    Abstract To evaluate the safety and efficacy of transarterial chemoembolization (TACE) in patients with unresectable hepatocellular carcinoma, a retrospective analysis of patients who underwent TACE utilizing up to one vial of LC BeadM1 (70-150 μm) embolics with doxorubicin was performed. If tumor devascularization was not achieved, up to one additional vial of LC Bead (100-300 μm) embolics with doxorubicin was subsequently administered. Ninety-four sessions of TACE were completed among 48 patients. After a mean follow-up of 182 days, complete response was achieved in 10 patients (21%), partial response in 19 (40%), stable disease in 8 (17%), and progressive disease in 10 (21%). Disease control was achieved in 79% of patients: 90% of patients treated with 70- to 150-μm embolics alone vs. 70% of patients treated with both sizes of embolics (

    Abstract Image-guided percutaneous interventions for treatment of pseudoaneurysms have become more common in cases with failed or limited endovascular access but may be technically difficult due to anatomic location and surrounding structures. We present the case of a man with a history of intraductal papillary mucinous neoplasm after Whipple procedure with postoperative hemorrhage from a gastroduodenal artery pseudoaneurysm. The pseudoaneurysm was successfully treated by an anterior abdominal percutaneous approach utilizing a blunt-tip needle technique to avoid adjacent bowel injury.

    Abstract Spontaneous mesocaval shunt (SMCS) is an uncommon cause of refractory encephalopathy after transjugular intrahepatic portosystemic shunt (TIPS) creation. We report a patient who presented with refractory hepatic encephalopathy (HE) post-TIPS creation for variceal bleeding in whom a SMCS was found. Percutaneous transhepatic occlusion of the SMCS reduced the portal vein flow diverted from the liver and provided sufficient liver perfusion to reverse the HE while maintaining satisfactory decompression of esophageal varices.

    Abstract Lower gastrointestinal hemorrhage remains a common disease, frequently presenting with acute life-threatening symptoms. Although prompt detection and treatment are imperative, it is difficult to diagnose lower gastrointestinal hemorrhage in an accurate and efficient manner. Most available modalities are time consuming. Computed tomographic angiography of the abdomen and pelvis, on the other hand, has the unique capability of rapidly detecting whether life-threatening hemorrhage is occurring and accurately localizing it, thus facilitating definitive treatment. We present a case in which computed tomographic angiography was invaluable in the detection and subsequent empirical transarterial embolization of a lower gastrointestinal hemorrhage and offer evidence as to why it should be a first-line tool in the management of these patients.
