    Abstract Maximum tumor diameter (DMAX) is considered by many Authors as an im prognostic factor in gastric cancer (GC) correlated with tumor depth (pT). Aim of this retrospective study was to evaluate the prognostic value of DMAX and its predictive role of tumor stage in a series of 164 gastric cancer patients. One hundred and sixty-four patients underwent gastrectomy between January 1994 and December 2004. DMAX value was measured on resected specimens fixed in 10% formalin solution with cut-off of 40 mm. Tumors according to these measurements were classified in DMAX1 < or = mm and DMAX2 >40 mm. The prognostic significance of DMAX along with other factors was evaluated with log-rank test and Cox regression. The correlation between DMAX and other variables was calculated by Spearman test. Overall 5-year survival rate is 56.6%. DMAX resulted significantly linked to survival at univariate analysis (p = 0.0001). The Spearman correlation test showed that DMAX is strongly correlated with tumor stage (positive predictive value of 88.5%), pT, pN, residual of tumor and grading. Our results showed that patients with large tumor at an increased risk for tumor advancement and, therefore, DMAX could represent a useful parameter for selection of patients for neoadjuvant protocols.

    Abstract This book describes theory and techniques of empathic communication, normalization and de-escalation procedures for the management of aggressive or violent patients in clinical critical settings'. Consisting of 9 chapters, it discusses in detail the self-regulation of empathy in potential dangerous interactions, as well as common mistakes and nonprofessional reactions. It also explores the basic concept of neurobiology of violence and aggression behaviour, such as brain circuitry and neuromodulators, and other rapid tranquillization guidelines. The final chapter focuses on the crucial topics of post-aggression debriefing.     Author's details Massimo Biondi is a full professor of psychiatry and head of the psychiatry residency program at the Sapienza University of Rome, Italy. He is the head of the Department of Neuroscience and Mental Health and of the acute psychiatric inpatient ward at the Policlinico Umberto I Hospital in Rome. He has authored 30 books and more than 450 scientific papers on topics ranging from psychopathology to stress and psychosomatic medicine. He serves as editor-in-chief of the peer-reviewed journal Rivista di Psichiatria, indexed in PubMed/Medline. He also serves on the editorial boards of the high-impact peer-reviewed journal Psychotherapy and Psychosomatics and several other peer-reviewed journals. Angelo Picardi is a psychiatrist and psychotherapist, currently working as a researcher at the Centre for Behavioural Sciences and Mental Health of the Italian National Institute of Health in Rome, Italy.     Title translation Clinical significance and prognostic value of tumor maximum diameter in gastric carcinoma.
    Abstract Maximum tumor diameter (MTD) is considered by many authors as an important prognostic factor in gastric cancer and, in some series, is reported to be strongly correlated with depth of infiltration. Aim of this study was to evaluate the prognostic value of tumor diameter in a monoistitutional series of 153 gastric cancer patients. The Spearman correlation coefficient was also calculated between MTD and other known prognostic factors. For statistical analysis, patients were grouped as follows: MTD 1, < or = 40 mm, and MTD2, > 40 mm. In our series, MTD resulted significantly linked to survival at univariate analysis (p = 0.0001), but multivariate analysis did not evidence MTD as an independent prognostic indicator. The Spearman correlation test documented that MTD2 is strongly correlated with tumor depth (pT), nodal status (pN) and p-stage (p < 0.01) and is a good predictor of locally advanced stage. This retrospective study showed that patients with larger tumor are at an increased risk for tumor advancement and, therefore, MTD could represent a useful parameter for choosing the most appropriate therapeutic strategy.

    Abstract Neuropsychiatry mainly deals with processes of the mind related to neurological diseases. Only secondarily, it deal with depressive and anxiety syndromes, as reactions to the presence of an acute, chronic, or degenerative neurological disease. This review illustrate the most frequent neuropsychiatric disorders in the context of the main neurological diseases, focusing on secondary psychic reactions to the development of neurological diseases. The main purpose of this paper is therefore to amplify the sensitivity of psychiatric colleagues towards these clinical conditions in order to create a communication channel with fellow neurologists and neuroradiologists that is not limited to simple counseling.

    Abstract The literature on the connection between obesity, metabolic syndrome, and subclinical hypothyroidism is critically analyzed in this narrative review. These conditions are frequently observed among adult populations and various studies and meta-analyses have assessed their association. The prevalence of subclinical hypothyroidism in obese individuals is higher than in non-obese subjects and this trend is more pronounced in unhealthy obesity phenotypes. However, the diagnosis and treatment of subclinical hypothyroidism can be difficult in obese patients. Exaggerated body fat is linked to thyroid hypoechogenicity as evident through ultrasonography and euthyroid obese people have greater TSH, FT3, and FT3/FT4 ratios than non-obese individuals in a euthyroid condition. Moreover, a reduced expression of the TSH receptor and altered function of deiodinases has been found in the adipose tissue of obese patients. Current data do not support the necessity of a pharmacological correction of the isolated hyperthyrotropinemia in euthyroid obese patients because treatment with thyroid hormone does not significantly improve weight loss and the increase in serum TSH can be reversible after hypocaloric diet or bariatric surgery. On the other hand, obesity is linked to elevated leptin levels. Inflammation can raise the risk of Hashimoto thyroiditis, which increases the likelihood that obese patients will experience overt or subclinical hypothyroidism. Both metabolic syndrome and subclinical hypothyroidism are associated with atherosclerosis, liver and kidney disease. Hence, the association of these two illnesses may potentiate the adverse effects noted in each of them. Subclinical hypothyroidism should be identified in patients with obesity and treated with appropriate doses of L-thyroxine according to the lean body mass and body weight. Randomized controlled trials are necessary to verify whether treatment of thyroid deficiency could counteract the expected risks.
