    Abstract The anterior talofibular ligament (ATFL) is one of the lateral ankle ligaments stabilizing the ankle joint, primarily involved with restricting foot supination. There has been limited research on precise ATFL anatomy and variations, and several studies have conflicting results. The objective of this study was to determine if a correlation exists between ATFL variation and sex, height, weight, and age. In this study, 15 male ankles and 24 female ankles were dissected free of overlying structures to reveal the ATFL, which was classified based on the number of fascicles. Nine of the ligaments had one fascicle, 13 had two incompletely separated fascicles, 12 had two completely separated fascicles, and three had three fascicles. Two ankles had no ATFL. Ligament length and width were measured using the program ImageJ; average length was 19.2 mm and average width was 9.59 mm. Male ligaments were longer and wider than female ligaments. A multivariate regression model was used to assess the influence of sex, height, weight, age, ligament length, and ligament width in predicting ligament variant type; these factors were determined to have no influence. This study found a large amount of ATFL variability, but no correlation between height, weight, age, ligament length, ligament width and ATFL variation. Male ligaments were longer and wider than female ligaments.

    Abstract Following passage of the Patient Protection and Affordable Care Act (ACA) in the United States, the Kentucky Health Benefit Exchange, Kynect, began operating in Kentucky in October 2013. Kentucky expanded Medicaid eligibility in January 2014. Together, Kynect and Medicaid expansion provided access to affordable health care coverage to hundreds of thousands of individuals in Kentucky. However, following the Kentucky gubernatorial election in 2015, the newly inaugurated governor moved to dismantle Kynect and restructure the Medicaid expansion, jeopardizing public health gains and the state economy. As the first state to announce both the closure and restructuring of a state health insurance marketplace and Medicaid expansion, Kentucky may serve as a test case for the rest of the nation for reversal of ACA-related health policies. This article describes Kynect and the Kentucky Medicaid expansion and examines the potential short-term and long-term impacts that may occur following changes in state health policy. Furthermore, this article will offer potential strategies to ameliorate the expected negative impacts of disruption of both Kynect and the Medicaid expansion, such as the creation of a new state insurance marketplace under a new governor, the implementation of a private option, and increasing the state minimum wage for workers.

    Abstract Age-related macular degeneration (AMD), the most common form of irreversible blindness in the industrially developed world, can present years before a patient begins to lose vision. For most of these patients, AMD never progresses past its early stages to the advanced forms that are principally responsible for the vast majority of vision loss. Advanced AMD can manifest as either an advanced avascular form known as geographic atrophy (GA) marked by regional retinal pigment epithelium (RPE) cell death or as an advanced form known as neovascular AMD marked by the intrusion of fragile new blood vessels into the normally avascular retina. Physicians have several therapeutic interventions available to combat neovascular AMD, but GA has no approved effective therapies as of yet. In this chapter, we will discuss the current strategies for limiting dry AMD in patients. We will also discuss previous attempts at pharmacological intervention that were tested in a clinical setting and consider reasons why these putative therapeutics did not perform successfully in large-scale trials. Despite the number of unsuccessful past trials, new pharmacological interventions may succeed. These future therapies may aid millions of AMD patients worldwide.

    Abstract Background: Meniscal tear in older adults often accompanies knee osteoarthritis and is commonly treated with arthroscopic partial meniscectomy (APM) when patients have persistent pain after a trial of physical therapy. Cross-sectional evidence suggests that synovitis is associated with baseline pain in this patient population, but little is known about the relationship between synovitis and postoperative recovery or progression of knee osteoarthritis.
    Purpose/hypothesis: Intra-articular extended-release triamcinolone may reduce inflammation and thereby improve outcomes and slow disease progression. This article presents the rationale behind the Corticosteroid Meniscectomy Trial (CoMeT) and describes its study design and implementation strategies.
    Study design: Randomized controlled trial.
    Methods: CoMeT is a 2-arm, 3-center, randomized placebo-controlled trial designed to establish the clinical efficacy of extended-release triamcinolone administered via intra-articular injection immediately after APM. The primary outcome is change in Knee injury and Osteoarthritis Outcome Score Pain subscore at 3-month follow-up. Synovial biopsy, joint fluid aspirate, and urine and blood sample analyses will examine the associations between various objective measures of baseline inflammation and pre- and postoperative outcome measures and clinical responses to triamcinolone intervention. Quantitative 3-T magnetic resonance imaging will evaluate cartilage and meniscal composition and 3-dimensional bone shape to detect early joint degeneration.
    Results: We discuss methodologic innovations and challenges.
    Conclusion: To our knowledge, this is the first randomized double-blind clinical trial that will analyze the effect of extended-release triamcinolone acetonide on pain, magnetic resonance imaging measures of structural change and effusion/synovitis, soluble biomarkers, and synovial tissue transcriptomics after APM.

    Abstract Objectives: Problems relating to patients' medication are common during hospital discharge, often resulting in unnecessary harm to patients and even hospital readmission. To overcome these issues and improve patient discharge, an evidence-based approach was used to develop an innovative model of care for the supply of medication at hospital discharge. The model increases pharmacy involvement, uses community pharmacies in the supply process and encourages patient follow-up after discharge. This study aimed to determine if the proposed new model of care was considered feasible and acceptable by the target population.
    Methods: This formative evaluation involved qualitative interviews and focus groups to explore the opinions of stakeholders involved in delivery or use of the new model of care. A range of stakeholders participated, including a variety of hospital and community-based healthcare professionals as well as expert patients and carers. Thematic analysis of the data was undertaken.
    Results: The study provided a holistic overview of stakeholder perceptions of the new model of care. Overall, stakeholders were enthusiastic about the new model. Three themes emerged during analysis, providing an informative review of the new model. Themes included: impact, resources required and ensuring safety and quality of the new model of care. Potential issues were identified within each theme along with ideas for overcoming these issues.
    Conclusions: The new model of care appears to be a positive step towards improving patient discharge from hospital. Future work will involve a pilot of the new model of care using the study findings to assist with implementation.
