    Abstract This article summarizes the current knowledge in the field of clinical presentation and diagnosis of aspirin-exacerbated respiratory disease (AERD). The definition, prevalence, natural history, and clinical presentation of this distinct clinical syndrome are described. The classification and tolerance of particular groups of cyclooxygenase inhibitors by patients with AERD are presented. The authors comprehensively discuss provocation tests with aspirin as the most reliable method to confirm the diagnosis of AERD.

    Abstract Chronic obstructive pulmonary disease (COPD) affects approximately 10% of adults older than 40 years and is an important causes of disability and death in elderly subjects. A large proportion of COPD patients suffer from cardiovascular comorbidities. Thromboembolic events contribute considerably to morbidity and mortality in these subjects. This review summarizes the current evidence regarding the association of COPD with increased thromboembolic risk. We discuss multiple mechanisms potentially linking these conditions and available pharmacological interventions reducing the risk of thrombotic arterial and venous events with special attention paid to new oral anticoagulants.

    Abstract Background: The transmission of tuberculosis may affect the incidence rate of the disease in Poland. Genetic methods are of assistance in tracing the infection transmission, identifying its sources, determining the risk groups, and focusing on the preventive actions.
    Objectives: The objectives of this study lie in an assessment of tuberculosis transmission by genetic methods with the assistance of the standard epidemiologic interview.
    Methods: The genome DNA of 275 Mycobacterium tuberculosis (Mtb) strains from tuberculosis patients, inhabitants of the city of Krakow, was subjected to a genetic analysis via the spoligotyping method and the IS6110-Mtb1-Mtb2 polymerase chain reaction (PCR) method. If the DNA profiles were identical in both of the PCRs, they were considered identical and classified within one molecular family.
    Results: Among 275 strains, 104 genetic patterns (spoligotypes) were identified. Two hundred and three strains were divided into 66 molecular families (clusters) and analyzed with the IS6110- Mtb1-Mtb2 PCR method. Eighteen clusters were separated. In the Mtb1-Mtb2 clusters, 21 patients were in the risk groups (the homeless, prisoners, and nursing home residents). We did not confirm any direct or temporary contacts between the patients constituting the Mtb1-Mtb2 clusters (apart from the risk groups). However, the patients in these clusters often lived in the same parts of Krakow.
    Conclusions: The standard epidemiologic interview in tuberculosis patients should be combined with genetic methods. Active transmission of tuberculosis occurs largely among the individuals maintaining probably periodic contacts. The patients who are in the risk groups may play an important role in the transmission of tuberculosis.

    Abstract Purpose: Obstructive sleep apnea increases the risk of cardiovascular diseases. Alternations in prostacyclin and thromboxane concentrations and balance could constitute one of mechanisms linking sleep apnea and cardiovascular events. Thus we aimed to assess the concentrations of 6-keto-prostaglandin F1α (6-keto-PGF1α) (metabolite of prostacyclin) and thromboxane B2 (TXB2) (metabolite of thromboxane A2) in urine and blood of obstructive sleep apnea patients and controls (snoring subjects with otherwise normal polysomnogram).
    Material and methods: Overnight urine and morning blood samples were taken from subjects and controls at baseline and in sleep apnea group during continuous positive airway pressure (CPAP) treatment. Samples were analyzed using mass chromatography/gas spectrometry.
    Results: We analyzed data from 26 obstructive sleep apnea subjects (mean apnea-hypopnea index 45.4±17.3) and 22 well-matched controls. At baseline sleep apnea patients, when compared to controls, have higher 6-keto-PGF1α in urine (0.89±0.15 vs 0.34±0.06, p=0.01) and blood (24.49±1.54 vs 19.70±1.77, p=0.04). TXB2 levels in urine and blood were not different across groups. CPAP treatment significantly decreased 6-keto-PGF1α in urine (0.92±0.17 vs 0.22±0.10, p=0.04), but not in blood. TXB2 levels during CPAP treatment did not change significantly.
    Conclusions: These results suggest augmented systemic prostacyclin production in obstructive sleep apnea patients, which potentially could constitute a protective mechanism against detrimental effects of sleep apnea.

    Abstract Background: Effective control of asthma is the primary goal of its treatment. Despite an improved understanding of asthma pathogenesis and accessibility of novel therapies, the rate of uncontrolled asthma remains high.
    Objective: To find potential factors associated with asthma control in patients with aspirin-exacerbated respiratory disease (AERD).
    Methods: Clinical data were collected from a specifically structured questionnaire. Demographics, a history of upper airway symptoms, asthma course, exacerbations expressed as emergency department (ED) visits/hospitalizations, and asthma treatment were considered. Spirometry, skin prick tests, total IgE concentration, and blood eosinophil count were evaluated. Asthma control was assessed through the Asthma Control Test (ACT).
    Results: Out of 201 AERD patients, 41 (20.4%), 69 (34.3%), and 91 (45.3%) had controlled, partially controlled, and uncontrolled asthma, respectively. A multivariate ordered logistic regression analysis revealed that hospitalizations for asthma in the previous 12 months (OR 2.88; 95%CI, 1.11-7.46), ED visits for asthma throughout its duration (OR 1.05; 95%CI, 1.004-1.10), and total IgE concentration (OR 1.28; 95%CI, 1.02-1.60) were positively associated with poor asthma control, whereas FEV1 values (OR 0.98; 95%CI, 0.96-0.99) and medical care at a referential specialty clinic (OR 0.50; 95%CI, 0.27-0.95) were positively associated with good asthma control.
    Conclusions: The prevalence of uncontrolled asthma in AERD patients is high and similar to that observed in different asthmatic populations. Owing both to the specificity and complexity of the disease, AERD patients should stay under regular care of well experienced referential medical centers to ensure that this asthma phenotype is dealt with effectively.

    Abstract So far the only method of diagnostic assessment of tubercle bacillus infection was a skin tuberculin reaction - but it has low specificity and sensitivity. The article discusses the new tests in the diagnosis of a latent phase of tuberculosis infection and the active form of the disease based on the measurement of interferon gamma (IFN-gamma) released by lymphocyte T after the stimulation by antigens which are specific for M. tuberculosis: ESAT-6, CFP-10. The sensitivity and specificity of QuantiFERON-TB Gold and T-SPOT.TB tests in diagnosis of infection as well as in active form of disease, comparison between the results of tuberculin skin test and the results obtained by the new tests, and problem of incompatibility of results obtained thanks to those tests are discussed.

    Abstract Aspirin-induced asthma (AIA) is a distinct clinical entity characterised by hypersensitivity to aspirin and other non-steroidal anti-inflammatory drugs (NSAIDs) typically occurring in the form of asthma and severe rhinosinusitis. Its prevalence is about 10%, but AIA probably is underdiagnosed. The diagnosis can only be established with certainty by challenge tests, using increasing doses of aspirin. The crucial cell in AIA is the eosinophil, while the role of mast cell is more elusive. At the biochemical level, AIA is distinguished by profound underlying arachidonic acid disturbances. An overproduction of leukotrienes was observed in patients with aspirin hypersensitivity at baseline and following aspirin challenge. In some patients altered prostaglandin release following aspirin was encountered. The inhibition of COX-1, but not COX-2, was shown to precipitate post-challenge symptoms precipitated by aspirin. The new insights into eicosanoid molecular biology and genetics have recently emerged. The patients with aspirin-sensitive asthma require often prolonged oral, inhaled and nasal corticotherapy. Antileukotrienes also can be used. Aspirin and NSAIDs should be avoided. Nevertheless, highly specific COX-2 inhibitors are well tolerated. Aspirin desensitisation, followed by daily aspirin treatment, is a valuable therapeutic option.
