    Abstract This retrospective case series assessed the early effectiveness of combined spontaneous portosystemic shunt (SPSS) embolization and preemptive transjugular intrahepatic portosystemic shunt (TIPS) creation for alleviation of medically refractory hepatic encephalopathy (HE) and prevention of portal hypertension complications in patients with liver cirrhosis. Eight patients with liver cirrhosis (5 men and 3 women; mean age, 61 years [SD ± 10]) and HE (overt [West-Haven Grade 2-4], n = 7; covert [West-Haven Grade 1], n = 1) refractory to lactulose and rifaximin therapy who underwent concurrent or staged SPSS embolization and TIPS creation between 2018 and 2022 were included in this study. The primary outcomes were 3-month improvement in HE and postprocedural HE-related hospitalizations. HE improvement was achieved in 7 (87.5%) of 8 cases. Among all patients, there was 1 HE-related hospitalization within 90 days that responded to repeat embolization with no further admissions. No patients developed new ascites, variceal hemorrhage, or other portal hypertension complications within 3 months.

    Abstract This series presents a hybrid technique for obliteration of gastric varices (GVs) termed retrograde-antegrade accelerated trap obliteration that employs sclerosant agent instillation under concurrent inflow and outflow vessel occlusion with coils or plugs. Six patients (mean age, 56 y) with GVs were treated in 2014 and 2015. Technical success rate was 100%. Five patients completed 30-day follow-up. There were no procedure-related complications, and clinical success rate was 100%, with no bleeding recurrence over a mean follow-up of 298 days ± 178. GV obliteration rate was 100% (n = 4) at a mean of 157 days ± 158. This limited experience suggests that the described technique represents a viable approach to GV obliteration.

    Abstract Variceal hemorrhage is a life-threatening sequela of liver cirrhosis that requires a careful and comprehensive approach to management. Transjugular intrahepatic portosystemic shunt creation with or without variceal embolization or obliteration represents a minimally invasive image-guided intervention used for the management of varices. This review focuses on the role of transjugular intrahepatic portosystemic shunt and embolization or obliteration in the setting of variceal hemorrhage, with an emphasis on the useful aspects of patient evaluation and selection, practical approaches to procedure planning and valuable elements of interventional technique, and clinical outcomes as they pertain to portal venous decompression and variceal embolotherapy.
