    Abstract Proper advice regarding home-based care of normal infants is no less important than managing critical diseases. Otherwise parents tend to follow traditional practices running in the family which are often deleterious. Clinicians must also be aware of the faulty traditional practices so that they can actively enquire them in the child’s history. The author enlisted various such faulty traditional practices as experienced by him during his OPD practice in various public and corporate hospitals over a period of five years. Topics relating mainly to the infant age group have been chosen for this article. Relevant search was done on these topics in standard paediatric textbooks, journals, and websites (like WHO, NICE, AAP) to find the validity of these practices as well as the standard recommendations on these topics. It was found that the standard recommendations often differ from the traditional practices. Detailed text was included from various authentic sources regarding few dubious topics related to home-based infant care. Clinicians should be aware of the correct recommended methods regarding the day to day homebased infant care. They should counsel the parents accordingly so as to avoid any harm caused by faulty traditional practices and social beliefs.

    Abstract Elevation of the intermediate amino acid metabolite Homocysteine (Hcy) causes Hyperhomocysteinemia (HHcy), a metabolic disorder frequently associated with mutations in the methionine-cysteine metabolic cycle as well as with nutritional deficiency and aging. The previous literature suggests that HHcy is a strong risk factor for cardiovascular diseases. Severe HHcy is well-established to correlate with vascular pathologies primarily via endothelial cell death. Though moderate HHcy is more prevalent and associated with an increased risk of cardiovascular abnormalities in later part of life, its precise role in endothelial physiology is largely unknown. In this study, we report that moderate elevation of Hcy causes endothelial dysfunction through impairment of their migration and proliferation. We established that unlike severe elevation of Hcy, moderate HHcy is not associated with suppression of endothelial VEGF/VEGFR transcripts and ROS induction. We further showed that moderate HHcy induces a sub-lethal ER stress that causes defective endothelial migration through abnormal actin cytoskeletal remodeling. We also found that sub-lethal increase in Hcy causes endothelial proliferation defect by suppressing mitochondrial respiration and concomitantly increases glycolysis to compensate the consequential ATP loss and maintain overall energy homeostasis. Finally, analyzing a previously published microarray dataset, we confirmed that these hallmarks of moderate HHcy are conserved in adult endothelial cells as well. Thus, we identified adaptive UPR and metabolic rewiring as two key mechanistic signatures in moderate HHcy-associated endothelial dysfunction. As HHcy is clinically associated with enhanced vascular inflammation and hypercoagulability, identifying these mechanistic pathways may serve as future targets to regulate endothelial function and health.

    Abstract The important role of melatonin in plant growth and metabolism together with recent advances in the potential use of nanomaterials have opened up interesting applications in agriculture. Various nanovehicles have been explored as melatonin carriers in animals, and it is now important to explore their application in plants. Recent findings have substantiated the use of silicon and chitosan nanoparticles (NPs) in targeting melatonin to plant tissues. Although melatonin is an amphipathic molecule, nanocarriers can accelerate its uptake and transport to various plant organs, thereby relieving stress and improving plant shelf-life in the post-harvest stages. We review the scope and biosafety concerns of various nanomaterials to devise novel methods for melatonin application in crops and post-harvest products.

    Abstract A 9-year-old girl patient presented with left-sided weakness and joint contractures developing over a period of 18 months. She was known to be suffering from β-thalassemia major and was on regular blood transfusions. Eighteen months ago, she had suffered from an episode of ischemic cerebrovascular accident affecting the right side of her brain. Magnetic resonance angiogram revealed vaso-occlusive disease affecting mainly the anterior cerebral circulation, resembling Moyamoya disease. She was advised to carry out regular physiotherapy but her parents discontinued it, which resulted in the gradual development of joint contractures and muscle wasting.

    Abstract OBJECTIVE: The COVID-19 pandemic has hit countries such as Italy, Spain, France, UK, and the USA with great force, whereas the number of cases in countries such as India and most parts of Africa is comparatively lower. Such observation has made many people believe that the low-income countries may be more immune to COVID-19. Theories such as warm climate, weaker strain of the virus, and cross-protection by malaria have been popularly put forward. One such interesting theory is that since the general people of the low- income countries are mostly habituated to dwell in lesser hygienic condition and with lesser medical attention throughout their lifetime; hence, they have naturally acquired better immunity and more resilience against many infective diseases. MATERIALS AND METHODS: We sought to investigate the above claim by comparing the case fatality rate (CFR) as well as number of cases per million population versus the gross domestic product at purchasing power parity per capita of different countries. RESULTS: We found that while the number of cases showed a slight decline in the lower-income countries, the CFR was independent of the financial condition of the country. CONCLUSION: We conclude that the theory of better immunity in economically poor countries is a misconception. We suggest that people must come out of these misconceptions and resort to strict home isolation.

    Abstract OBJECTIVE: The novel coronavirus pandemic is ravaging throughout the world. It has infected more than 1.2 million people and killed more than 64,000. Frantic research is underway to find prevention and cure. Of late, Bacillus Calmette–Guérin (BCG) has been speculated as a possible protection from COVID-19. We sought to investigate the evidence behind the claim. MATERIAL AND METHODS: Data were collected regarding the total number of COVID-19 cases per million and total number of COVID-19 deaths per million in various countries. The BCG vaccination policies of these countries were also obtained. RESULTS: It was seen that the countries with no universal BCG policy had a mean 1272.9 (median 795) cases per million and 80.7 deaths (median 18) per million population. On the contrary, the countries with a universal BCG vaccination policy had a mean 131.2 (median 40) cases per million and 4 deaths (median 1) per population. The difference is highly significant (P < 0.001). CONCLUSION: The data strongly support the hypothesis that BCG may offer protection from COVID-19. Heterologous protection offered by BCG through production of trained immunity, epigenetic reprogramming of monocytes, non-specific activation of NK cells, and increase of pro-inflammatory cytokines (particularly, tumor necrosis factor [TNF]-alpha and interleukin 1 beta) production may be the mechanism behind its cross- protection against the novel coronavirus.

    Abstract An extended version of task group report (TG)-119 dosimetric tests was introduced and tested on the TrueBeam linear accelerator setup. Treatment plan results and quality assurance (QA) results of RapidArc (RA) and intensity-modulated radiotherapy (IMRT) were compared to understand the limitation and efficacy of the RA and IMRT system of the linear accelerator. Test structure sets were drawn on OCTAVIUS four-dimensional (4D) phantom computed tomography scan data for this study. We generated treatment plans based on the specified goal in the Eclipse™ treatment planning system using RA and IMRT in the study phantom. We used the same planning objectives for RA and IMRT techniques. Planar dose verification was performed using electronic portal imaging device and OCTAVIUS 4D phantom. The treatment log file was further analyzed using Pylinac (V2.4.0 (Open Source Code library available on Github, runs under Python programming language)) to compare the dosimetric outcome of RA and IMRT. Dose to the planning target volume (PTV) 1-5 and organ at risk (OAR) were analyzed in this study for the efficiency comparison of RA and IMRT. The primary objective was accomplished by adhering to the dose constraints associated with PTV 2 and the OAR. RA and IMRT also met the secondary objective. The tertiary goal of dose delivery to PTV 4 was met with RA but not IMRT. This study can be utilized to compare different institutions' planning and patient-specific QA (PSQA) procedures. The findings of this study were in line with the published works of the literature. A multi-institutional planning and delivery accuracy audit can be built using this structure and set of planning objectives having similar PSQA phantom. The TG-119 report incorporated test challenges that were combined in a single study set and a single plan. This reduces the complexity of performing the original TG-119 tests, whereas keeping the challenges as introduced in the TG-119 report. This study's planning and dosimetric results could be further utilized for dosimetry audit with any institute having a linear accelerator and OCTAVIUS 4D phantom for PSQA.
