    Abstract Loin d'être un simple support des rapports sociaux, l'espace en constitue une dimension transversale majeure, à travers sa structuration matérielle et symbolique et les actions et interactions qui s'y rapportent et le reconfigurent constamment. À ce titre, il concerne l'ensemble des disciplines des sciences humaines et sociales, qu'elles l'aient de longue date constitué en objet scientifique privilégié, comme la géographie, ou qu'elles s'y intéressent aujourd'hui de façon plus soutenue que par le passé, comme la sociologie. Fruit d'un colloque qui s'est tenu en 2014, cet ouvrage vise à mettre en partage la thématique de l'espace entre ces disciplines, en confrontant, à la faveur d'objets d'étude, de catégories d'espaces et de terrains d'enquête variés, leurs questionnements épistémologiques, leurs cadres théoriques et conceptuels, leurs grilles d'analyse et d'interprétation, leurs démarches méthodologiques. Il espère constituer une étape dans le renforcement de ce dialogue interdisciplinaire, qui paraît indispensable pour appréhender cette dimension dans toute sa complexité

    Abstract A manual medicine consultation has existed in the canton of Vaud for ten years now, developed within Unisanté, the university centre for general medicine and public health in Lausanne. For a long time, this consultation was the only one of its kind set up in a university setting in Switzerland. Despite a growing interest and a more important development in German-speaking Switzerland and Germany, this approach remains relatively unknown to patients and therapists in the French-speaking cantons. This birthday is an opportunity to put the spotlight on what manual medicine is, to retrace the history of the establishment of the consultation in the canton and to illustrate its practice through a clinical vignette.

    Abstract PRIORISATION OF OLD CRITICALLY-ILL PATIENTS FOR AN ICU ADMISSION There are currently no national or international recommendations for admission decisions to an intensive care unit (icu) for patients over 80 years of age. The decision, whether, or not to admit an elderly patient to intensive care is probably one of the most difficult decisions for an intensivist with the double risk of loss of chance in the event of refusal or non-beneficial care in the event of acceptance. Doubt should always benefit the patient with icu admission in case of prognosis uncertainty. In that case the patient should be reassessed after a few days for tailoring of the level of care. The best criterion for judging the accuracy of decisions is the vital but also the functional prognosis and the long-term expected quality of life for the patient and his relatives. Current and future demographic changes as well as financial constraints justify producing general guidelines in order to ease the decision-making process and reduce practice heterogeneity. The principle of distributive justice must apply in situations of strain on icu bed availability, as in times of a Covid wave.

    Abstract Background: This study hypothesized that patients with extubation failure exhibit a loss of lung aeration and heterogeneity in air distribution, which could be monitored by chest EIT and lung ultrasound. Patients at risk of extubation failure were included after a successful spontaneous breathing trial. Lung ultrasound [with calculation of lung ultrasound score (LUS)] and chest EIT [with calculation of the global inhomogeneity index, frontback center of ventilation (CoV), regional ventilation delay (RVD) and surface available for ventilation] were performed before extubation during pressure support ventilation (H0) and two hours after extubation during spontaneous breathing (H2). EIT was then repeated 6 h (H6) after extubation. EIT derived indices and LUS were compared between patients successfully extubated and patients with extubation failure.
    Results: 40 patients were included, of whom 12 (30%) failed extubation. Before extubation, when compared with patients with successful extubation, patients who failed extubation had a higher LUS (19 vs 10, p = 0.003) and a smaller surface available for ventilation (352 vs 406 pixels, p = 0.042). After extubation, GI index and LUS were higher in the extubation failure group, whereas the surface available for ventilation was lower. The RVD and the CoV were not different between groups.
    Conclusion: Before extubation, a loss of lung aeration was observed in patients who developed extubation failure afterwards. After extubation, this loss of lung aeration persisted and was associated with regional lung ventilation heterogeneity.
