    Abstract Background and aims: The impact of health financing on the performance of the entire health system, including access, quality, and efficiency of healthcare, has been emphasized in the Astana Declaration, and the need to strengthen primary healthcare (PHC) and policy integration has been emphasized. After about two decades, the family medicine (FM) program in Iran is still facing great challenges. The aim of this study is to explore strategies for strengthening financing of the FM program in Iran, a vital component of PHC.
    Methods: A qualitative study was conducted in 2021. Purposeful sampling was used to select 34 policymakers, managers, and experts from various levels of the Ministry of Health, Iran universities of medical sciences, plan and budget organization of Iran, and health insurance organization in Iran. Thirty-four semistructured interviews were conducted to collect data, which were analyzed by content analysis.
    Results: Through the analysis of interviews, our study has identified five strategies (identification and management of sustainable resources, pooling of sustainable resources, modeling of service provision, payment system model and its implementation process, and FM management structure), and 13 actions for strengthening financing of the FM program in Iran.
    Conclusion: Our study has identified five strategies and 13 actions for strengthening the financing of the FM program in Iran. These strategies and actions should be considered by policymakers during the review of the FM program in Iran. Without implementation of the suggested strategies and action, allocated resources may be wasted.

    Abstract PLA (Poly-lactic acid)-wood provides more biodegradability through natural fibers, a significant advantage of pure PLA. Nevertheless, these bio-composites may have inferior mechanical properties compared to non-degradable polymer composites, considering the lower strength of natural particles compared to synthetic fibers. This research examines the fatigue behavior of additive-manufactured biopolymer PLA-wood and assesses its comparability with pure PLA. Therefore, solid fatigue test samples were printed using the FDM (fused deposition modeling) method. Afterward, fully reversed rotary bending fatigue experiments were performed at 4 different stress levels (7.5 to 15 MPa) to extract the S-N curve of PLA-wood. Moreover, the fatigue fracture surfaces of the PLA-wood were investigated and compared at the highest and lowest stress levels using an FE-SEM (Field Emission Scanning Electron Microscopy), indicating more ductile fracture marks at higher stress levels. The fatigue lifetime of the PLA-wood decreased by 87.48% at the highest stress level (15 MPa), rather than the result at the lowest stress level (7.5 MPa). Additionally, the results demonstrated that the fatigue characteristics of the printed pure PLA and PLA-wood were comparable, suggesting that the 3D-printed PLA-wood with the used printing parameters can be an alternative choice.

    Abstract The motor symptoms of Parkinson's disease (PD) have been shown to significantly improve by Levodopa. However, despite the widespread adoption of Levodopa as a standard pharmaceutical drug for the treatment of PD, cognitive impairments linked to PD do not show visible improvement with Levodopa treatment. Furthermore, the neuronal and network mechanisms behind the PD-induced cognitive impairments are not clearly understood. In this work, we aim to explain these cognitive impairments, as well as the ones exacerbated by Levodopa, through examining the differential dynamic patterns of the phase-amplitude coupling (PAC) during cognitive functions. EEG data recorded in an auditory oddball task performed by a cohort consisting of controls and a group of PD patients during both on and off periods of Levodopa treatment were analyzed to derive the temporal dynamics of the PAC across the brain. We observed distinguishing patterns in the PAC dynamics, as an indicator of information binding, which can explain the slower cognitive processing associated with PD in the form of a latency in the PAC peak time. Thus, considering the high-level connections between the hippocampus, the posterior and prefrontal cortices established through the dorsal and ventral striatum acting as a modulatory system, we posit that the primary issue with cognitive impairments of PD, as well as Levodopa's cognitive deficit side effects, can be attributed to the changes in temporal dynamics of dopamine release influencing the modulatory function of the striatum.

    Abstract Background: Low back pain is a common, multifaceted disorder that directly affects diverse aspects of people's lives in terms of health, personal and social lives. A variety of pathological disorders, including low back pain, may benefit from hydrotherapy.
    Objective: This study aimed to systematically analyze the efficacy of aquatic exercise on pain intensity, disability, and quality of life among adults with low back pain.
    Data sources: A systematic search was conducted in PubMed, Web of Science, Medline, and Scopus up to February 2023 for randomized controlled trials (RCTs) that which examined the impact of aquatic exercise. The most relevant articles were selected based on research criteria. The PEDro scale was applied to assess the quality of the included studies. Review Manager 5.3 was used for conducting all analyses.
    Study selection: Out of 856 articles, 14 RCTs (
    Results: Pooled results illustrated that aquatic exercises significantly reduced pain (mean differences (MD): -3.82;
    Conclusion: The current review showed that aquatic exercise regimens were effective among adults with low back pain. High-quality clinical investigations are still needed to support the use of therapeutic aquatic exercise in a clinical setting.

    Abstract تجربة الاقتراب من الموت، أو NDE، هي ظاهرة جذبت انتباه الباحثين في القرن الماضي. وقد حازت هذه الظاهرة على اهتمام الباحثين وحتى عامة الناس في ايران في العقد الأخير. وقد فسرها البعض عمومًا على أنها نتاج نشاط الدماغ وأنها أوهام تشكلت بفعل التجربة. من ناحية أخرى، ذهب البعض الى صحة ما تم الإبلاغ عنه حول هذه الظاهرة، وتم استخدامها كدليل لإثبات أصول العقيدة، رغم وجود أدلة عقلية أو نقلية تثبت بطلانها. نسعى في هذه المقالة، ومن خلال التحليل المبني على مبادئ الحكمة المتعالية، الى البحث في هذه التجارب وإثبات أن ظاهرة تجربة الاقتراب من الموت بسبب انخفاض الارتباط بين الروح والجسد ليس لها أي تباين متأصل مع النوم أوالغيبوبة. وأن ما يشاهده من حصلت لهم ظاهرة الاقتراب من الموت، لا يختلف عما يشاهده الناس في الحلم أو الغيبوبة، وهو مشاهدات النفس في عالم الخيال.
