    Abstract Objective: Exposure to natural disasters during and after pregnancy may increase adverse mental health outcomes. Hurricanes Irma and Maria struck Puerto Rico in September 2017. Our objectives were to understand hurricane-related experiences, maternal health concerns, and the impact of hurricane experiences on postpartum depressive symptoms (PDS).
    Methods: We used data from the 2018 Pregnancy Risk Assessment Monitoring System to describe differences in maternal hurricane experiences among women who were pregnant during and after the 2017 hurricanes. We assessed maternal concerns and PDS. We estimated adjusted prevalence ratios (aPRs) and 95% CIs for the associations between hurricane experiences and PDS.
    Results: The most frequently reported hurricane experiences were losing power for ≥1 week (97%) and feeling unsafe due to lack of order/security (70%). Almost 30% of women who were pregnant during the hurricanes reported missing prenatal care. PDS were reported by 13% of women. Most hurricane experiences were associated with an increased prevalence of PDS. Feeling unsafe (aPR = 2.4; 95% CI, 1.2-4.9) and having difficulty getting food (aPR = 2.1; 95% CI, 1.1-4.1) had the strongest associations.
    Conclusions: Most women who were pregnant during or after hurricanes Irma and Maria struck Puerto Rico reported negative hurricane experiences, and most experiences were associated with an increased prevalence of PDS. Understanding the experiences of pregnant women during and after disasters and identifying risks for adverse mental health outcomes after pregnancy are important to inform emergency preparedness and prenatal and postpartum care.

    Abstract Abstract Background After its landfall in Puerto Rico in 2017, Hurricane Maria caused the longest blackout in United States history, producing cascading effects on a health care system that had already been weakened by decades of public sector austerity and neoliberal health reforms. This article addresses how health care professionals and administrators experienced the health care system’s collapse and the strategies used by them to meet their communities' health needs. Methods Data were collected between September 2018 and February 2020. Ethnographic observations in health care facilities and semi-structured qualitative interviews with representatives of the health care system were conducted. This paper focuses on data from interviews with health care providers (n = 10) and administrators (n = 10), and an ethnographic visit to a pop-up community clinic. The analysis consisted of systematic thematic coding of the interview transcripts and ethnographic field notes. Results Results provide insight on how participants, who witnessed first-hand the collapse of Puerto Rico’s health care system, responded to the crisis after Maria. The prolonged power outage and lack of a disaster management plan were partly responsible for the death of 3,052 individuals who experienced extended interruptions in access to medical care. Participants reported a sense of abandonment by the government and feelings of mistrust. They also described the health sector as chaotic and lacking clear guidelines on how to provide services or cope with personal crises while working under extreme conditions. In such circumstances, they developed resilient responses to meet communities’ health needs (e.g., itinerant acupuncture services, re-locating physicians to local pharmacies). Conclusions Participants’ narratives emphasize that the management of Hurricane Maria was fraught with political and economic constraints affecting Puerto Rico.     Abstract Background: After its landfall in Puerto Rico in 2017, Hurricane Maria caused the longest blackout in United States history, producing cascading effects on a health care system that had already been weakened by decades of public sector austerity and neoliberal health reforms. In the context of governmental abandonment, the authors argue for the importance of developing alternative strategies in post-disaster health care provision among health professionals and administrators who work at the front lines of recovery.

    Abstract Objectives: Hurricane Maria caused catastrophic damage in Puerto Rico, increasing the risk for morbidity and mortality in the post-impact period. We aimed to establish a syndromic surveillance system to describe the number and type of visits at 2 emergency health-care settings in the same hospital system in Ponce, Puerto Rico.
    Methods: We implemented a hurricane surveillance system by interviewing patients with a short questionnaire about the reason for visit at a hospital emergency department and associated urgent care clinic in the 6 mo after Hurricane Maria. We then evaluated the system by comparing findings with data from the electronic medical record (EMR) system for the same time period.
    Results: The hurricane surveillance system captured information from 5116 participants across the 2 sites, representing 17% of all visits captured in the EMR for the same period. Most visits were associated with acute illness/symptoms (79%), followed by injury (11%). The hurricane surveillance and EMR data were similar, proportionally, by sex, age, and visit category.
    Conclusions: The hurricane surveillance system provided timely and representative data about the number and type of visits at 2 sites. This system, or an adapted version using available electronic data, should be considered in future disaster settings.

    Abstract The goal of this study was to evaluate the prevalence of anoplocephalide eggs in housed horses at the Santa Maria Jockey Club. Stool samples from 66 animals were obtained from august to november 2006. Anoplocephalide presence was investigated by using the modified method of sedimentation Dennis, Stone and Swanson. A questionnaire was applied to evaluate the criteria adopted by the animal caretakers in order to choose the antiparasitic drug to be used. The anoplocephalides were detected in 100% of the animals. These results demonstrate that parasite prevalence was high due of improper treatments, including incorrect choice and application of the drug. In general, these factors are linked because the animal treatment is determined by the animal caretaker and not by the veterinarian, leading to serious infection by the presence of these agents. O objetivo deste trabalho foi avaliar a prevalência de anoplocefalídeos de eqüinos estabulados no Jockey Club de Santa Maria através de análise coproparasitológica. Amostras de fezes oriundas de 66 animais foram obtidas de agosto a novembro de 2006. Pelo emprego do método de sedimentação Dennis, Stone e Swanson modificado verificou-se a presença de ovos de anoplocefalídeos. Foi aplicado um questionário a fim de avaliar a tomada de decisão da administração de droga antiparasitária pelos treinadores. Os anoplocefalídeos foram detectados em 100% das amostras analisadas. Os resultados obtidos demonstraram uma alta prevalência desses parasitas, podendo ser justificado, dentre outros fatores, pela adoção de sistemas de tratamento antiparasitário inadequados, os quais incluem principalmente a escolha e a forma de aplicação incorreta das drogas, associados à atribuição da decisão do tratamento dos animais ser de responsabilidade do próprio tratador, e não do Médico Veterinário, contribuindo para o agravamento da infecção por esses agentes.
