Abstract Although guidelines are available for hereditary hemochromatosis, a high percentage of the recommendations within them are not shared between the different guidelines. Our main aim is to provide an objective, simple, brief, and practical set of recommendations about therapeutic aspects of HFE hemochromatosis for p.Cys282Tyr (C282Y/C282Y) homozygous genotype, based on the published scientific studies and guidelines, in a form that is reasonably comprehensible to patients and people without medical training. This final version was approved at the Hemochromatosis International meeting on 12th May 2017 in Los Angeles.

Abstract Purpose: Parkinson's disease (PD) can lead to sexual dysfunction. Yet, studies have shown that neurologists do not often discuss possible sexual health consequences with their patients. Thus, in this study, we investigated the communication on sexual health between healthcare workers and PD patients in Flanders, Belgium.
Methods: Through an online survey, PD patients were contacted. Sexual dysfunction was measured with the Arizona Sexual Experience Scale (ASEX), stage of PD with the Hoehn and Yahr scale, and self-made questions on the communication between healthcare workers and PD patients.
Results: In total, 100 PD patients responded, of which 23% had possible sexual dysfunction. Of these respondents, 60% had never received any information about possible sexual health consequences. More than half (58%) of the patients felt their neurologist should provide information on possible sexual health consequences, though only 14% had ever received information from their neurologist. Male respondents expressed a greater need for information than female respondents (p = .049), although no difference between both groups in receiving information was found (p = .294). In addition, low to middle educated people generally received more information than higher educated people (p = .018).. The more severe the disease is, as measured by the Hoehn and Yahr scale, the more information a patient receives (p = .012). The most frequently mentioned barriers in discussing sexual health are a lack of initiative by the neurologist (41%) and awkwardness to discuss sexuality (41%).
Conclusion: This study showed that PD patients expect information from neurologists on possible sexual health consequences, but seldom receive information. More attention should be given to training neurologists in discussing sexual health. Abstract This work focuses on the in-plane shear behavior of carbon fiber-reinforced polyamide 6 cross-ply laminates. Bias-extension tests are performed at different temperatures and deformation rates. Forming behavior is compared with the prediction of the pin-jointed net (PJN) idealization, and the effects of temperature and deformation rate are investigated. A key objective is to estimate the locking angle of the material. The results show that the shear behavior is comparable to the one of woven fabrics for limited deformations, with the PJN holding up to a shear angle of 30°. The load required to deform the specimens is higher for lower temperatures or faster deformation rates, and the locking angle of the material is 45°. After locking, fiber slippage and compaction are significant rather than inter-ply fiber rotation prior to the locking angle. Moreover, the deformation localization shows that heterogeneities in the shear pattern are more pronounced at the lower deformation rates. Although the average shear angle measured in the bias-extension test is not affected by the deformation rate, the localization of shear deformation at lower deformation rates causes a higher variability of the shear angle within the test.

Abstract Rain-fed agriculture is the main source of livelihood for most of Burundi’s population, especially in the northeastern part of the country. This research is aimed at examining how smallholder farmers in the Northeastern region of Burundi perceive climate change and variability and at identifying the methods that are used to adapt, based on data from 200 small farmers and on actual weather data recorded between 1986 and 2017. We find that the majority of farmers (54%) perceive significant increases in temperature and unpredictability of rainfall duration and intensity and are making adjustments to adapt their agriculture in response to changes in climate. Over 80% of farmers have implemented at least one adaptation strategy among the nine evaluated. Changing crop varieties, changing fertilizers, and planting shade trees are the main adaptation strategies that were being implemented by farmers across the study area. The results of a binary regression model showed that the agricultural education and experience of farmers, as well as farm and family size, livestock ownership, climate information access, credit access, and farm income, strongly influence smallholder farmers’ decisions to adapt to climate change. The main obstacles are the lack of information on climate and adaptation strategies, and poverty, which makes it difficult to cope with the increased costs of farming. Understanding farmers’ perceptions of climate change and variability on a local level would provide information on how to develop adaptation strategies. The present study suggests the need for strengthening farmers’ capacities and improving the policy framework for adaptation to climate change in order to improve farmers’ livelihoods. Implications for policymakers will, therefore, include making flexible credit facilities, and investing in training extension agents on both climate change outreach and coping strategies.

Abstract Introduction: Non-occlusive mesenteric ischemia (NOMI) is an infrequent cause of acute mesenteric ischemia with atypical symptoms and a high mortality.
Patients and methods: We report two cases of NOMI characterized by their difference in etiology, treatment and outcome.
Results: In the first case, the patient developed NOMI after septic shock, she was successfully treated with surgery. The second patient experienced two episodes of cardiac arrest followed by NOMI. He received only supportive care and died shortly after. We present a short review of the literature, discussing epidemiology, pathophysiology, clinical presentations, diagnosis and management of NOMI.
Conclusion: NOMI is a rare disease with a difficult diagnosis. Early recognition and treatment with supportive therapy, vasodilator drugs and possibly surgery can significantly lower mortality.

Abstract In this study, we assess the validity of an African-scale groundwater pollution model for nitrates. In a previous study, we identified a statistical continental-scale groundwater pollution model for nitrate. The model was identified using a pan-African meta-analysis of available nitrate groundwater pollution studies. The model was implemented in both Random Forest (RF) and multiple regression formats. For both approaches, we collected as predictors a comprehensive GIS database of 13 spatial attributes, related to land use, soil type, hydrogeology, topography, climatology, region typology, nitrogen fertiliser application rate, and population density. In this paper, we validate the continental-scale model of groundwater contamination by using a nitrate measurement dataset from three African countries. We discuss the issue of data availability, and quality and scale issues, as challenges in validation. Notwithstanding that the modelling procedure exhibited very good success using a continental-scale dataset (e.g. R

Abstract Fibromyalgia (FM) is a chronic illness that does not have clear physical consequences, yet research shows that FM patients often have a low body image. An online cross-sectional study was conducted in Flanders, Belgium, among FM women who are connected to the Flemish League for fibromyalgia patients. An adjusted Body Image Scale (BIS) was used to assess body image, the General Health Questionnaire-12 (GHQ-12) was used for mental health, and the Visual Analogue Scale Fibromyalgia Impact Questionnaire (VASFIQ) was used for FM symptoms. Medication use was assessed by using a 4-point Likert scale. Time since diagnosis and age was assessed. A total of 103 women with FM responded. Linear regression showed that BIS was best predicted in a model by using VASFIQ, GHQ-12, time since diagnosis, and sleep medication, wherein only the GHQ-12 was significant as a variable (B = 0.292; p = 0.009). This model explained 19.3% of the variance. The role of sleep medication use disappeared when controlling for mental health. Mental health was more clearly associated with body image than medication use, or even fibromyalgia symptoms. Thus, having negative mental health is associated with a negative body image. In order to improve the body image of FM patients, symptom control alone is not enough; improving mental health is equally important.

Abstract Flash floods are a significant threat to arid and semi-arid regions, causing considerable loss of life and damage, including roads, bridges, check dams and dikes, reservoir filling, and mudslides in populated areas as well as agricultural fields. Flood risk is a complex process linked to numerous morphological, pedological, geological, anthropic, and climatic factors. In arid environments such as where Bayech basin is located in southwestern Tunisia, the hydrometric data are insufficient due to the absence of measuring points. Using the hybrid fuzzy Analytical Hierarchy Process (F-AHP) and the frequency ratio statistical methods, this study aims to map flooding risks in an ungauged basin that is extremely prone to flooding. Data related to soil texture, slope, land use, altitude, rainfall, drainage density, and distance from the river were used in the risk analysis. The obtained flood risk maps from both F-AHP and FR models were validated on the basis of the Receiver Operating Characteristic (ROC), the Area Under the Curve (AUC), and the inventory map. Results revealed that areas of high and very high susceptibility to flooding are mainly located in the downstream part of the basin, where the town of Gafsa is located. Other upstream sites are also at risk. In this basin, slope is predominantly behind runoff accumulation, whereas soil type plays a major role in amplifying waterproofing and therefore overflow. The results derived from both methods clearly demonstrate a viable and efficient assessment in flood-prone areas. The F-AHP and FR methods have ROC values of 95% and 97%, respectively. Considering these results in the decision-making process, these outputs would enable the implementation of the necessary measures to mitigate flood risk impacts ensure sustainable development along with an effective management in Tunisian arid environments, for the well-being of local communities at risk.

Abstract Small bowel obstruction (SBO) due to intussusception in adults is a rare condition. Diagnosis at the time of admission is usually challenging. More often than not, a bowel intussusception in adults is secondary to an organic condition, frequently malignancies. Therefore, a surgical approach is indicated most of the times. We report the case of a forty-nine years old lady presenting with a SBO secondary to small bowel metastases with two ileo-ileal intussusceptions, one of which was missed at initial surgical exploration. A giant cell carcinoma of the lung (GCCL) with small bowel metastases was diagnosed subsequently. The case is presented as well as a brief review of literature.

Abstract We estimated vulnerability and pollution risk of groundwater at the pan-African scale. We therefore compiled the most recent continental scale information on soil, land use, geology, hydrogeology and climate in a Geographical Information System (GIS) at a resolution of 15 km × 15 km and at the scale of 1:60,000,000. The groundwater vulnerability map was constructed by means of the DRASTIC method. The map reveals that groundwater is highly vulnerable in Central and West Africa, where the watertable is very low. In addition, very low vulnerability is found in the large sedimentary basins of the African deserts where groundwater is situated in very deep aquifers. The groundwater pollution risk map is obtained by overlaying the DRASTIC vulnerability map with land use. The northern, central and western part of the African continent is dominated by high pollution risk classes and this is very strongly related to shallow groundwater systems and the development of agricultural activities. Subsequently, we performed a sensitivity analysis to evaluate the relative importance of each parameter on groundwater vulnerability and pollution risk. The sensitivity analysis indicated that the removal of the impact of vadose zone, the depth of the groundwater, the hydraulic conductivity and the net recharge causes a large variation in the mapped vulnerability and pollution risk. The mapping model was validated using nitrate concentration data of groundwater as a proxy of pollution risk. Pan-African concentration data were inferred from a meta-analysis of literature data. Results shows a good match between nitrate concentration and the groundwater pollution risk classes. The pan African assessment of groundwater vulnerability and pollution risk is expected to be of particular value for water policy and for designing groundwater resources management programs. We expect, however, that this assessment can be strongly improved when better pan African monitoring data related to groundwater pollution will be integrated in the assessment methodology.
