    Abstract Background: The sense of gain has gradually become the main evaluation index for the effectiveness of China's deepening reform and is affected by many factors. However, there is no relevant research on the sense of gain of health-care reform (SGHR) and its influencing factors. The purpose of this study was to explore the influence of subjective socioeconomic status (SSS) on SGHR and the mediating role of self-rated health (SRH) between them.
    Methods: Data (25,149 samples total) from China Family Panel Studies (CFPS) in 2018 were included in the analysis. A nonparametric test was used to explore the differences in demographic characteristics of SGHR, and a correlation analysis and mediating effect model were used to explore the influence of SSS on SGHR and the mediating effect of SRH.
    Results: Demographic characteristics such as age, urban and rural areas, educational background, marriage and choice of medical treatment had significant differences in the distribution of perceived acquisition of medical reform. SSS, SRH and SGHR are statistically positively correlated with each other. SSS has a positive statistical correlation with SGHR, and may have an indirect effect through SRH.
    Conclusions: SSS is an important predictor of SGHR, and SRH may play a partially mediating role in SGHR.

    Abstract Acute kidney injury (AKI) has been attracting increasing attention in recent years due to its high mortality and highly associated with chronic kidney disease and end-stage kidney failure. Accumulated evidence indicates that inflammation and reactive oxygen species play important roles in the development of AKI. In fact, various drug molecules with specific antioxidant or anti-inflammatory characters have been demonstrated to be effective therapeutic drugs for AKI. Nevertheless, there are no specific medications until now because some potential drugs are limited by their low stability, poor renal targeting, and rapid clearance. Advanced nanotechnology has been devoted to providing functional nanomaterials for clinical application. Among of them, nanostructures with at least one dimension in ultra-small size have been reported that they possess renal targeting property. Taking advantages of efficiently delivering drugs to kidney or directly serving as antioxidants, such nanostructures have been regarded as promising therapeutic targets for AKI management. This review summarizes the recent advances in designing and constructing nanostructures with at least one dimension in ultra-small size to improve renal targeting and AKI management via scavenging ROS and inhibiting inflammation.

    Abstract Background: Risk assessment of vascular thrombosis in SLE patients with the presence of antiphospholipid antibodies (aPL) remains a challenge. The adjusted global antiphospholipid syndrome score (aGAPSS) has been validated and used to predict aPL-related thrombosis in SLE patients in some countries. Relevant data of aGAPSS in thrombotic evaluation in SLE population from China has not been reported. We aim to validate aGAPSS in thrombosis assessment in Chinese patients with SLE and to explore the correlations of aGAPSS with routine laboratory parameters and their clinical significance as well.
    Methods: A total of 166 consecutive SLE patients were retrospectively analyzed. Multivariate logistic regression analysis was performed to examine the impact of multiple cardiovascular risk factors and laboratory parameters in recurrent thrombosis risk in SLE. ROC was conducted to explore the discriminative ability of aGAPSS and platelet (PLT), activated partial thromboplastin time (APTT), alone or in combination.
    Results: Significantly higher value of aGAPSS was seen in SLE patients with vascular thrombosis. ROC curve indicated that aGAPSS of 3.5 or more had the best diagnostic accuracy for the prediction of aPL-related thrombosis in SLE patients. PLT with cutoff of 187.5 x 109/L and APTT with 37.5 seconds were predictors of aPL-related thrombosis as well. The combination of aGAPSS with PLT and APTT improved AUC compared to aGAPSS alone.
    Conclusions: The aGAPSS could predict the risk of aPL-related vascular thrombosis in SLE patients from China. The combination of aGAPSS with PLT and APTT was first time proved to have better predictive performance in thrombosis risk assessment in SLE.

    Abstract Objective: To assess the willingness and factors influencing the choice of primary healthcare (PHC) institutions among patients with chronic conditions in China.
    Design: A nationwide population-based study with binary logistic regression was conducted and used to estimate the ORs of the influencing factors of health-seeking at PHC institutions using the Anderson model as a theoretical framework.
    Setting: The China Family Panel Studies (CFPS) database.
    Participants: The study sample included 7967 patients with chronic conditions identified from the 2016 and 2018 CFPS databases.
    Results: From 2016 to 2018, the rate of choosing PHC institutions for patients with chronic conditions dropped from 51.0% to 47.7%. The logistic regression results showed that patients with low family income (OR value of >60 000 group was 0.57, 95% CI 0.43 to 0.74), low education level (OR value of bachelor degree or above was 0.54, 95% CI 0.35 to 0.83;), older age (OR value of >65 group was 1.31, 95% CI 1.08 to 1.60;), hypertension and diabetes (OR 1.26, 95% CI 1.13 to 1.41), living in rural areas (OR value of urban was 0.47, 95% CI 0.38 to 0.60), immigrating from rural to urban areas (OR 1.64, 95% CI 1.26 to 2.13), reporting good health (OR value of very good was 1.33, 95% CI 1.05 to 1.68) and those from areas with a high proportion of PHC institutions (OR 1.05, 95% CI 1.02 to 1.07) were more inclined to choose PHC institutions. Conversely, patients with urban employee health insurance (OR 0.62, 95% CI 0.49 to 0.80) and more than one chronic disease (OR 0,83, 95% CI 0.75 to 0.92) preferred choosing a hospital.
    Conclusions: The patients' willingness to choose PHC institutions was low. The health-seeking preference of patients with chronic conditions is derived from medical needs and is influenced by the predisposing factors and tendencies of enabling resources. Measures should be taken to improve the capacity of PHC institutions.

    Abstract Purpose: To evaluate the efficacy and safety of slanted bilateral lateral rectus recession (S-BLRc) for the treatment of convergence insufficiency-type intermittent exotropia (CI-IXT) in children and to probe the relationship of the slanted amount and surgical outcomes.
    Methods: Retrospective study. Fifty-eight patients with CI-IXT, aged 4 to 10 years old, underwent S-BLRc procedures. According to the different slanted amount between the upper and lower poles of lateral rectus, all the patients were grouped: Group A (slanting 1 mm, n = 22), Group B (slanting 1.5 mm, n = 18) and Group C (slanting 2 mm, n = 18). The successful surgical outcome was defined as deviation in the primary position ranging from exotropia< 8
    Results: The average deviations were significantly decreased from - 37.1
    Conclusions: Slanted bilateral lateral rectus recession is an effective and safe procedure for the treatment of CI-IXT in children. S-BLRc can successfully collapse exotropia both at distance and at near, decrease NDD and benefit to gain binocular vision. The correction of NDD was associated with the slanted amount.

    Abstract Objective: In order to improve nursing quality management and protect patient medical safety, it is necessary to change the default mode and completely integrate information technology and nursing quality control utilising lean management.
    Methods: A database was created, the nurse quality control scoring standard was entered into the computer and after the inspection, and various inspection reports were entered into the computer to precisely and promptly preserve data. The computer was then utilised to precisely assess the intensity and quality of nursing work, compute, count, and analyse the stored data, output the quality of nursing work in each department as a report, and adopt lean management for the gathered issues.
    Results: To reach the objective of raising nursing quality, data analysis makes it simple to identify flaws and consistently strengthen the weak points. In order to create an information-based nursing quality control system with a simple and effective method as well as results that are scientific and objective, lean management is brought into the construction process.

    Abstract Abstract Background With the increase in age, the probability of cognitive impairment in the older people is increasing. More and more evidence shows that participating in leisure activities, especially chess and cards, is beneficial to the cognition and mental state of the older people. But the evidence for causal inference is limited. There is a need to conduct a fully powered randomized controlled trial (RCT) and long-term follow-up to test the effectiveness of intervention measures in improving cognitive function and mental state. This paper describes the methodology of a cluster RCT designed to determine the effect of chess and cards leisure activities on the cognitive function of the older people over 60 years old. Methods/design A cluster RCT consisting of 8 clusters will be conducted in four regions of Ningxia, China (Helan, Litong, Qingtongxia, and Shapotou). Clusters will be randomly assigned to the advocacy intervention group, which is also delivered with free leisure activities tools (chess and cards), or the control group. A baseline survey will be conducted before the intervention. A mid-term and final survey will be carried out twelve and twenty-four months after the intervention, respectively. The primary outcome is a change in cognitive function, and the secondary outcomes are changes in anxiety, depression, and loneliness. Discussion The results of this RCT will be helpful to (1) confirm the effectiveness of chess and cards leisure activities in improving the cognitive function of the older people over 60 years old; (2) determine the relationship between the frequency and duration of chess and cards leisure activities and cognitive function; (3) provide evidence of promoting participation in leisure activities through education campaigns and free provision of chess and cards tools; and (4) provide valuable information for successful aging. Trial registration Chinese Clinical Trial Registry: ChiCTR2200066817. Registered on 19 December 2022.

    Abstract To investigate the effects of ocular torsion on horizontal motor fusion and stereopsis in normal adults and to probe the effects of torsion on peripheral fusion, macular fusion and foveal fusion. Twenty-five normal adults aged 30-38 were enrolled in this study. During the synoptophore assessment, the break points (BP) and recovery points (RP) of convergent fusion (CF) and divergent fusion (DF) and random-dots stereopsis were measured and analyzed at intorsion and extorsion of 3°, 5°, 7°, and 9°. According to the different sizes of the retinal areas stimulated by the synoptophore slides, fusion was classified into three categories: peripheral fusion (p-F), macular fusion (m-F) and foveal fusion (f-F). The p-F, m-F and f-F were analyzed and compared at the same torsional angle. There were significant differences in BPCF, RPCF, BPDF and RPDF among different torsion angles (ANOVA, P < 0.05). The Tukey's multiple comparison test showed that BPCF and RPCF of p-F, m-F and f-F decreased significantly at extorsion and intorsion ≥ 5°, compared with baseline (0° torsion) (P < 0.05). Compared with the baseline, BP of DF decreased significantly at torsion angles ≥ 3°, ≥ 5° and ≥ 7° for p-F, m-F, and f-F, respectively (P < 0.05), and RP of DF decreased significantly at torsion angles ≥ 5°, ≥ 9° and ≥ 7° for p-F, m-F, and f-F, respectively (P < 0.05). Comparison among p-F, m-F and f-F revealed significant differences only in BPCF at an intorsion of 3°, extorsion of 3° and the baseline (ANOVA, P < 0.05). There was a significant difference in the proportion of subjects with different sizes of RDS at different torsional angles (Fisher's exact test, P = 0.000). Fine stereopsis was damaged with increasing torsion. Torsion within the normal range of cyclofusion affects the horizontal motor fusion of convergent and divergent fusion and stereopsis. Torsion ≥ 5° should be considered during strabismus surgery for regaining fine binocular vision.
