (HBV) prophylaxis develop HBIG resistance. In this study, we investigated the mutation patterns in the major hydrophilic region (MHR) of amino acid sequences 100 to 160.
    Methods: Using the gene sequence analyzer for amino acid sequences 0 to 226 in the S/pre-S region we analyzed blood samples of 15 patients showing HBIG resistance after high-dose HBIG prophylaxis.
    Results: Various mutations in the MHR were observed in 14/15 samples: Gly145Arg mutation in 8/13 Adr subtype and 1/2 Ayw subtype samples (60%). The next most common mutation was Gly165Trp in 8/13 Adr subtype but neither of 2 Ayw subtype samples (53.3%). Concurrent antiviral resistance was noted in 5 patients: lamivudine (n = 5), or entecavir (n = 3), but not adefovir, suggesting the occurrence of simultaneous, antiviral cross-resistances. Two patients underwent retransplantation due to the progression of HBV infection despite vigorous antiviral therapy. At diagnosis of HBV recurrence, the mean HBV DNA load was 6.5 × 10(6) copies/mL; 4 patients showed paradoxical coexistence of anti-HBs and HBsAg. Currently, 2 subjects show low-level HBV DNA replication in peripheral blood, although the other 12 had no DNA replication after prolonged antiviral therapy.
    Conclusions: This study suggested that various mutations in the "a" determinant were associated with HBIG resistance. Since treatment failure to rescue antiviral therapy was often associated with delayed detection of HBV recurrence rather than concurrent antiviral resistance, frequent HBV surveillance using more sensitive screening tests, such as HBeAg and HBV DNA polymerase chain reaction assay, seems to be mandatory.

    Abstract Students may need to decide whether to invest limited resources evenly across all courses and thus end with moderate grades in all, or focus on some of the courses and thus end with variable grades. This study examined which pattern of grades is perceived more favorably. When judging competency, people give more weight to positive than negative information, in which case heterogeneous grades would be perceived more favorably as they have more positive grades than homogeneous moderate grades. Furthermore, high school students are told to demonstrate their passion in college applications. Nonetheless, people generally overweigh negative information, which can result in a preference for a student with homogeneous grades lacking extremely negative grades. The college admissions decisions in particular may also involve emphasis on long-term stable, consistent, and responsible character, which the homogeneous grades may imply. Study 1 found that laypeople, undergraduate students, and admissions officers preferred to admit a student with homogeneous grades to a college than a student with heterogeneous grades even when their GPAs were the same. Study 2 used a heterogeneous transcript signaling a stereotypic STEM or humanities student, and found that while undergraduate students were more split in their choices, laypeople and admissions officers still preferred a student with homogeneous grades. Study 3 further replicated the preference for a student with homogeneous grades by using higher or lower average GPAs and wider or narrower range of grades for the heterogeneous grades. Possible reasons and limitations of the studies are discussed.

    Purpose: To compare the diagnostic performance of synthetic and acquired high b-value (1500 s/mm
    Study type: Retrospective.
    Population: A total of 108 patients (median age 49 years [range 32-77]) with 133 breast cancers.
    Field strength/sequence: 3T, echo-planar imaging.
    Assessment: Three radiologists independently reviewed image sets of both synthetic (S-b1500) and acquired (A-b1500) high b-value DWI. Malignancy confidence of the lesion was scored using a 6-point Likert-type scale.
    Statistical test: Jack-knife alternative free-response receiver-operating characteristic 1 (JAFROC1) analysis was used. Sensitivity and positive predictive value (PPV) were compared using generalized estimating equations. An independent t-test was used to compare the confidence. An intraclass correlation coefficient was calculated to compare interobserver agreement.
    Results: The JAFROC1 figures of merit values were 0.816 and 0.808 in S-b1500 and A-b1500, respectively, with no statistically significant difference (P = 0.637). Sensitivity was higher in synthetic than in A-b1500 for readers 2 (P = 0.015) and 3 (P = 0.037). Although sensitivity was higher in S-b1500 than in A-b1500 for reader 1, the difference was not significant (P = 0.487). The PPV of S-b1500 was not significantly different from that of A-b1500 (P = 0.397). The malignancy confidence of true-positive tumors was higher in S-b1500 than in A-b1500 (P = 0.013). Interobserver agreement was good for both sequences.
    Data conclusions: The synthetic high b-value DWI may improve the diagnostic sensitivity for breast cancer detection without affecting PPV compared with acquired high b-value DWI.

    Abstract In a data sample of approximately 1.3 fb;{-1} collected with the D0 detector between 2002 and 2006, the orbitally excited charm state D_{s1};{+/-}(2536) has been observed with a measured mass of 2535.7+/-0.6(stat)+/-0.5(syst) MeV/c;{2} via the decay mode B_{s};{0}-->D_{s1};{-}(2536)mu;{+}nu_{mu}X. A first measurement is made of the branching ratio product Br(b[over ]-->D_{s1};{-}(2536)mu;{+}nu_{mu}X)xBr(D_{s1};{-}-->D;{*-}K_{S};{0}). Assuming that D_{s1};{-}(2536) production in semileptonic decay is entirely from B_{s};{0}, an extraction of the semileptonic branching ratio Br(B_{s};{0}-->D_{s1};{-}(2536)mu;{+}nu_{mu}X) is made.

    Abstract Data recorded by the D0 experiment at the Fermilab Tevatron Collider are analyzed to search for neutral Higgs bosons produced in association with b quarks. This production mode can be enhanced in the minimal supersymmetric standard model (MSSM). The search is performed in the three b quark channel using multijet triggered events corresponding to an integrated luminosity of 1 fb(-1). No statistically significant excess of events with respect to the predicted background is observed and limits are set in the MSSM parameter space.

    Abstract We report the first measurement of the branching fraction for the inclusive decay B-->X(s)l(+)l(-), where l is either an electron or a muon, and X(s) is a hadronic recoil system that contains an s quark. We analyzed a data sample of 65.4 x 10(6) B meson pairs collected with the Belle detector at the KEKB e(+)e(-) asymmetric-energy collider. We find B(B-->X(s)l(+)l(-))=[6.1 +/-1.4(stat) +1.4-1.1(syst)] x 10(-6) for dilepton masses greater than 0.2 GeV/c(2).

    Abstract We report the first observation of a B meson decay that is not accessible by a direct spectator process. The channel B(0)-->D(+)(s)K- is found in a sample of 85 x 10(6) BB; events, collected with the Belle detector at KEKB, with a branching fraction B(B(0)-->D(+)(s)K-)=(4.6(+1.2)(-1.1)+/-1.3) x 10(-5). We also obtain evidence for the B0-->D(+)(s)pi(-) decay with branching fraction B(B0-->D(+)(s)pi(-))=(2.4(+1.0)(-0.8)+/-0.7) x 10(-5). This value may be used to extract a model-dependent value of |V(ub)|.

    Abstract We present the first simultaneous measurement of the ratio of branching fractions, R=B(t --> Wb)/B(t --> Wq), with q being a d, s, or b quark, and the top-quark pair production cross section sigma(tt[over]) in the lepton plus jets channel using 0.9 fb(-1) of pp[over] collision data at sqrt(s)=1.96 TeV collected with the D0 detector. We extract R and sigma(tt[over]) by analyzing samples of events with 0, 1, and > or =2 identified b jets. We measure R=0.97(+0.09)/(-0.08)(stat+syst) and sigma(tt[over])=8.18(+0.09)(-0.84)(stat+syst) +/- 0.50(lumi) pb, in agreement with the standard model prediction.

    Abstract Backgrounds and aims: In chronic hepatitis B virus (HBV) infection, quantitative HBV surface antigen (qHBsAg) is useful for monitoring viral replication and treatment responses. We aimed to determine whether pre-S mutations have any effect on circulating qHBsAg.
    Methods: Plasmids expressing 1–8 amino acid deletion in pre-S1 ("pre-S1Δ1-8") and 3-25 amino acid deletion in pre-S2 ("pre-S2Δ3-25") were constructed. At 72 h posttransfection into Huh7 cells, qHBsAg were measured using electrochemiluminescence immunoassay analyzer. To mimic milieus of quasispecies, we co-transfected either pre-S1Δ1-8 or pre-S2Δ3-25 with wild type (WT).
    Results: Pre-S mutations affected transcription and replication ability of HBV because of altered overlapping polymerase. Compared with WT, extracellular qHBsAg in pre-S1Δ1-8 and pre-S2Δ3-25 were on average 3.87-fold higher and 0.92-fold lower, respectively, whereas intracellular qHBsAg in pre-S1Δ1-8 and pre-S2Δ3-25 were 0.57-fold lower and 1.60-fold higher, respectively. Immunofluorescence staining of cellular HBsAg showed that pre-S1Δ1-8 had less staining and that pre-S2Δ3-25 had denser staining. As ratios of either pre-S1Δ1-8 or pre-S2Δ3-25:WT increased from 0:10 to 10:0 gradually, relative extracellular qHBsAg increased from 1.0 to 3.85 in pre-S1Δ1-8 co-transfection, whereas those decreased from 1.0 to 0.88 in pre-S2Δ3-25 co-transfection.
    Conclusion: Pre-S mutations exhibit different phenotypes of genome replication and HBsAg expression according to their locations. Thus, qHBsAg level for diagnosis and prognostification in chronic HBV infection should be used more cautiously, considering emergences of pre-S deletion mutants.
