    Abstract Cancer care has been greatly impacted during the COVID-19 pandemic. The number of cases and deaths caused by the COVID-19 pandemic continues to escalate throughout the United States and the world. Worldwide, over 150 million people have been diagnosed with the coronavirus and more than 3 million have died. Now that we have gained additional experience with COVID-19, we are starting to learn its full impact on oncology care and its effects on the practice of medicine and clinical research.

    Abstract In recent years, bioethical discourse around the topic of 'genetic enhancement' has become increasingly politicized. We fear there is too much focus on the semantic question of whether we should call particular practices and emerging bio-technologies such as CRISPR 'eugenics', rather than the more important question of how we should view them from the perspective of ethics and policy. Here, we address the question of whether 'eugenics' can be defended and how proponents and critics of enhancement should engage with each other.

    Abstract Objectives: Our study aimed to determine the publication rates of podium presentations from the 2017 and 2018 Society of Gynecologic Oncology (SGO) Annual Meetings; and to examine rates and predictors of oral presentations that resulted in publication.
    Methods: We reviewed podium presentations given at the 2017 and 2018 SGO Annual Meetings. Abstracts were evaluated for publication from January 1, 2017 to March 30, 2020 and January 1, 2018 to June 30, 2021, respectively, to allow for a 3 year period of publication.
    Results: In 2017 and 2018, 43 of 75 (57.3%) and 47 of 83 (56.6%) podium presentations were published within 3 years, respectively. No significant difference was found between the mean time to publication within 3 years (13.0 months vs 14.1 months for 2017 and 2018, respectively; p=0.96). Similarly, the mean difference of journal impact factors between both years did not reach significance (6.57 and 10.7 for 2017 and 2018, respectively; p=0.09). The median impact factor (IF) was 4.54 (range: 40.3) and 4.62 (range: 70.7) in 2017 and 2018, respectively. Of the presentations published, 53.4% (2017) and 38.3% (2018) appeared in the journal Gynecologic Oncology. Significant positive correlations for the likelihood of publication were determined among the following: funding status (r=0.93) including funding involving National Institutes for Health (r=0.91) or pharmaceutical (r=0.95), clinical trial study design (r=0.94), and pre-clinical research (r=0.95) (all p<0.005).
    Conclusions: At the 2017 and 2018 SGO Annual Meetings 57% of podium presentations were published in a peer-reviewed journal within 3 years. Publication in peer-reviewed journals is crucial for timely distribution of clinical information to the medical community.

    Abstract Background: Exposure to opioid analgesics have historically raised concern for a risk of developing opioid use disorder. Prescriber audit-and-feedback interventions may reduce opioid prescribing, but some studies have shown detrimental effects for current users. We examined the effectiveness of an audit and feedback intervention, named Portrait, to reduce initiation of opioid analgesics among opioid-naïve patients experiencing pain.
    Methods: REDONNA was a single-blinded, two-arm (Early vs Delayed mailing) randomized trial of a portrait for eligible family physicians (FPs) in British Columbia (BC), Canada. The primary outcome was the change in the number of initiations of opioid analgesic prescriptions written by FPs for acute/chronic pain management. We compared outcomes for a 6-month window before vs. after each mailed intervention, using differences in percent differences (DPD) with 95% confidence intervals (CI) and odds ratios (OR) from logistic regressions adjusted for clustering of patients by FP.
    Results: In the Early (n = 2260) and Delayed (n = 2156) groups, opioid initiations per month were the same in the Before (2.10 Early; 2.06 Delayed) and After (1.94 Early; 1.95 Delayed) windows. The DPD was -2.1% (CI: -4.4% to 0.3%), and ORs were: 0.98 (CI: 0.96 to 1.01) for any opioid, 0.97 (CI: 0.94 to 1.01) for codeine (62% of initiations), and 1.0 (CI: 0.97 to 1.07) for tramadol (25% of initiations). There were no differences in mean quantity of tablets, mean milligrams of morphine equivalents (MME), or mean number of days.
    Conclusion: Portrait had no impact on FPs' rates of prescribing opioid analgesics to opioid-naïve patients experiencing pain.
    Trial registration: The study was registered prospectively on 30 March 2020 at the ISRCTN Register (https://www.isrctn.com/ISRCTN34246811).

    Abstract Background: The objective of this study was to investigate the utility of neutrophil gelatinase-associated lipocalin (NGAL) and calprotectin (CPT) to predict long-term graft survival in stable kidney transplant recipients (KTR).
    Methods: A total of 709 stable outpatient KTR were enrolled >2 months post-transplant. The utility of plasma and urinary NGAL (pNGAL, uNGAL) and plasma and urinary CPT at enrollment to predict death-censored graft loss was evaluated during a 58-month follow-up.
    Results: Among biomarkers, pNGAL showed the best predictive ability for graft loss and was the only biomarker with an area under the curve (AUC) > 0.7 for graft loss within 5 years. Patients with graft loss within 5 years (n = 49) had a median pNGAL of 304 [interquartile range (IQR) 235-358] versus 182 (IQR 128-246) ng/mL with surviving grafts (P < .001). Time-dependent receiver operating characteristic analyses at 58 months indicated an AUC for pNGAL of 0.795, serum creatinine-based Chronic Kidney Disease Epidemiology Collaboration estimated glomerular filtration rate (eGFR) had an AUC of 0.866. pNGAL added to a model based on conventional risk factors for graft loss with death as competing risk (age, transplant age, presence of donor-specific antibodies, presence of proteinuria, history of delayed graft function) had a strong independent association with graft loss {subdistribution hazard ratio (sHR) for binary log-transformed pNGAL [log2(pNGAL)] 3.4, 95% confidence interval (CI) 2.24-5.15, P < .0001}. This association was substantially attenuated when eGFR was added to the model [sHR for log2(pNGAL) 1.63, 95% CI 0.92-2.88, P = .095]. Category-free net reclassification improvement of a risk model including log2(pNGAL) in addition to conventional risk factors and eGFR was 54.3% (95% CI 9.2%-99.3%) but C-statistic did not improve significantly.
    Conclusions: pNGAL was an independent predictor of renal allograft loss in stable KTR from one transplant center but did not show consistent added value when compared with baseline predictors including the conventional marker eGFR. Future studies in larger cohorts are warranted.
