    Title translation Oral anticancer drug: Feedback from six years of tripartite consultations in a hospital center.
    Abstract Introduction: Tripartite consultations with a coordination between hospital and community care givers were implemented within hospital center for patients who start an oral anticancer regimen.
    Methods: Six years after the implementation, we wanted to assess this patient's pathway and describe how adjustments were necessary over the time.
    Results: A total of 961 patients received tripartite consultations. The medication review process revealed that nearly half of patients had polypharmacy (≥5 drugs/day). A pharmaceutical intervention was formulated in 45 % of cases and they were all accepted. For 33 % of patients, a drug interaction was identified and required for 21 % of them, discontinuation of one drug of their personal treatment. Coordination with general practitioner and community pharmacists were achieved for all patients. 390 patients benefitted from nursing telephone follow-up which represents approximately 20 calls per day to assess tolerance and compliance with treatments. Over time, organisational adjustments were necessary to adapt to the increase in activity. The scheduling of consultations has been improved thanks to the creation of a shared agenda and consultation reports have been expanded. Finally, an hospital functional unit was created to facilitate the financial valuation of this activity.
    Discussion: The feedback collected from the teams showed a real desire to perpetuate this activity even if it would seem that an improvement in human resources is still relevant as well as a better optimisation of the coordination between all the participants.

    Abstract Introduction: Drug-drug interactions with cyclin-dependent kinases inhibitors 4 and 6 (CDK4/6) are known and should be taken into account.
    Case report: A 68-year-old woman, on prior Simvastatin therapy, developed severe rhabdomyolysis after three weeks of Ribociclib initiation. She showed general weakness with mobility problems and was admitted to our hospital.
    Management and outcome: Ribociclib and Simvastatin were discontinued and the patient received intensive intravenous hydration. She finally recovered her mobility after two weeks.
    Discussion: We hypothesize that Simvastatin induced rhabdomyolysis by possible interaction with Ribociclib. Ribociclib is a strong inhibitor of CYP 3A4 and a potential inhibitor of OATP1B1 membrane transporter. Simvastatin plasma concentration may reach toxic levels due to Ribociclib inhibition. To assess the relevance of our hypothesis, we used the Drug Interaction Scale. With a total score of 7, the interaction is considered as "probable." Because of the high risk of severe rhabdomyolysis, the concomitant use of Simvastatin with Ribociclib should be avoided or otherwise careful monitoring of creatine kinase is warranted.

    Title translation Cochlear implant rehabilitation in older adults – self-assessments of listening success and challenges
    Abstract Hintergrund: In den letzten Jahren stieg die Anzahl der CI-Versorgungen im höheren Alter an und in der Forschung wurde dieser Zielgruppe mehr Aufmerksamkeit geschenkt. Aufgrund bisheriger Forschungsergebnisse lässt sich annehmen, dass ältere Menschen durch ein CI in vielerlei Hinsicht deutlich profitieren können. Allerdings wurde der subjektiven Wahrnehmung von Erfolgen und Herausforderungen der Patienten im Rahmen der Basis- und Folgetherapie bisher wenig Beachtung geschenkt.
    Methode: In der vorliegenden Arbeit wurden Patientendokumentationen einer Stichprobe von denjenigen n=22 Personen der Kölner Uniklinik aus den Jahren 2017–2019 mittels qualitativer Inhaltsanalyse retrospektiv untersucht, die ab dem 70. Lebensjahr einseitig mit einem CI versorgt wurden und auf der kontralateralen Seite ein Hörgerät nutzten. Ausgewertet wurden sowohl die audiometrischen Daten aus der Ton- und Sprachaudiometrie als auch persönliche Angaben zum subjektiv erlebten Hörerfolg und Handling der Hörtechnik.
    Ergebnisse: Aus den Ergebnissen geht hervor, dass das postoperative Sprachverstehen gemessen mit dem OLSA in Ruhe sowohl in der Gesamtgruppe als auch intraindividuell besser war als das präoperative Ergebnis. Mindestens die Hälfte der Personen waren mit ihrem CI teilweise oder vollständig zufrieden. Der überwiegende Anteil der Personen trug den Sprachprozessor während des gesamten Tages. Das Sprachverstehen in Gruppen, im Störschall, beim Telefonieren, beim Fernsehen und das Musikhören stellte die meisten Personen laut eigener Einschätzung jedoch vor größere Herausforderungen. Auch im Umgang mit dem technischen Zubehör zeigten einige Personen Unterstützungsbedarf. Ein umfassendes Bild über die sozialen Kontakte der befragten Personen mit CI ließ sich aufgrund der vorliegenden Daten nicht abbilden. Es deutete sich jedoch an, dass die Entwicklung des Sprachverstehens mit CI von alleinlebenden Personen als erschwert wahrgenommen wurde.
    Fazit: Die postoperativen Schwierigkeiten in herausfordernden kommunikativen Situationen im Alltag sowie bei der Mediennutzung und der Technikhandhabung, die in der untersuchten Stichprobe über 69 Jahre offensichtlich wurden, sollten in einer Beratung bezüglich einer CI-Versorgung und in der anschließenden Hörrehabilitation beachtet und mit den Patienten thematisiert werden, um die prä- und postoperativen Prozesse zu optimieren. ; Background: The recent increase in the number of cochlear implantations in older people has sparked new interest in researching this target population. Findings in previous studies suggest that a cochlear implant (CI) brings considerable benefits for older people. However, limited attention has been given to the views of CI recipients themselves, and how they perceive success and challenges in the time following implantation.
    Method: Records of n=22 former patients at Cologne University Hospital were examined using qualitative content analysis. All patients were fitted with a CI in one ear between 2017 and 2019 when they were 69 years or older. They all used a hearing aid for auditive support on the other ear. Audiometric data from pure tone audiometry and speech audiometry was collected and analyzed along with personal comments on how individuals perceived the success and how they were able to handle the hearing technology.
    Results: Findings suggest that individuals’ speech comprehension in the OLSA in quiet improved considerably after implantation and therapy. This was observed both in the collective group and in intraindividual comparison. Half of the group stated that they were partly satisfied or completely satisfied with their CI. Most of the patients reported that they kept the sound processor on throughout the entire day. However, according to their self-reports, most participants felt challenged in their understanding of speech in groups, with background noise, when talking on the phone, watching TV, and/or when listening to music. Some of the participants reported that they required support with handling the technical equipment. It was not possible to depict a comprehensive picture of the participants’ social contacts based on the collected data. However, our findings suggest that developing an understanding of speech after cochlear implantation seems to be perceived as more difficult for persons living on their own.
    Conclusion: CI patients over 69 years reported difficulties in everyday life, particularly in challenging communicative situations, in media use and in using technical accessories. These difficulties should be taken into consideration during preoperative counselling regarding cochlear implantation and also during the rehabilitation process that follows implantation. More importantly, these difficulties should be directly addressed with patients to optimize the pre- and postoperative processes involved.

    Abstract Lurasidone (DS-RAn) has demonstrated efficacy in the acute treatment of bipolar depression, both as monotherapy, and as combination therapy with lithium or valproate. To evaluate the recurrence prevention efficacy of lurasidone for the maintenance treatment of bipolar I disorder, patients received up to 20 weeks of open-label lurasidone (20-80mg/d) combined with lithium or valproate during an initial stabilization phase. A total of 496 patients met stabilization criteria and were randomized to 28 weeks of double-blind treatment with lurasidone (20-80mg/d) or placebo, in combination with lithium or valproate. Based on a Cox proportional hazard model, treatment with lurasidone reduced the probability of recurrence of any mood episode by 29% (primary endpoint), however, the reduction did not achieve statistical significance. Probability of recurrence on lurasidone was significantly lower in patients with an index episode of depression (HR, 0.57; P=0.039), in patients with any index episode who were not rapid-cycling (HR, 0.69; P=0.046), and when recurrence was based on MADRS, YMRS, or CGI-BP-S severity criteria (HR, 0.53; P=0.025; sensitivity analysis). Long-term treatment with lurasidone combined with lithium or valproate was found to be safe and well-tolerated, with minimal effects on weight or metabolic parameters.
