    Abstract In important recent studies about organized crime in contemporary Italy you generally do not find any historical materials with regard to the Bourbon's age. This research focuses on the phenomenon of Camorra, which turned out in Naples in the post-Restoration as a strong territorial power based on extortion. I've studied it in the judicial police documentation you can find in public archives since 1840. The historical sources referring to first years document a progressive identification of Camorra and Camorrists by the officials, who tended to distinguish this phenomenon from other criminal events that were common in the same area. Among the cases recorded by the institution during the Forties of the XIXth century, we generally find some materials about gambling, prostitution and prison extortion, which are the traditional fields of Camorra.-

    In the following ten-years period, new kinds of Camorra are recorded in several fields: the foods markets, the porterage services, the goldsmith's work and the contraband. From Social History my case-study goes on to Political History during the important movements in 1848. Police sources document some very interesting political engagements of several Camorrists both in liberal networks in Naples and in prison strong disorders. This active involvement in constitutional front, which goes on for all the Fifties, induce a deeper attention and new repression strategies by Bourbon's police towards this particular criminal groups in the capital of the Kingdom. During the fall of the Bourbon State in 1860, Camorrists' lasting liberal networks produce the well-known cooptation of them in the Guardia cittadina by Liborio Romano.-

    Police sources of the Italian State in the Sixties, and first excellent sociological literature about Camorra as well, document both continuative social aspects of the phenomenon, and by contrast a new liberal perception of the unbearable territorial organized crime in Naples to be repressed more seriously, like future antimafia

    Abstract Ten years after the death of Alessandro Liberati, the authors wonder how much the principles that guided the action of the founder of the Italian Cochrane Centre are still relevant. The guiding star of Liberati's professional life was the fight for useful research aimed at solving problems relevant to patients and their families. Consistently with the work of other researchers such as Sir Iain Chalmers and Paul Glasziou - they have recently offered important contribution on these issues - Alessandro Liberati worked hard to guarantee open, accessible, and transparent research to citizens, health decision makers and health professionals. Unfortunately, his final call to design a new research governance remained unanswered. Research is still too focused on individual therapeutic interventions rather than on overall care strategies. Studies are still too often unpublished or only partially published. Research is still influenced by conflicts of interest and peer review is often biased. Much work remains to be done, to ensure that publicly funded research is guided by the real needs of citizens and patients.

    Abstract Spectral contrast, the ability to measure frequency components of vastly different intensity, is critical in optical spectroscopy. For high spectral contrast at high spectral resolution, scanning etalons are generally used, as they allow cascading multiple dispersive elements. However, scanning instruments are inherently limited in terms of acquisition speed. Here we report a single-shot cascaded spectrometer design, in which light is dispersed along a single dispersion direction at every stage and thus can be recirculated in the same etalon multiple times.     Abstract Until recently, conventional mitral valve surgery has been the treatment of choice even in secondary mitral regurgitation. Recent evidence, however, advocates the use of transcatheter edge-to-edge mitral valve repair (TEER) of the mitral valve. This has been reflected by the change in guidelines of the American College of Cardiology/American Heart Association. We reviewed the literature to shed light on the risks and benefits of all interventions, surgical, transcatheter and guideline-directed medical therapy. Secondary mitral regurgitation occurs due to an imbalance between closing forces and tethering forces. Given the pathology extends beyond the valve alone, treatment should be directed at restoring the geometrical shape of the left ventricle alongside the valve. Myocardial revascularization plays a pivotal role in preventing recurrence. The role of papillary muscle approximation in addition to restrictive mitral annuloplasty should be considered in a select group of patients. We also reviewed the current literature on TEERs from the COAPT and Mitra-FR trials while highlighting the concept of proportionate/disproportionate MR which may help identify which patients benefit from mitral valve restoration. Treatment of this condition will require robust randomized trials alongside the use of state-of-the-art imaging technologies available with the full complement of the multidisciplinary team to ensure the best outcomes for each patient.

    Abstract Objectives: The current guidelines still do not include specific recommendations on the use of subvalvular repair (SV-r) for treatment of ischemic mitral regurgitation (IMR). Therefore, the objective of our study was to evaluate the clinical impact of mitral regurgitation (MR) recurrence and ventricular remodeling on long-term outcomes after SV-r combined with restrictive annuloplasty (RA-r).
    Methods: We performed a subanalysis of the papillary muscle approximation trial, studying 96 patients with severe IMR and coronary artery disease undergoing restrictive annuloplasty alongside subvalvular repair (SV-r + RA-r group) or restrictive annuloplasty alone (RA-r group). We analyzed treatment failure differences, the influence of residual MR, left ventricular remodeling, and clinical outcomes. The primary endpoint was treatment failure (composite of death; reoperation; or recurrence of moderate, moderate-to-severe, or severe MR) within 5 years of follow-up after the procedure.
    Results: A total of 45 patients showed failure of the treatment within 5 years, of which 16 patients underwent SV-r + RA-r (35.6%) and 29 underwent RA-r (64.4%,
    Conclusions: RA-r remains a surgical mitral repair technique with an increased risk of failure and mortality at 5 years compared with SV-r. The rates of recurrent MR are higher, and recurrence occurs earlier, with RA-r alone compared to SV-r. The addition of the subvalvular repair increases the durability of the repair, thus extending all of the benefits of preventing MR recurrence.
