    Abstract Objectives: The coronavirus disease 2019 (COVID-19) has become pandemic and turn in a challenge for Latin America. Understanding the dynamics of the epidemic is essential for decision making, and to reduce the health, economic, and social impacts of the pandemic. The present study aimed to estimate the effective reproductive number (Rt) of Severe Acute Respiratory Syndrome coronavirus 2 (SARS-Cov2) infection during the first 10 days of the outbreak in seven Latin American countries with the highest incidence of cases as of March 23, 2020. Furthermore, we chose to compare the seven countries with Spain and Italy given their history with the virus. Methods: Incidence data retrieved from the COVID-19 data repository by Johns Hopkins University were analyzed. The Rt was calculated for the first 10 days of the epidemic in Brazil, Ecuador, Chile, Colombia, Panama, Mexico, and Peru. Rt estimations were compared with Spain and Italy values for the same interval. Results: The median Rt for the first 10 days of the COVID-19 epidemic were 2.90 (2.67-3.14) for Spain and 2.83 (2.7-2.96) for Italy. Latin American Rt estimations were higher in Ecuador (3.95(3.7-4.21)), Panama (3.95(3.7-4.21)), and Brazil (3.95(3.7-4.21)). The smallest one was observed in Peru (2.36(2.11-2.63)). All Latin American countries had Rt greater than 2. Conclusions: The initial stages of the COVID-19 epidemic in Latin America suggested a high Rt. Interventions such as domestic and international travel restrictions, educational institutions closure, social distancing, and intensified case surveillance should be adopted to prevent the collapse of the health systems.

    Abstract Antecedente: La fiebre del chikunguña es una enfermedad tropical infecciosa, caracterizada por la aparición de fiebre, erupción cutánea y artralgias, cuya incidencia ha aumentado en los últimos años, en Asia y Latinoamérica. Aunque es una enfermedad con baja mortalidad, se ha relacionado con limitación funcional a largo plazo, por su compromiso articular. Objetivo: Llevar a cabo una revisión sistemática de la literatura sobre los factores de riesgo asociados a artralgia persistente en el paciente con fiebre de chikunguña. Metodología: Se buscó información en bases de datos como PubMed, Embase, ScienceDirect, Scopus y Google Académico, con el descriptor de búsqueda (chikungunya fever OR chikungunya) AND (arthralgia). Resultados: Se obtuvieron 4281 estudios de los cuales al final se seleccionaron seis estudios de cohorte por su similitud metodológica en la evaluación de factores asociados y de los que se obtuvieron diferentes resultados en el análisis de datos. Conclusión: No existe información concluyente acerca de factores modificables que permitan predecir la persistencia de artralgias en pacientes infectados por el virus de la fiebre del chikunguña. Hacen falta estudios que permitan evaluar la presencia de dichos factores, fin de disminuir la morbilidad asociada a la artralgia persistente.

    Abstract Introduction: Second-line drugs such as linezolid and daptomycin are used for treatment of vancomycin-resistant Enterococcus (VRE) infections. Objective: A systematic review to evaluate treatment of VRE bacteremia with linezolid versus daptomycin. Methods: A search was done in the databases of Pubmed, Embase, Scopus, ScienceDirect, CENTRAL, Lilacs and Google Scholar to identify studies comparing treatment with daptomycin or linezolid of patients infected by VRE up to July 2015. Result: Only 15 studies were included of a total of 1.307 records. There were no differences between daptomycin and linezolid with respect to mortality at 30 days. Microbiological cure was better with daptomycin (OR: 0.64; 95 % CI: 0.45-0.92), whereas there was no difference between the two antibiotics with respect to clinical cure, need to ICU admission, and the occurrence of adverse effects such as thrombocytopenia, neutropenia and renal failure. Conclusions: No significant differences were observed between daptomycin and linezolid in reference to mortality of patients infected with VRE, although daptomycin treatment produced a faster microbiological cure.

    Abstract Objetivo: Determinar la prevalencia de somnolencia en estudiantes de Medicina y su relación con el rendimiento académico. Materiales y Métodos: Se realizó un estudio descriptivo transversal con una muestra de 131 estudiantes de Medicina de la Universidad Pedagógica y Tecnológica de Colombia, de manera no probabilística, durante el primer semestre del 2015. Se aplicó un cuestionario vía correo electrónico relacionando variables como edad, sexo, semestre actual, promedio académico del último semestre y promedio acumulado, más la aplicación de la Escala de Somnolencia de Epworth validada localmente. Resultados: Las características de la muestra se obtuvieron con un promedio de edad de 20,4±2,36 años. Hombres 52,7%; estudiantes de básicas 50,4% y clínicas 49,6% con un promedio académico del último semestre de 38,6±2,68 y promedio acumulado de 38,6±1,81. Se presenta una prevalencia de somnolencia general del 41,2%, para el área de básicas 46,9% y clínicas 35,4%. Según el género se encuentra una prevalencia de somnolencia diurna excesiva del 43,4% en mujeres y 38,7% en hombres, sin ser estadísticamente significativa la diferencia en estas dos variables. No existe una correlación significativa entre el promedio del último semestre (R=-0,007) y el promedio acumulado (R=-0,077). Conclusiones: La prevalencia de somnolencia es alta en estudiantes de Medicina y su relación con el rendimiento académico no se evidencia en la población estudiada.     Abstract Una entidad poco frecuente es la manifestación neoplásica de las células de Schwann, que comúnmente se reporta en la región vestibular y en otras partes de la cabeza. No obstante, la manifestación de esta neoplasia benigna en miembros inferiores es muy rara. Por ello se reporta el caso clínico de una joven de 23 años de edad, quien, por molestias en la parte posterior de la pierna derecha con una sensación de masa, consultó el servicio de radiología. Luego de hacérsele una ecografía sobre la masa, se optó por una biopsia de la lesión, que evidenció, según criterio histopatológico, la presencia de un schwannoma. Este es el primer caso reportado en Latinoamérica sobre la atipia de esta entidad.

    Abstract Introducción: la tuberculosis (TB) hace parte del grupo de enfermedades infectocontagiosas, producida por Mycobacterium tuberculosis. Aunque se encuentra principalmente en los pulmones, puede afectar otros órganos. Objetivo: comparar los factores socioeconómicos y los métodos diagnósticos relacionados con tuberculosis pulmonar (tuberculosis pulmonar) y extrapulmonar (tuberculosis extrapulmonar) en el departamento de Boyacá durante 2015. Materiales y métodos: se realizó un estudio observacional de tipo retrospectivo. Contó con los datos proporcionados por la Secretaría de Salud de Boyacá ante los eventos de notificación obligatoria de posibles casos de TB reportados en el sistema Sivigila. Se seleccionaron 103 eventos reportados de diagnóstico de TB en el departamento de Boyacá, Colombia; luego se dividieron en grupos de análisis según el tipo de TB, excluyendo los datos duplicados. Resultados: el mayor número de aislamientos fue de tuberculosis pulmonar. En cuanto a los pacientes con tuberculosis extrapulmonar, requirieron mayor atención médica, y los aislamientos más frecuentes fueron, meníngeo, pleural y osteoarticular; adicionalmente, la desnutrición se asoció con la presentación de tuberculosis pulmonar. Conclusión: la generación de nuevos abordajes sociales, culturales y económicos frente al comportamiento de la TB es primordial para controlar esta enfermedad.

    Abstract Introduction: Helicobacter pylori infections affect almost 50% of the world population, constituting a risk factor for benign and malignant gastrointestinal diseases. The increased resistance to antibiotic treatment against this infection represents a dilemma in the search of other therapeutic alternatives.
    Objective: To determine the efficacy of the use of vitaminC and E supplements concomitantly to antibiotic treatment against H. pylori infections.
    Methods: We performed a systematic review on the MedLine (PubMed), Embase and Cochrane Central Register of Clinical Trials (CENTRAL) databases, searching for studies evaluating the efficacy of vitaminC and/or E supplements in the antibiotic treatment of H. pylori infections. The primary outcome was eradication of the infection. The secondary outcome was the adverse effects. The meta-analysis was conducted using the random effects method.
    Results: Ten studies were included and analyzed in two groups. The first group, which was comprised by 973patients, compared the use of supplementation with vitaminC and E, showing that, without discriminating the number of antibiotics used, there was no relationship with the eradication of the infection (OR: 1.98 [95%CI: 0.92-4.29] P=.08). The triple or quadruple antibiotic therapy had no effect on eradication rates either (OR 1.80 [95%CI: 0.64-5.08] P=.26 and OR: 2.84 [95%CI: 0.51-15.56] P=.22, respectively). No effect on the eradication rates was observed either in the group that only assessed the use of vitaminC, comprised by 702patients (OR: 1.17 [95%CI: 0.58-2.31] P=.65). Only four studies reported adverse effects, the most common one being nausea.
    Conclusions: Supplementation with vitaminC and E in the antibiotic treatment against H. pylori has no effect. However, the reviewed studies had several biases and differences in the dosage of the supplements and antibiotics administered.
