    Abstract Background: Complete heart blocks underwent to permanent pacemaker placement are a common complication of tricuspid valve replacement (TVR). If indicated, endocardial placement of a right ventricular (RV) lead is precluded in the presence of mechanical TVR.
    Case presentation: A 20-year-old female patient firstly underwent metallic prosthetic valve operation with tricuspid valve endocarditis in 2014. Three years after the operation, echocardiography revealed dysfunction of the prosthetic valve thus reoperation was decided. In the second operation, the patient underwent a bioprosthesis valve and AV complete block developed in the postoperative period. Left ventricular ejection fraction (EF) was 45% was found on echocardiography. Pacemaker dependence of the patient, it was aimed to place two electrodes into the left ventricle. Electrodes were placed the target two branches in coronary sinus (CS) and right atrium. Univentricular bifocal pacing was enabled to work.
    Conclusion: Electrode placement in the CS is a very good alternative to epicardial surgical lead placement in cases where endocardial lead placement from the right atrium to the RV is contraindicated. In patients with lower left ventricular EF who will be pacemaker dependent, the insertion of two electrodes into the CS to prevent pacemaker is a safe and effective treatment.

    Abstract Wolff-Parkinson-White (WPW) syndrome causes paroxysmal supraventricular tachycardia in which short PR intervals and delta waves are seen in electrocardiography, which may cause sudden cardiac death. A 19-year female presented with increasing episodes of wide and narrow QRS complex tachycardia for the past 5 years. She had tricuspid atresia and Fontan Bjork operation in her past history. She was then diagnosed with narrow QRS complex tachycardia; and WPW syndrome was discovered when she returned to sinus rhythm. Ablation was performed from the coronary sinus ostium region via the left subclavian vein. Fontan Bjork procedure leading to accessory connections stemming from the surgery in the atrio-infundibular anastomosis may be one reason for WPW syndrome. In this case, since the ablation area was close to the Fontan anastomotic line, it could not be determined clearly whether WPW syndrome was secondary to Fontan anastomosis or it was congenital occult WPW syndrome, which became overt following Fontan surgery.

    Abstract Background: To investigate the effect blood flow restriction (BFR) exercises on muscle size, strength and athletic performance in elite canoe athletes aged 18 to 25 years.
    Methods: This was a randomized controlled trial. The participants were divided into 2 groups: the intervention group (INT-gr) (n = 17, age: 18.59 ± 0.71 years) and the control group (CONT-gr) (n = 16, age: 18.81 ± 1.11 years). Anthropometric measurements, muscle size measured by ultrasound (US), strength measurements with an isokinetic dynamometer, and ergometer performance with an indoor ergometer were conducted before and after the exercise program. Knee flexion and extension and leg press one-repetition maximum (1 RM) tests were performed to determine the participants' training program. The INT-gr performed 1 RM 30% resistance training + BFR for 8 weeks, while the CONT-gr performed 1 RM 30% resistance training (RT) without BFR with their routine training program. US was used to measure the cross sectional area (CSA) and thickness of the quadriceps femoris (QF) and Hamstring (H) muscles in the pre-post design, and the isokinetic dynamometer was used to measure the strength of bilateral 60˚/s and 300˚/s peak torque (PT) values of the QF and H. Sports performance was tested on an indoor ergometer at distances of 200, 500, and 1000 m.
    Results: The changes in bilateral rectus femoris (RF) CSA and VL thickness measurements in the INT-gr were significant (P < .05). Ergometer performance measurements showed a significant improvement over CONT-gr at all distances (P < .05). In terms of strength scores measured by the isokinetic dynamometer, the right QF and H 300˚/s and the left QF 60˚/s PT values were significantly in favor of INT-gr.
    Conclusion: BFR exercises are effective to increase strength, muscle size, and ergometer performance in elite canoe athletes.

    Abstract Background: Triage refers to classifying and prioritizing patients based on the severity of their injuries or illnesses in the health care setting. The increasing number of elderly patients seeking care in emergency departments (EDs) highlights the need for special attention to the unique needs of this patient population.
    Aim: We aimed to compare the qSOFA, Emergency Severity Index (ESI), National Early Warning Score (NEWS), and Manchester Triage System (MTS) scores to assist ED physicians in assessing the severity of elderly patients' clinical conditions and triaging them appropriately.
    Methods: This cross-sectional study included 1066 patients aged 65 and over who presented to our ED as outpatients or by ambulance between September 1, 2022, and August 30, 2023. Scoring systems at the time of admission to the ED were recorded separately for outpatients and arriving by ambulance.
    Results: According to the qSOFA, patients with a score of 0 were 0.976 times less likely to arrive by ambulance compared to those scoring 1 and above (OR = 0.976, p = .934). According to the NEWS, patients in the moderate-risk category were 0.447 times less likely to arrive by ambulance (OR = 0.447, p = .054). According to the ESI score, patients requiring high resource use with normal vital signs were 146.758 times more likely to arrive by ambulance (OR = 146.758, p = .001).
    Conclusion: Significant differences in patients' methods of presentation to the ED were observed based on the MTS, qSOFA, NEWS, and ESI scores.

    Abstract Objective: In this study we aimed to determine the prediction level of admission diastolic blood pressure (DBP) on the prognosis and mortality in aortic dissection patients over 65 years old and under 65 years old.
    Methods: We included 72 patients in this retrospective study and study groups were divided into two groups according to 65 age. Demographic data, dissection type (Stanford A-B), DBP, systolic blood pressure (SBP), mean arteriel pressure (MAP), heart rate (/min) main complaints, preoperative length of stay, hospitalisation clinic (clinic/intensive care unit), length of hospitaliisation, complications during hospitalisation (renal failure etc..) and the outcome (death/dischargement) results were noted. Preoperative lenth of stay, hospitalisation length, outcome and complications were compared between groups according to SBP, DBP, MAP and heart rate.
    Results: Mean blood pressure values of the Stanford type B patients over 65 years old were higher than the other group (p<0.05). Fifty percent of patients under 65 years old were discharged but this ratio was 26.9% in the elder group. DBP was positively correlated with preoperative length of stay and hospitalisation length and negatively correlated with mortality. DBP under 65 mmHg was significantly related with high mortality (p<0.05). When the age and presentation time heart rate is added to each other, the values over 142 were significantly related with high mortality (p<0.05).
    Conclusions: The presentation time vital signs especially the DBP may be helpful for emergency clinicians to predict the prognosis and outcome in aortic dissection patients which has high mortality ratio in patients over 65 years of age.

    Abstract Purpose: Opiorphin is an endogenous inhibitor of enkephalin-degrading enzymes. It has a strong analgesic effect in chemical and mechanical pain models. We aimed to evaluate the tear opiorphin levels in ocular pain caused by corneal foreign bodies and demonstrate whether there is any correlation with pain levels obtained from the Visual Analog Scale (VAS) score and tear opiorphin level.
    Methods: Thirty-two healthy individuals and 34 individuals diagnosed with corneal foreign bodies were included in this study. Tear opiorphin levels were measured by the ELISA method using a commercially available kit. The difference in tear opiorphin levels between the patient and control groups were evaluated using the Mann-Whitney U test. The correlation between VAS scores and tear opiorphin levels was evaluated using the Spearman rank correlation coefficient.
    Results: The median values of tear opiorphin levels of the patient and control groups were 134 pg/mL (86.86-296.25) and 109.80 pg/mL (66.15-191.49), respectively. The Mann-Whitney U test showed a statistically significant difference in tear opiorphin levels between patient and control groups (P < 0.05). No ocular pain was reported in the control group. The median VAS score of the patient group was 6 points (1-9). No correlation was found between VAS scores and tear opiorphin levels in the patient group.
    Conclusions: The cornea is the most densely innervated tissue, and the highest opiorphin concentrations have been observed in tear. It is, therefore, expected that the stimulation or damage to the nerve endings in cornea would cause an increase in opiorphin secretion as a pain relief mechanism.

    Abstract Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) or Bland-White-Garland syndrome is a rarely seen congenital anomaly. Adult and infantile types are defined according to the degree of collateral development between the left coronary artery (LCA) and right coronary artery (RCA). If left untreated, ALCAPA has a 90% mortality rate in the first year of life, primarily due to myocardial ischaemia and heart failure. The degree of collateral development and the related LCA perfusion in ALCAPA syndrome determine the occurrence of symptoms. Herein, we present a case of a female patient who had previously, without any symptoms, given live birth to 13 babies. She had been experiencing exertional angina, which started long after the delivery of her 13th child. Since our patient had well-developed collaterals to the LCA, she was asymptomatic and able to give birth to the children via the vaginal route without any problems. Having well-formed collateral vessels between the RCA and LCA may prevent patients from developing symptoms, and even stressful conditions such as pregnancy may be tolerable.

    Abstract Aim: The increased number of emergency clinic patients causes the length of stay in the emergency department, low patient satisfaction and dismiss of real emergency patients. In this study, we aimed to determine the prediction levels of emergency clinicians according to working year on the outcome of the ambulance patients and outpatients presented to the emergency department (ED).
    Materials & methods: This prospective study included patients over 18 years old. The triage of outpatients was made by a senior nurse and patients were divided into three triage categories such as green, yellow and red. Then these patients were evaluated by the emergency physician at the examination areas. Ambulance patients were directly evaluated by the emergency physician. These ambulance patients were noted as yellow or red according to triage categories. The main complaints, triage category, presentation method, vital signs, predicted outcome noted by the clinicians.
    Results: The correct prediction levels of hospitalisation (clinic/intensive care unit) were higher in clinicians whose working year is between 6 and 10 years (p < 0.05). There was no significant difference between 6-10 year and >10 year group according to prediction level (p > 0.05). Prediction of dischargement was higher in 0-5 year group than 6-10 year (p < 0.05) and >10 year (p < 0.05) group.
    Conclusion: Experienced clinicians can make much more accurate prediction on length of stay and the prognosis of the emergency patients so crowded follow-up areas of the emergency room can be planned much more effectively.

    Abstract Background: The rapid spread of the new severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) which causes the coronavirus disease 2019 (COVID-19) facilitated the emergence of numerous variants. The present study aimed to investigate the prevalence and change of important "S" protein variants such as N501Y, DelH69/ 70, and E484K in SARS CoV-2 PCR positive patients diagnosed with COVID-19 who have referred to a private hospital within the period that mutations were detected during COVID-19 pandemic.
    Methods: One hundred and eighty-seven patients who have been referred due to the suspicion of COVID-19 between December 2020 and April 2021 and in whom SARS-CoV-2 was detected positive in the PCR test were enrolled into the study. These patients were randomly selected among 285 patients detected in these months, among those with the most accurate graphics and data. The RNA material extracted from the nasopharyngeal swab samples taken from the patients was analyzed and specifically N501Y, del69-70, and E484K mutations were investigated through the Real-Time PCR method.
    Results: The mean age of the patients was 37.5 ± 14.1 years. Mutations were detected in 84 (44.9%) samples in total (N501Y + DelH69/70 by 81%, DelH69/70 by 7.1%, E484K by 7.1% and N501Y + E484K by 4.8%). There was no sample detected with the N501Y mutation. The mutation rate between December - February was detected between 1% and 8%, and the mutation rate increased to 39% to 44% in April and March. While DelH69/70 mutation was detected in December 2020 only, it was observed that N501Y + DelH69/70 variants became dominant as of February 2021, and E484K and N501Y + E484K variants started to appear in March and April. It was observed that the variant rates included DelH69/70 (p < 0.001), N501Y (p < 0.001), N501Y + DelH69/70 (p < 0.001), and N501Y + E484K (p = 0.01) mutations increased significantly according to the months. The E484K mu-tation was significantly higher in males (p = 0.037). There was no differences between mutation rates between the age groups.
    Conclusions: Our findings indicate that the appearance of important SARS-CoV-2 variants gradually increases, that rates of mutation increase up to 40% within several months, that the N501Y + DelH69/70 variant gradually becomes to be dominant, and that different variations appear along with mutations.

Implant displacement into the maxillofacial spaces during surgery or after a period of use is a complication recognized in the literature. Foreign objects such as titanium dental implants in the maxillofacial region must be removed because they cause infection due to tissue reaction. In this report, the authors present the case of a patient whose dental implant slipped into the submental space after surgical integration and describe the surgical removal of the implant via an extraoral approach.
