    Abstract Purpose of review: The following overview article summarizes the most important aspects of diagnosis and screening and provides an overview on the current evidence of glucose-lowering and heart failure treatment in patients with diabetes.
    Recent findings: Patients with diabetes exhibit an increased risk to develop heart failure and the presence of both comorbidities has a major impact on the prognosis of these patients. Thus, it is of utmost importance to detect heart failure in patients with diabetes and to screen all patients with heart failure for the presence of diabetes. Moreover, the diagnosis of heart failure in diabetes often requires an adjustment of medical therapy. The presence of the 2 comorbidities, heart failure and diabetes, in a given patient which has a major impact on the prognosis and implementation of guideline-directed therapies to reduce cardiovascular risk in this high-risk population is of critical importance.

Daher ist eine frühzeitige Diagnose und effiziente Therapie beider Erkrankungen immens wichtig. Die vorliegende Arbeit gibt einen Überblick über die Diagnose und Therapie der Herzinsuffizienz und erläutertet die Besonderheiten, auf die bei Patienten mit Diabetes mellitus geachtet werden sollte.

    Title translation Evidence-based reduction of cardiovascular risk in patients with diabetes.
    Abstract Patients with type 2 diabetes mellitus exhibit a markedly increased propensity to develop cardiovascular diseases, such as myocardial infarction, stroke and heart failure. The results from large cardiovascular outcome trials conducted in recent years with novel hypoglycemic agents, such as dipeptidyl peptidase 4 (DPP-4) inhibitors, sodium-glucose transporter 2 (SGLT2) inhibitors and glucagon-like peptide 1 (GLP-1) receptor agonists have led to novel therapeutic strategies to reduce the cardiovascular risk in patients with diabetes. This overview article summarizes the evidence derived from these trials for the various substances. It also discusses the resulting treatment algorithms that have been brought together in the recently published European Society of Cardiology (ESC) guidelines on the topic of diabetes and cardiovascular diseases.

    Abstract Patients with chronic kidney disease (CKD) are at high risk to develop cardiovascular disease with its manifestations coronary artery disease, heart failure, arrhythmias, and sudden cardiac death. In addition, the presence of CKD has a major impact on the prognosis of patients with cardiovascular disease, leading to an increased morbidity and mortality if both comorbidities are present. Therapeutic options including medical therapy and interventional treatment are often limited in patients with advanced CKD, and in most cardiovascular outcome trials, patients with advanced CKD have been excluded. Thus, in many patients, treatment strategies for cardiovascular disease need to be extrapolated from trials conducted in patients without CKD. The current article summarizes the epidemiology, clinical presentation, and treatment options for the most prevalent manifestations of cardiovascular disease in CKD and discusses the currently available treatment options to reduce morbidity and mortality in this high-risk population.

    Abstract Patients with diabetes mellitus have an increased risk for the development of cardiovascular diseases. The presence of both comorbidities has a major impact not only on the prognosis of the patients but is also decisive for the implementation of evidence-based treatment strategies for reduction of the cardiovascular risk. The new guidelines of the European Society of Cardiology (ESC) were published in 2023 and provide clear recommendations for the management of cardiovascular diseases in patients with diabetes. The most relevant aspects of these guidelines are summarized in the following overview article.
