    Abstract Background: Intermittent abdominal pressure ventilator (IAPV) use started in the 1930s for ventilatory assistance with muscular dystrophy patients. Later, the device was perfected and expanded for other neuromuscular disorders (NMD). In recent years, the morbidity and mortality tracheotomies and trach tubes related renewed the interest around IAPV. However, there are no guidelines for its use. This study aimed to establish a consensus among physicians involved in its practice to provide IAPV suggestions for the treatment of patients with NMD.
    Method: A 3-step modified Delphi method was used to establish consensus. Fourteen respiratory physicians and one psychiatrist with strong experience in IAPV use and/or who published manuscripts on the topic participated in the panel. A systematic review of the literature was carried out according to the PRISMA to identify existing evidence on IAPV for patients with neuromuscular disorders.
    Results: In the first round, 34 statements were circulated. Panel members marked 'agree' or 'disagree' for each statement and provided comments. The agreement was reached after the second voting session for all 34 statements.
    Conclusions: Panel members agreed and IAPV indications, parameter settings (including procedure protocol), potential limitations, contraindications, complications, monitoring, and follow-up are described. This is the first expert consensus on IAPV.

    Abstract La presión transpulmonar (Ptp) es la diferencia entre la presión en la vía aérea (Pva) y la presión pleural (Ppl). La medición de la presión esofágica (Pes), como subrogado de la Ppl, permite el cálculo de la Ptp diferenciando la mecánica pulmonar y de la caja torácica. Para ello, existen dos métodos, el de Talmor y el definido por Gattinoni. Ambos métodos aportan información útil, particularmente en sujetos con síndrome de distrés respiratorio agudo (SDRA) para individualizar la estrategia de protección pulmonar. El objetivo del siguiente paso a paso es describir el cálculo de la Ptp, con ambos métodos, durante la fase de soporte total de la ventilación mecánica invasiva.

    Abstract Neste artigo são discutidas as bases da política econômica destinada ao combate da Covid-19. De um modo geral, notou-se que a ideologia econômica neoliberal determinou os rumos das ações econômicas do governo atual. Após analisar um conjunto de medidas de apoio aos empresários, trabalhadores e segmentos sociais vulneráveis, concluiu-se que a política econômica fracassou e, consequentemente, estimulou o aumento do número de empresas que encerraram suas atividades, bem como contribuiu para elevar fortemente o nível de desemprego que, registre-se, já era elevado anteriormente ao início da pandemia.

    Abstract Objective: To investigate the function of a novel primate-specific long non-coding RNA (lncRNA), named FLANC, based on its genomic location (co-localised with a pyknon motif), and to characterise its potential as a biomarker and therapeutic target.
    Design: FLANC expression was analysed in 349 tumours from four cohorts and correlated to clinical data. In a series of multiple in vitro and in vivo models and molecular analyses, we characterised the fundamental biological roles of this lncRNA. We further explored the therapeutic potential of targeting FLANC in a mouse model of colorectal cancer (CRC) metastases.
    Results: FLANC, a primate-specific lncRNA feebly expressed in normal colon cells, was significantly upregulated in cancer cells compared with normal colon samples in two independent cohorts. High levels of FLANC were associated with poor survival in two additional independent CRC patient cohorts. Both in vitro and in vivo experiments demonstrated that the modulation of FLANC expression influenced cellular growth, apoptosis, migration, angiogenesis and metastases formation ability of CRC cells. In vivo pharmacological targeting of FLANC by administration of 1,2-dioleoyl-sn-glycero-3-phosphatidylcholine nanoparticles loaded with a specific small interfering RNA, induced significant decrease in metastases, without evident tissue toxicity or pro-inflammatory effects. Mechanistically, FLANC upregulated and prolonged the half-life of phosphorylated STAT3, inducing the overexpression of VEGFA, a key regulator of angiogenesis.
    Conclusions: Based on our findings, we discovered, FLANC as a novel primate-specific lncRNA that is highly upregulated in CRC cells and regulates metastases formation. Targeting primate-specific transcripts such as FLANC may represent a novel and low toxic therapeutic strategy for the treatment of patients.

    Abstract Rates of primary and secondary syphilis have increased in the past decade, warranting renewed attention to the diagnosis and treatment of this disease. Men who have sex with men are particularly affected; however, increases in infection rates have also been noted in women, as well as in all age groups and ethnicities. Physicians need to vigilantly screen high-risk patients. The concurrent rise in congenital syphilis also requires special attention and reemphasizes the need for continued early prenatal care and syphilis screening for all pregnant women. Syphilis infection in patients coinfected with human immunodeficiency virus has also become more common. New experimental diagnostic approaches, including using the B cell chemoattractant chemokine (CXC motif) ligand 13 as a cerebrospinal fluid marker, may help identify suspected neurosyphilis cases. Additionally, point-of-care immunochromatographic strip testing has been suggested for screening high-risk populations in developing countries. Nontreponemal screening tests followed by treponemal confirmatory tests continue to be standard diagnostics; however, interpreting false-negative and false-positive test results, and identifying serofast reactions, can be challenging. Although doxycycline, tetracycline, ceftriaxone, and azithromycin have been used to successfully treat syphilis, penicillin remains the drug of choice in all stages of infection and is the therapy recommended by the Centers for Disease Control and Prevention. Close follow-up is necessary to ensure treatment success.

    Abstract O artigo discute a reforma agrária no Brasil desde a década de 1980, realçando as contradições envolvidas neste processo. Inicialmente, faz-se uma breve síntese das políticas adotadas pelos diferentes governos entre 1975 e 2010. Em seguida, são analisadas diversas informações relativas à quantidade total de famílias assentadas no país, bem como à quantidade de assentados em cada período específico. A principal conclusão do estudo é que a política agrária brasileira, além de assentar pouca gente em um período tão longo, não tem provocado alterações importantes no processo de concentração de terras no Brasil, uma vez que o Índice de Gini ainda continua bastante elevado.

    Abstract En este artículo pretendemos establecer un estado de la cuestión sobre la evolución de las principales ciudades, villas y otros espacios menores que flanquean Granada, comprendidos entre Guadix y Loja. Para ello hemos hecho uso tanto de las fuentes escritas como de los datos que la Arqueología nos ha suministrado hasta el momento. Y hemos podido constatar cómo los principales espacios siguen una evolución muy similar pasando de husún con albacar a finales del siglo IX a mudun ya en el siglo XI, mientras que el resto de núcleos menores se verán muy determinados por su ubicación en un espacio fronterizo castellano-nazarí. The aim of this study is to present the state of the question on the evolution of the main towns, villages and other minor settlements that surround Granada from Guadix to Loja. We have made use of written records and archaeological evidence found to date. We are able to confirm that the main settlements follow a common evolution from husun (fortresses) with an albacar (refuge) at the end of the ninth century to that of mudun (towns) by the eleventh century, whereas smaller settlements are conditioned by their location on the Castilian-Nasrid frontier.

    Abstract Although nonlinear dynamics have been mastered by physicists and mathematicians for a long time (as most physical systems are inherently nonlinear in nature), the recent successful application of nonlinear methods to modeling and predicting several evolutionary, ecological, physiological, and biochemical processes has generated great interest and enthusiasm among researchers in computational neuroscience and cognitive psychology. Additionally, in the last years it has been demonstrated that nonlinear analysis can be successfully used to model not only basic cellular and molecular data but also complex cognitive processes and behavioral interactions. The theoretical features of nonlinear systems (such unstable periodic orbits, period-doubling bifurcations and phase space dynamics) have already been successfully applied by several research groups to analyze the behavior of a variety of neuronal and cognitive processes.-

    Additionally the concept of strange attractors has lead to a new understanding of information processing which considers higher cognitive functions (such as language, attention, memory and decision making) as complex systems emerging from the dynamic interaction between parallel streams of information flowing between highly interconnected neuronal clusters organized in a widely distributed circuit and modulated by key central nodes. Furthermore, the paradigm of self-organization derived from the nonlinear dynamics theory has offered an interesting account of the phenomenon of emergence of new complex cognitive structures from random and non-deterministic patterns, similarly to what has been previously observed in nonlinear studies of fluid dynamics.-

    Finally, the challenges of coupling massive amount of data related to brain function generated from new research fields in experimental neuroscience (such as magnetoencephalography, optogenetics and single-cell intra-operative recordings of neuronal activity) have generated the necessity of new research strategies which incorporate complex pattern analysis as an important feature of their algorithms. Up to now nonlinear dynamics has already been successfully employed to model both basic single and multiple neurons activity (such as single-cell firing patterns, neural networks synchronization, autonomic activity, electroencephalographic measurements, and noise modulation in the cerebellum), as well as higher cognitive functions and complex psychiatric disorders. Similarly, previous experimental studies have suggested that several cognitive functions can be successfully modeled with basis on the transient activity of large-scale brain networks in the presence of noise.-

    Abstract Background: Candidiasis is one of the most important among recurrent invasive yeast infections in patients, thus antifungal treatment becomes a challenge.
    Aims: The aim of this study was to evaluate the in vitro activity of clinical Candida albicans isolates from blood cultures to fluconazole, amphotericin B and anidulafungin, in a hospital from Rio Grande do Sul, Brazil.
    Methods: The susceptibility of 153 isolates to the 3 drugs mentioned was tested according to Clinical and Laboratory Standars Institute. Minimal inhibitory and fungicidal concentrations (MIC, MFC, respectively) of each drug were determined, as well as the epidemiological cutoff value (ECV).
    Results: All of the isolates were susceptible to anidulafungin, MIC and MFC ≤ 1 μg/ml; however, when compared with ECV, 3% of the isolates exhibited higher values against fluconazole, 96% were susceptible, 3% susceptible dose-dependent, and 1% resistant. Also, it was observed that 21% of the isolates exhibited higher values than ECV. One isolate was resistant to amphotericin B; the other ones, susceptible, based on the MFC; furthermore, 1.5% of the isolates exhibited higher values.
    Conclusions: C. albicans isolates exhibited more susceptibility to anidulafungin, and 90% of them (MIC90) exhibited the lowest values against amphotericin B. Based on ECV and Pfaller classification, isolates could be resistant to fluconazole, demonstrating the importance of the combination of these parameters.
