    Abstract Pasteurella multocida is a small facultative anaerobic Gram-negative coccobacillus. Bites or scratches from cats or dogs are common transmission route causing zoonotic infections in humans. The pathogen rarely cause prosthetic joint infection. We report the first case, to our knowledge, of a prosthetic joint infection in a patient underwent liver transplantation caused by this pathogen. Pasteurella multocida is a high pace growing pathogen. Physician should raise awareness with related history especially in patients with immunosuppressive status. Management with the proper antibiotics administration in conjunction with timely surgical intervention could prevent devastating complications and preserve the artificial joint.

    Abstract Tests of the planktonic foraminifer Globigerinoides ruber (white; d'Orbigny) have become a standard tool for reconstructing past oceanic environments. Paleoceanographers often utilize the Mg/Ca ratios of the foraminiferal tests for reconstructing low-latitude ocean glacial-interglacial changes in sea surface temperatures (SST). We report herein a comparison of Mg/Ca measurements on sample pairs (n = 20) of two G. ruber (white) morphotypes (G. ruber sensu stricto (s.s.) and G. ruber sensu lato (s.l.)) from surface and downcore samples of the western Pacific and Indian Oceans. G. ruber s.s. refers to specimens with spherical chambers sitting symmetrically over previous sutures with a wide, high arched aperture, whereas G. ruber s.l. refers to a more compact test with a diminutive final chamber and small aperture. The G. ruber s.s. specimens generally show significantly higher Mg/Ca ratios compared to G. ruber s.l. Our results from the Mg/Ca ratio analysis suggest that G. ruber s.l. specimens precipitated their shells in slightly colder surface waters than G. ruber s.s. specimens. This conclusion is supported by the differences in delta18O and delta13C values between the two morphotypes. Although it is still unclear if these two morphotypes represent phenotypic variants or sibling species, our findings seem to support the hypothesis of depth and/or seasonal allopatry within a single morphospecies.

    Abstract Changes in the local freshwater budget over the last 22,000 years have been estimated from a sediment core located in the southern South China Sea (SCS) using a combined approach of Mg/Ca and oxygen isotopes on the planktonic foraminifera Globigerinoides ruber (white) sensu stricto (s.s.). Core MD01-2390 (06?28,12N, 113?24,56E; water depth 1591?m) is located near the glacial paleo-river mouths of the Baram, Rajang and North Sunda/Molengraaff Rivers that drained the exposed Sunda Shelf. The delta18Oseawater record reveals lower average values (-0.96?0.18 per mil) during the Last Glacial Maximum (LGM) when compared with modern values (-0.54?0.18 per mil). Low salinity during the LGM is interpreted to reflect a higher freshwater contribution due to a greater proximity of the core site to the mouths of the Baram, Rajang and North Sunda/Molengraaff Rivers at that time. A general deglacial increasing trend in salinity due to the progressive landward displacement of the coastline during deglacial shelf flooding is punctuated by several short-term shifts towards higher and lower salinity that are likely related to abrupt changes in the intensity of the East Asian summer monsoon. Thus, the deglacial delta18Oseawater changes reflect the combined effects of sea-level-induced environmental changes on the shelf (e.g. phases of retreat and breakdown of the shelf drainage systems) and East Asian monsoon climate change. Lower salinity than at present during the Early Holocene may be attributed to an increase in summer monsoonal precipitation that is corroborated by previous marine and terrestrial studies that report a Preboreal-Early Holocene monsoon optimum in the Asian monsoon region.

    Abstract Background: This study aims to determine whether nail size or the difference between canal and nail diameter (CN difference) affects the union rate and time of femoral shaft fracture treated with an interlocking intramedullary nail (IMN).
    Methods: This was a retrospective review of 257 patients with femoral shaft fractures treated with IMN at a tertiary trauma medical center. All the IMN inserted were the same (Stryker T2 Femoral Nail). The patients were divided into groups based on nail size (10-, 11-, 12-, or 13-mm) and CN difference (< 1, 1-2, or > 2 mm), and union rate and time to union were compared.
    Results: The 10-, 11-, 12-, and 13-mm groups based on nail size had 113, 74, 54, and 16 patients, respectively. The overall union rate was 97% (257/265). No significant differences in union rate or time to union were observed among these 4 groups. The groups based on CN differences of < 1-, 1 to 2, and > 2 mm comprised 143, 79, and 35 patients, respectively. Again, no significant differences were noted in union rate or mean time to union among the groups.
    Conclusions: Similar union rate and time to union were observed, regardless of nail size or CN difference. This finding indicates that most simple femoral shaft fractures can be treated with a standard, reamed 10-mm IMN. A larger nail insertion is unnecessary and presents more risks; comparatively, the use of a small nail with less reaming is simpler, requires shorter operative times, results in less blood loss, and is less expensive.

    Abstract Incubator care is essential for premature infants during early hospitalization. As the infants' conditions improve, incubator weaning becomes necessary. This retrospective study aimed to evaluate the effect of body weight gain and status of intake-calorie gain on the incubator weaning process for very low birth weight (VLBW) premature infants. The study included 127 VLBW premature neonates. We analyzed data on clinical characteristics potentially associated with the weaning period and the end-weaning body weight (EWBW), including body weight gain status, intake-calorie gain status, and disease conditions. The neonates were weaned from the incubators at a mean postmenstrual age (PMA) of 35.1 ± 1.3 weeks; postnatal days, 37.7 ± 18.2 days; and body weight, 1882.8 ± 157.1 g. The total weaning period was 3.5 ± 3.1 days. Regarding the weaning period, there was a strong positive relationship only in the end-weaning PMA and the daily body weight within 3 days before incubator weaning. Further, regarding the factors associated with EWBW, only the end-weaning PMA and necrotizing enterocolitis had a significant positive impact. Body weight gain and the status of intake-calorie gain showed no association with either the weaning period or the EWBW and, thus, were not related to the incubator weaning process.

    Abstract In 2015, Taiwan’s government announced the “Greenhouse Gas Reduction and Management Act”, the goal of which was a 50% reduction in carbon emissions by 2050, compared with 2005. The residential and commercial sectors produce approximately one third of all carbon emissions in Taiwan, and the number of construction renovation projects is much larger than that of new construction projects. In this paper, we considered the life-cycle CO 2 (LCCO 2 ) of a building envelope renovation project in Tainan and focused on local construction methods for typical row houses. The LCCO 2 of 744 cases with various climate zones, orientations, and insulation and glazing types was calculated via EnergyPlus, SimaPro, and a local database (LCBA database), and the results were then used to develop a machine learning model. Our findings showed that the machine learning model was capable of predicting annual energy consumption and LCCO 2 . With regard to annual energy consumption, the RMSE was 227.09 kW·h (per year) and the R 2 was 0.992. For LCCO 2 , the RMSE was 2792.47 kgCO 2 eq and the R 2 was 0.989, which indicates a high-confidence process for decision making in the early stages of building design and renovation.

    Abstract Background: Henoch-Schönlein purpura (HSP) is a common vasculitis in childhood characterized by purpura, arthritis, abdominal pain and renal involvement. However, bullous HSP is a rare cutaneous manifestation, and a few cases have been reported.
    Case presentation: Herein, we report a 15-year-old male with bullous HSP who presented with severe abdominal pain and hemorrhagic bullous lesions over his lower extremities. He was treated with corticosteroid, after which the symptoms improved dramatically. No recurrence was noted after follow-up, though scarring was present. We also reviewed the literature related to bullous HSP and identified 39 cases, most of whom were treated with corticosteroids.
    Conclusion: Clinicians should be aware of the atypical types of HSP, including bullous HSP. Most patients with bullous HSP have a good prognosis.
