    Abstract Home appliances are considered to account for a large portion of smart homes' energy consumption. This is due to the abundant use of IoT devices. Various home appliances, such as heaters, dishwashers, and vacuum cleaners, are used every day. It is thought that proper control of these home appliances can reduce significant amounts of energy use. For this purpose, optimization techniques focusing mainly on energy reduction are used. Current optimization techniques somewhat reduce energy use but overlook user convenience, which was the main goal of introducing home appliances. Therefore, there is a need for an optimization method that effectively addresses the trade-off between energy saving and user convenience. Current optimization techniques should include weather metrics other than temperature and humidity to effectively optimize the energy cost of controlling the desired indoor setting of a smart home for the user. This research work involves an optimization technique that addresses the trade-off between energy saving and user convenience, including the use of air pressure, dew point, and wind speed. To test the optimization, a hybrid approach utilizing GWO and PSO was modeled. This work involved enabling proactive energy optimization using appliance energy prediction. An LSTM model was designed to test the appliances' energy predictions. Through predictions and optimized control, smart home appliances could be proactively and effectively controlled. First, we evaluated the RMSE score of the predictive model and found that the proposed model results in low RMSE values. Second, we conducted several simulations and found the proposed optimization results to provide energy cost savings used in appliance control to regulate the desired indoor setting of the smart home. Energy cost reduction goals using the optimization strategies were evaluated for seasonal and monthly patterns of data for result verification. Hence, the proposed work is considered a better candidate solution for proactively optimizing the energy of smart homes.

    Abstract Many studies have demonstrated a reduced number and vasculogenic capacity of endothelial colony-forming cells (ECFCs) in diabetic patients. However, whether the vasculogenic capacity of ECFCs is recovered or not when combined with pericyte precursors, mesenchymal stem cells (MSCs), under hyperglycemic conditions has not been studied. Thus, we investigated the role of MSCs in ECFC-mediated vascular formation under high-glucose conditions. The ECFCs and MSCs were treated with normal glucose (5 mM; NG) or high glucose (30 mM; HG) for 7 days. The cell viability, proliferation, migration, and tube formation of ECFCs were reduced in HG compared to NG. Interestingly, the ECFC+MSC combination after HG treatment formed tubular structures similar to NG-treated ECFCs+MSCs. An in vivo study using a diabetic mouse model revealed that the number of perfused vessels formed by HG-treated ECFCs+MSCs in diabetic mice was comparable with that of NG-treated ECFCs+MSCs in normal mice. Electron microscopy revealed that the ECFCs+MSCs formed pericyte-covered perfused blood vessels, while the ECFCs alone did not form perfused vessels when injected into the mice. Taken together, MSCs potentiate the vasculogenic capacity of ECFCs under hyperglycemic conditions, suggesting that the combined delivery of ECFCs+MSCs can be a promising strategy to build a functional microvascular network to repair vascular defects in diabetic ischemic regions.

    Abstract Objective: Triple negative breast cancer (TNBC) is one subtype of breast cancer. It is characterized by lack of estrogen receptor, progesterone receptor, and human epidermal growth factor receptor 2. Compared with non-TNBC, TNBC is more aggressive, of higher grade, and frequently metastatic with poor prognosis, which is correlated with upregulated microvascular density. Endothelial colony-forming cells (ECFCs) mediate neovascularization, which is the crucial contributor to cancer growth and metastasis. The present study aimed to determine whether angiogenic responses of ECFCs are regulated differently by TNBC compared with non-TNBC.
    Methods: MDA-MB-231 and MCF7 cells were utilized for TNBC and non-TNBC, respectively. Bone-marrow-derived human ECFCs were treated with a conditioned medium (CM) of cancer cells to investigate the paracrine effect on angiogenesis. Also, ECFCs were co-cultured with cancer cells to evaluate the angiogenic effect of direct cell-to-cell interaction. Angiogenic responses of ECFCs were evaluated by proliferation, migration, and tube formation. Gene expression profiles of pro-angiogenic factors were also analyzed.
    Results: Migration and tube formation of ECFCs were increased by treatment with CM of MDA-MB-231, which correlated with a higher gene expression profile of pro-angiogenic factors in MDA-MB-231 compared to MCF7. Interestingly, ECFCs co-cultured with MDA-MB-231 showed further increase of tube formation, suggesting synergic mechanisms between the paracrine effect and direct interaction between the cells.
    Conclusion: The angiogenic potential of ECFCs was enhanced by TNBC through both direct and indirect mechanisms. Therefore, the investigation of signaling pathways to regulate ECFC-mediated angiogenesis will be important to the discovery of anti-angiogenic therapies to treat TNBC patients.

    Abstract With rising concerns about the functional role of long-term care hospitals in the Korean medical system, this study aimed to observe the experience of admission in the long-term care hospitals and their association with medical expenditures among patients with colorectal cancer, and to investigate disparities among vulnerable populations. Data were obtained from the National Health Insurance Senior Cohort Database in South Korea for the period 2008-2019. With 6,305 patients newly diagnosed with colorectal cancer between 2008 and 2015, we conducted a regression analysis using the Generalized Estimating Equation model with gamma distribution to investigate the association between health expenditure and the experience of long-term care hospitals. We also explored the interaction effect of disability or income, followed by subgroup analysis. Among patients who received care at long-term care hospitals, the health expenditure within one year and five years after the incidence of colorectal cancer was found to be higher than in those who did not receive such care. It was observed that the low-income and disabled groups experienced higher disparities in health expenditure. The rise in health expenditure highlights importance for functional improvement, aligning with these initial purpose of long-term care hospitals to address the growing healthcare needs of the elderly population and ensure efficient healthcare spending, of long-term care hospitals. To achieve this original intent, it is imperative for government initiatives to focus on reducing quality gaps in long-term care hospital services and addressing cost disparities among individuals with cancer, including those with disabilities or low-income.

    Abstract Purpose: Elderly patients with type 2 diabetes mellitus (T2DM) may have a higher risk of physical disability. This study investigated the incidence of gastric cancer according to physical disability status in elderly patients with T2DM.
    Methods: The National Health Insurance Service claims data were used. A total of 76,162 participants aged 60 years or above, diagnosed with T2DM, were included. The association between physical disability status and gastric cancer incidence was evaluated using the Cox regression analysis. Additionally, subgroup analysis was performed according to region.
    Results: A total of 9,154 (12.0%) individuals had physical disability. Gastric cancer incidence was more common in participants with physical disability (3.3%) than those without (2.4%). A higher risk of gastric cancer incidence was found in elderly T2DM patients with physical disability (Hazard Ratio (HR) 1.18, 95% Confidence Interval (95% CI) 1.04-1.34). Such tendencies were maintained regardless of region, although the effect of physical disability status on gastric cancer incidence was particularly significant in individuals residing in non-metropolitan areas (HR: 1.19, 95% CI: 1.01-1.40).
    Conclusion: Elderly patients with T2DM who had physical disability showed a higher risk of gastric cancer incidence. The findings suggest a need to monitor elderly T2DM patients with disability as they may be susceptible to difficulties in accessing cancer-related healthcare.

    Abstract Abstract Ultrafast optical switching in plasmonic platforms relies on the third‐order Kerr nonlinearity, which is tightly linked to the dynamics of hot carriers in nanostructured metals. Although extensively utilized, a fundamental understanding on the dependence of the switching dynamics upon optical resonances has often been overlooked. Here, all‐optical control of resonance bands in a hybrid photonic‐plasmonic crystal is employed as an empowering technique for probing the resonance‐dependent switching dynamics upon hot carrier formation. Differential optical transmission measurements reveal an enhanced switching performance near the anti‐crossing point arising from strong coupling between local and nonlocal resonance modes. Furthermore, entangled with hot‐carrier dynamics, the nonlinear correspondence between optical resonances and refractive index change results in tailorable dispersion of recovery speeds which can notably deviate from the characteristic lifetime of hot carriers. The comprehensive understanding provides new protocols for optically characterizing hot‐carrier dynamics and optimizing resonance‐based all‐optical switches for operations across the visible spectrum.

    Abstract Background: Disparities in mortality according to disability status require investment, as individuals with disabilities form the largest subset of the vulnerable population. This study aimed to investigate the association between mortality and disability status in patients with gastric cancer as well as how regional disparities modify this relationship. Methods: Data were obtained from the National Health Insurance claims database in South Korea for the period of 2006–2019. The outcome measures were all-cause 1-year, 5-year, and overall mortality. The main variable of interest was disability status, categorized into “no disability”, “mild disability”, and “severe disability”. A survival analysis based on the Cox proportional hazards model was conducted to analyze the association between mortality and disability status. Subgroup analysis was conducted according to region. Results: Of the 200,566 study participants, 19,297 (9.6%) had mild disabilities, and 3243 (1.6%) had severe disabilities. Patients with mild disabilities had higher 5- and overall mortality risks, and those with severe disabilities had higher 1-year, 5-year, and overall mortality risks than those without disabilities. These tendencies were generally maintained regardless of the region, but the magnitude of the differences in the mortality rates according to disability status was higher in the group residing in non-capital regions than in the group living in the capital city. Conclusion: Disability status was associated with all-cause mortality in patients with gastric cancer. The degree of the differences in mortality rates among those with “no disability”, “mild disability”, and “severe disability” was augmented in the group residing in non-capital regions.
