    Abstract Assigning medical codes for patients is essential for healthcare organizations, not only for billing purposes but also for maintaining accurate records of patients' medical histories and analyzing the outputs of certain procedures. Due to the abundance of disease codes, it can be laborious and time-consuming for medical specialists to manually assign these codes to each procedure. To address this problem, we discuss the automatic prediction of ICD-9 codes, the most popular and widely accepted system of medical coding. We introduce a two-stream deep learning framework specifically designed to analyze multi-modal data. This framework is applied to the extensive and publicly available MIMIC-III dataset, enabling us to leverage both numerical and text-based data for improved ICD-9 code prediction. Our system uses text representation models to understand the text-based medical records; the Gated Recurrent Unit (GRU) to model the numerical health records; and fuses these two streams to automatically predict the ICD-9 codes used in the intensive care unit. We discuss the preprocessing and classification methods and demonstrate that our proposed two-stream model outperforms other state-of-the-art studies in the literature.

    Abstract Objective: This study aimed to analyse clinical and radiological features (phenotypes) to predict revision risk after functional endoscopic sinus surgery in patients with chronic rhinosinusitis with nasal polyps.
    Method: Phenotype characteristics of the patients with chronic rhinosinusitis with nasal polyps who underwent functional endoscopic sinus surgery were analysed retrospectively.
    Results: The rates of asthma, aspirin sensitivity, smoking and a positive prick test result were significantly higher in revision functional endoscopic sinus surgery cases (

    Abstract Soğuk savaşın bitimini takiben SSCB’nin dağılması sonucunda Orta Asya ve Güney Kafkasya’da birçok yeni devlet bağımsızlığını kazandığı gibi çok kutuplu küresel sistemde sona ermiş, ABD’nin küresel güç olduğu yeni bir düzen başlamıştır. Ancak bu durum birçok bölgede istikrarsızlığa sebep olmuş, ilk olarak başını Rusya Federasyonu’nun çektiği Bağımsız Devletler Topluluğu kurulmuş daha sonra ise Çin’in de büyüyen bir dev olarak ortaya çıkmasıyla ABD’nin tek kutuplu düzenine karşı olarak Şanghay İşbirliği Örgütü kurulmuştur. Türkiye dış politikasında örgütü, yıllardır üyeliği için ayak diretilen AB’ye alternatif olarak görmekte olup ilk olarak 2007 yılında üyelik başvurusu yapmıştır. Ancak Türkiye’nin üyelik sürecinin olumlu ve olumsuz yönleriyle çok boyutlu olarak analiz edilmesi gerektiği gibi üyeliğinin; Türkiye’nin Rusya ve Çin ile karşılıklı güven ve faydaya dayalı uzun vadeli bir ilişkiyi de inşa etmesiyle mümkün olduğu görülmektedir.

    Abstract Küreselleşme süreciyle birlikte özellikle 20. yüzyılın ikinci yarısı itibariyle yerel yönetimler ve belediyelerdiplomaside aktör olarak ön plana çıkmaya başlamıştır. Yerel yönetimlerin bu diplomatik faaliyetleri kamu diplomasisinin bir alt dalı olarak yerel yönetim diplomasisi adıyla isimlendirilmiştir. Bu çalışmada ilk olarak yerel yönetim diplomasisi kavramı ana çerçevede ele alınmaktadır. Sonrasında ise yerel yönetim diplomasisi kavramı veTürkiye’deki yerel yönetim diplomasisine dair hukuki altyapı ele alınmaktadır. Ayrıca kardeş şehir uygulaması ve İstanbul’daki belediyelerin kardeş şehir uygulamaları incelenmektedir. Çalışmada yerel yönetim diplomasisinin tam olarak ne olduğu, kardeş şehir uygulamasına neden ve nasıl başvurulması gerektiği ile İstanbul’daki belediyelerin kardeş şehir uygulamalarının çeşitlerineyönelik cevap aranmıştır. Sonuç olarak çalışmayla literatürde yerel yönetimlerin diplomatik girişimlerinin çok farklı şekillerde isimlendirildiği, sürdürülebilir ve işlevsel kardeş şehir ilişkilerinin kurulması için bazı etkenlerin olduğu görülmektedir. Bunun yanında Türkiye’deki yerel yönetim diplomasisi faaliyetleri için kanunlarla bir hukuki alt yapı oluşturulmakla İstanbul belediyelerinin Türkiye’de yerel yönetim diplomasisinde en önemli aktörler olarak kültürel ve insani faaliyetler de dahil birçok konuda öne çıktıkları görülmüştür.

    Abstract The bariatric food pyramid is a standard for long-term healthy living and nutritional habits of patients who have undergone bariatric surgery, taking their gastric capacity and special nutritional requirements into account. This study aimed to evaluate how the compliance with the pyramid affects the anthropometric change in patients who have undergone bariatric surgery, depending on the period after surgery. 81 patients who have undergone bariatric surgery between August 2016 and September 2018 participated in the study. The patients were evaluated in the postoperative period and were divided into three groups according to the year they had the operation. Food consumption frequency information was obtained from the patients, and the amount of food consumed per day was recorded in grams. Protein, vegetable, fruit, grain, and oil consumption was calculated according to the pyramid and calculated as portions. A statistically significant difference was found for all three groups in terms of weight loss and body mass index (BMI) changes before and after surgery (

    Abstract Background: This study aims to reveal the relationship between lung involvement and visceral adipose tissue changes between chest-computed tomography (CT) scans taken in short intervals in COVID-19 patients.
    Methods: The retrospective study included 52 patients who tested positive for SARS-CoV-2. All patients had two chest CT exams. Lung involvement measurements were calculated by using an artificial intelligence tool. Visceral and subcutaneous fat tissue was measured at the level of the first lumbar vertebra on chest CT. Additionally, demographic and laboratory data were collected.
    Results: 52 patients were included (36.5 % female, mean age 50). Visceral fat area and visceral fat thickness changes were significantly positive predictors of total lung involvement changes (p=0.033, p=0.00024). Subcutaneous fat area and subcutaneous fat thickness changes were not associated with lung involvement change (p>0.05). CRP, IL-6, d-dimer, and ferritin levels were higher in patients who need intensive care units.
    Conclusion: Visceral adipose tissue changes may indicate that it can have a role as a reservoir of virus involvement.

    Abstract Most metallic and ceramic materials are comprised of a space-filling collection of crystalline grains separated by grain boundaries. While this grain structure has been studied for more than a century, there few rigorous results regarding its global properties available in the literature. We present a new, rigorous result for three-dimensional grain structures that relates the integral of the Gaussian curvature over the grain boundaries to the numbers of grains and quadruple junctions. The result is numerically verified for a grain structure consisting of periodic truncated octahedra.

    Abstract Objectives: To assess the possible relationship between various predictive factors (patient or surgery related) and the development of nasal septal perforations (NSP) with the help of a large study group.
    Methods: One hundred and forty-three patients were included in the study. The presence of the following factors was evaluated and compared between the NSP and healthy group: types of surgeries, presence of unilateral or bilateral mucosal tears, concomitant inferior turbinate interventions, smoking, accompanying diabetes mellitus (DM) or allergic rhinitis (AR), types of nasal packings, duration of the surgery (minutes), and the experience of the surgeon (senior/junior). Nasal septal deviations were grouped into two: simple cartilage crests at the septum base and other-more complicated-deviations.
    Results: NSP was detected in six (4.2%) patients after a mean follow-up of 9.3 ± 3.7 (min: 6 max: 14) months. None of these patients suffered from (AR) or DM. Four of these patients had unilateral and one patient had bilateral mucosal tears during the surgeries. None of the above-mentioned factors-including mucosal tears, type of the deviation or experience of the surgeon-had a significant effect on NSP.
    Conclusion: Untreated bilateral corresponding mucosal tears are the main cause of NSP. When immediately repaired, unilateral or bilateral tears do not affect the development of NSP significantly. Patient-related factors such as age, DM, smoking, AR, and procedure-related factors such as duration, the complexity of the septal deviation, type of the nasal packing, and experience of the surgeon also do not have a significant effect on NSP.

    Abstract The topological transitions that occur to the grain boundary network during grain growth in a material with uniform grain boundary energies are believed to be known. The same is not true for more realistic materials, since more general grain boundary energies in principle allow many more viable grain boundary configurations. A simulation of grain growth in such a material therefore requires a procedure to enumerate all possible topological transitions and select the most energetically favorable one. Such a procedure is developed and implemented here for a microstructure represented by a volumetric finite element mesh. As a specific example, all possible transitions for a typical configuration with five grains around a junction point are enumerated, and some exceptional transitions are found to be energetically similar to the conventional ones even for a uniform boundary energy. A general discrete formulation to calculate grain boundary velocities is used to simulate grain growth for an example microstructure. The method is implemented as a C++ library based on SCOREC, an open source massively parallelizable library for finite element simulations with adaptive meshing.
