    Abstract Na podstawie przeglądu aktualnego piśmiennictwa przedstawiono zasady i praktyczne efekty zastosowania wysokoprzepływowego leczenia tlenem (WLT) wśród chorych intensywnej terapii i poddanych zabiegom operacyjnym. Rezultaty stosowania WLT wybiegają poza uzyskanie stabilnego i kontrolowanego stężenia tlenu w powietrzu oddechowym. Dodatkowe efekty związane są z uzyskiwaniem dodatnich ciśnień w fazie wydechu, zjawiska wypłukiwania CO2 i czynnościowego zmniejszania przestrzeni martwej, wzrostu końcowo - wydechowej objętości płuc w wyniku ograniczania mikro-niedodmy i poprawy dystrybucji objętości oddechowej. W wyniku optymalnego nawilżania i ogrzewania mieszaniny wdechowej dochodzi do zmniejszenia oporów wentylacji i pracy oddychania. Opisane efekty WLT skłaniają do prób jej wykorzystywania nie tylko jako narzędzia tlenoterapii biernej, ale również jako urządzenia alternatywnego dla wentylacji nieinwazyjnej lub wczesnej intubacji. Zakres zastosowań ocenionych w piśmiennictwie dotyczy ostrej hypoksemicznej niewydolności oddechowej, wstępnej fazy ARDS, POChP, okresu okołooperacyjnego oraz zastosowań podczas procesów diagnostycznych (gastroskopia, bronchoskopia). Szczególną formą WLT jest poddawana aktualnie ocenie adaptacja metody dla chorych z tracheostomią, usprawniająca głównie procesy nawilżania mieszaniny oddechowej. WLT wymaga dalszych ocen w dużych, randomizowanych badaniach, jednak dotychczasowe efekty stosowania są zachęcające.

    Abstract This research analysed early neonatal outcomes of complex and simple gastroschisis following planned elective preterm delivery in relation to prenatal ultrasound assessment of bowel conditions. A retrospective study of 61 neonates with prenatal gastroschisis diagnosis, birth, and management at a single tertiary centre from 2011 to 2021 showed a 96.72% survival rate with no intrauterine fatalities. Most cases (78.7%) were simple gastroschisis. Neonates with complex gastroschisis had longer hospital stays and time to full enteral feeding compared to those with simple gastroschisis-75.4 versus 35.1 days and 58.1 versus 24.1 days, respectively. A high concordance of 86.90% between the surgeon's and perinatologist's bowel condition assessments was achieved. The caesarean delivery protocol demonstrated safety, high survival rate, primary closure, and favourable outcomes compared to other reports. Prenatal ultrasound effectively evaluated bowel conditions and identified complex gastroschisis cases.

    Abstract A simulation model and the results of experimental tests of a vibration generator in applications for the hot-dip galvanizing process are presented. The parameters of the work of the asynchronous motor forcing the system vibrations were determined, as well as the degree of unbalance enabling the vibrations of galvanized elements weighing up to 500 kg to be forced. Simulation and experimental tests of the designed and then constructed vibration generator were carried out at different intensities of the unbalanced rotating mass of the motor. Based on the obtained test results, the generator operating conditions were determined at which the highest values of the amplitude of vibrations transmitted through the suspension system to the galvanized elements were obtained.

    Abstract The aim of the study was to investigate whether the use of pre- and postoperative gabapentin can decrease postoperative pain, morphine consumption, anxiety and side effects, as well as improve patient satisfaction. A total of 56 patients, 9−17 years of age, undergoing a modified Ravitch procedure, were randomised (allocation ratio 1:1) to receive multiple perioperative doses of gabapentin (preoperatively 15 mg/kg, postoperatively 7.5 mg/kg, two times per day for three days) or a placebo. All the patients received intravenous infusion of morphine, paracetamol and non-steroidal anti-inflammatory drugs. Metamizole was given as a “rescue drug”. The observation period included the day of surgery and three postoperative days. The primary outcomes were postoperative pain intensity (at rest, during deep breathing and coughing). Additional outcomes included the consumption of morphine, the total number of doses of metamizole, anxiety, postoperative side effects and patient satisfaction. Median average and maximal pain scores (on the day of surgery and on the second postoperative day) were significantly lower only in the gabapentin group at rest (p < 0.05). Compared to the placebo group, gabapentin treatment reduced the demand for morphine on the first postoperative day (median 0.016 vs. 0.019 mg/kg/h; p = 0.03) and the total number of metamizole doses (median 1 vs. 2 p = 0.04). Patient satisfaction was significantly greater in the gabapentin group (median 10 vs. 9; p = 0.018). Anxiety and postoperative side effects were similar in both groups (p > 0.05). Pre- and postoperative gabapentin administration as part of a multimodal analgesic regimen may decrease postoperative pain, opioid consumption and demand for a “rescue drug”, as well as improve patient satisfaction.

    Abstract Sufentanil, a potent α-1 agonistic opioid, was synthesized in mid-1970s. It was introduced into clinical practice ten years later, gaining some popularity over the last twenty years. A piperidine derivative, sufentanil has been reported to be 6-10 times more potent than fentanyl, depending on the route of administration; it has been registered for intravenous, epidural and subarachnoid administration. Its reported off-label use has included intra-articular and intranasal administration; moreover, it has been applied as an adjunct in peripheral blocks. In the review, contemporary uses of sufentanil, together with detailed pharmacokinetics and pharmacodynamics are presented. The author concludes that the limited side effects of sufentanil, together with its attractive pharmacokinetic profile, should promote its wider use in clinical practice.
