    Abstract Proper understanding of the optical function of the eye is the foundation of clinical understanding of ophthalmologists. Although teaching principals of optics has always been a part of ophthalmology residency curriculum, it seems that successful strategies other than lecture-based methods are needed to engage students and facilitate the understanding of optical principals. A collaborative team of physicists (optics Ph.D.), ophthalmologists and medical education experts designed an optics lab for ophthalmology residents to help them practically simulate different optical phenomena. The educational course consisted of 4 sessions of 90 min to practice the optical tests using the lab instruments. Each class consisted of 6-9 residents, divided into 3 groups with a fully equipped unit, and two mentors (an optics Ph.D. professor and an ophthalmology professor). A quasi-experimental design with pre-post test was used to evaluate the effectiveness of the training workshop in changing the ophthalmology residents' optical knowledge and attitude. Thirty-five residents participated in the study. The average score of residents' performance before the workshop was 5.21 (out of 100), which increased significantly to 66.1 after the workshop. Also, the average knowledge of residents, which was measured as self-reported, increased significantly from 28.85 to 71.09. The average score of students' attitudes and interest was increased from 40.49 to 74.81. It seems that training workshops and labs are effective to bring about change in knowledge and attitude of ophthalmology residents toward optics as a new teaching strategy that would be implemented in their curriculum.

    Abstract Purpose: To evaluate the visual, refractive, and topographic outcomes of accelerated corneal collagen cross-linking (CXL) in the pediatric age group.
    Methods: In this retrospective case series study, 89 eyes of 56 patients with progressive keratoconus (KCN) who were under or equal to 18 years old at the time of surgery were included. All patients underwent accelerated corneal CXL. A thorough baseline and follow-up ophthalmic examination including uncorrected distance visual acuity, best corrected visual acuity (BCVA), slit-lamp and fundus examination, and corneal tomography by Scheimpflg camera Pentacam (Oculus, Wetzlar, Germany) were performed.
    Results: The mean age of patients was 16.2 ± 1.8 years. Mean follow-up was 16.46 ± 11.6 months (range, 6-40 months). The mean BCVA improved significantly from 0.26 ± 0.26 to 0.16 ± 0.19 (logMAR) after accelerated CXL (
    Conclusion: Our study shows that accelerated CXL increases visual acuity and stabilizes or improves keratometric indices in pediatric patients with progressive KCN without any serious complication for a mean follow-up time of 16 months.

    Abstract Background: In recent years, the mare's milk has been introduced as a rich source of nutrients with hypoallergic characteristics which is widely used for Iranian infants.
    Objectives: The present study aimed to investigate the heavy metal concentration of mare's milk and its consumption risk assessment.
    Methods: About 88 mare's milk was collected from Yazd, the centre of Iran, during the summer of 2020. The raw mare's milk was digested and analysed for mineral and heavy metal content (As, Ca, Cd, Co, Cu, Fe, Mg, Mn, Ni, P, Pb and Zn) by ICP-OES. To estimate the health hazard for consumers the Estimated Daily Intake (EDI), Hazard Quotient (HQ) and Hazard Index (HI) of heavy metals were determined.
    Results: The Ca ranged from 260.52 to 201.43 mg/L, which was the highest mineral in mare's milk followed by P and Mg. By increasing the age, P and Ca content was increased. The obtained ranges of Cu, Co, Fe, Mn and Zn were 72.12-75.11, 1.12-9.3, 180.69-230.21, 31.24-47.13 and 1060-1200 μg/L, respectively. The Cd and Arsenic content of mares' 8-11 years of age had higher concentrations. The highest Pb content was reported in mares 4-7 years old (10 μg/L). Although, Pb, Cd and As content of the mare's milk was evaluated lower than the permissible limit. Also, the HQ value was As > Cd > Pb > Zn > Ni > Cu for infants, toddlers and adults. The HI of mare's milk was 0.16, 0.15 and 0.022 for infants, toddlers and adults, respectively.
    Conclusions: Mare's milk could be an effective nutrition source for infants and children suffering from milk protein allergies.

    Abstract Purpose: To evaluate the corneal topographic patterns in an adult Iranian population and investigate its correlation with the refractive status of the eye.
    Methods: In a cross-sectional study named "Tehran Study," 1023 samples were selected by the cluster sampling method from the downtown area of Tehran. Eight hundred and forty-nine adults aged over 15 years participated. All selected participants were refracted and underwent topography imaging.
    Results: The patients' ages ranged from 15 to 91 years with a mean of 40.33 ± 16 years. The most frequent topographic patterns were symmetric bowtie (SB) (34%), SB with inferior steepening (SB-IS) (14.1%), and round (10.5%). The orders changed in categorization by refractive status: The most frequent pattern in all subgroups (emmetropia, myopia, and hyperopia) was SB with frequencies 32.7%, 35.8%, and 22.5%, respectively. Although the second order was asymmetric bowtie (AB) with AB-IS in the emmetropic and myopic subgroups, in the hyperopic subgroup, round pattern had the second place. The third place was different in all groups. The rarest patterns in the whole were SB with skewed radial axis (SRAX) and AB with SRAX. The first prevalent topographic pattern was SB in all age groups and in both genders. The prevalence of round pattern, irregular pattern, and SRAX significantly increased in older ages, and the prevalence of SB decreased in older ages. The first observed prevalent pattern was SB in both sexes, but the second most prevalent pattern was AB-IS and round in females and males, respectively.
    Conclusions: Corneal topographic pattern might be related to the refractive status of the eye. The information about normal topographic patterns provides a reference for comparison with diseased corneas.

    Abstract Background and purpose: Sequential organ failure assessment (SOFA) is used to assess the severity of the disease and mortality rate in patients admitted to ICU. SOFA requires experiments that are impractical in centers with limited resources. The aim of this study was to compare the diagnostic value of bedside SOFA (b SOFA) and SOFA scores in mortality of ICU patients. Materials and methods: This cohort study was performed in 60 patients with Systemic Inflammatory Response Syndrome (SIRS) older than 18 years of age. SOFA and b SOFA scores were recorded at admission, then daily until admission at ICU. Results: According to the Lin and Bland-Altman agreement coefficient, there was a very good agreement between SOFA and b SOFA scores at days 1, 15, 16, 17, 18, and 20. At days 3, 5, 7, 14, and 19 we observed a good agreement and significant correlation between SOFA and b SOFA scores. Findings showed a moderate agreement between SOFA and b SOFA scores at days 2, 4, 6, 8, 9, 10, 11, 12, and 13, but at days 13 and 15 there was a direct relationship between the two scores (P>0.05). Area Under Curve (AUC) method in ROC Curve showed no significant difference in diagnostic value between SOFA and b SOFA scores except at day 17 (P>0.05), indicating acceptable diagnostic value of b SOFA score compared to SOFA score. The sensitivity and specificity of the tools in terms of prognosis were found to be very close at different times showing similar diagnostic values. Conclusion: The study showed diagnostic accuracy, and high sensitivity and specificity for predicting mortality and strong positive correlation between the two tools, so b SOFA is a suitable alternative due to lower cost with greater ease.

    Abstract Background: Designing educational interventions tailored to the needs of Contact Lens Wearers (CLWs) are important and necessary. The present study aimed to determine the information needs of CLWs to educate and provide information to them to increase their knowledge and reduce complications and non-compliance behaviors.
    Methods: A qualitative approach was applied and semi-structured interviews were conducted in three contact lenses (CL) clinics in Iran among all their practitioners and 24 purposively selected patients. Data were analyzed using the Lundman and Graneheim conventional content analysis.
    Results: The qualitative analysis revealed knowledge and skill themes as two main categories. The knowledge category includes five subcategories of basic information for the CLWs, acquaintance with the CL, caring for CL, hygiene and vigilance of CL, and challenges of using CL. The skill category consists of two subcategories, including handling/insertion and removal of the CL, and stabilization of learned information. Moreover, 36 sub-subcategories emerged from these seven subcategories that reflected the information needs of CLWs.
    Conclusions: A clear understanding of CLWs' information needs can help to design and develop appropriate educational approaches to overcome training barriers such as physicians' time constraints and high workload. Moreover, it can help deal with CLWs' insufficient knowledge and provide the required information simply and practically with the possibility of enough repetition.
