    Abstract Conocida la condición de vulnerabilidad del Perú a enfrentar un evento sísmico de gran intensidad y en consecuencia un Tsunami, este estudio realiza un análisis del nivel de exposición de las Instituciones Educativas (IE) según su ubicación geográfica, para lo cual se utiliza la plataforma geoespacial Sistema de Información para la Gestión del Riesgo de Desastres (SIGRID), que alberga la información oficial, actualizada, técnico y científica producida en el Perú. La metodología implica dividir la provincia constitucional del Callao en 3 sectores delimitados a los que al hacer la superposición de capas de información para Microzonificación sísmica se obtuvo que de las 757 IE expuestas el 69% están ubicados en Zona II Depósitos de arena de compacidad media a densa y el 9% ubicados en zonas tipo IV y V Taludes inestables con fuerte pendiente y Depósitos de escombros y/o desechos respectivamente. Para el análisis frente a Tsunamis se obtuvo que de las 149 IE ubicadas en zona de inundación el 31.5% serian inundados a consecuentes de un sismo de intensidad 8.5 Mv. y el 68.5% a consecuencia de un sismo de intensidad 9.0 Mv.

    Abstract Antecedentes: La mortalidad materna es un indicador de calidad en la atención de las embarazadas y se considera un problema de salud pública por las repercusiones que tiene para los infantes y sus familias. Su principal factor desencadenante es el embarazo de alto riesgo. Objetivo: Identificar las condiciones que llevaron al diagnóstico de embarazo de alto riesgo y describir las alteraciones de salud en las gestantes de un hospital de nivel III. Métodos: Estudio retrospectivo en embarazadas que consultaron por embarazo de alto riesgo durante el 2017. Resultados: La edad media de las pacientes fue 26 años; 54% procedían del área urbana; el 72.7% pertenecían al régimen subsidiado, el antecedente personal más frecuente fue la cirugía previa (20.9%); la media de controles prenatales fue de 4.7 iniciados alrededor de la semana 11.7. La condición que originó el diagnóstico de embarazo de alto riesgo fue el control prenatal insuficiente con un 25.6%, determinado durante la gestación. El tipo de parto más común fue el vaginal espontáneo con un 38.8% y no se presentaron muertes maternas. Conclusión: De acuerdo con perfil epidemiológico de las gestantes con embarazo de alto riesgo, se resalta el control prenatal insuficiente como principal causa, lo que sugiere reforzar o rediseñar las estrategias de atención por parte de los actores involucrados que permitan implementar controles de calidad en la caracterización y cuidado de las gestantes del Departamento.

    Abstract Untreated HCV mono and HCV/HIV coinfected women have lower degrees of liver fibrosis (LF) compared to men. Direct acting antiviral (DAA) therapy attains viral eradication in > 90% of patients with progressive LF decline in parallel. Gender-related differences in LF regression in the long term assessed by non-invasive liver fibrosis markers (NILFM) in HCV mono and HCV/HIV coinfected after DAA treatment have not been explored so far. 374 HCV-infected adult patients, 214 of them HCV/HIV coinfected, were followed-up for 24 months after starting DAA therapy. LF was assessed by NILFM: transient elastometry (TE) and several biochemical indexes (APRI, Forns, FIB-4). Men had significantly more advanced LF at baseline than women assessed by NILFM. No LF differences at baseline in age, HIV coinfection course (CD4, HIV viral load), and HCV features (HCV viral load, genotype) were detected. No significant gender differences in LF decline after comparing 24-month and baseline LF values were observed. LF changes after DAA therapy were similar in HCV mono and HCV/HIV coinfected patients and in both sexes. Gender did not influence the course of LF decline after DAA assessed by NILFM: TE (P = 0.8), APRI (P = 0.9), Forns (P = 0.4) and FIB-4 (P = 0.7) by multivariate analysis. No gender differences in the 24 month LF decline after DAA with independence of having HCV mono or HCV/HIV coinfection were found.

    Abstract The possible relationship between HAART and the development of sexual disturbances of HIV-infected patients remains yet unresolved because of the inconsistency of the results of the different studies. To analyze the current knowledge on this topic, MEDLINE files were searched for articles dealing with any manifestation of sexual dysfunction in the HAART era. Selected references from these articles as well as communications to the main HIV meetings were also reviewed. Sexual dysfunction seems to be a very common event after the introduction of HAART. The average prevalences of sexual dysfunction among the different studies was 51%, erectile dysfunction 46%, decreased libido 44%, ejaculatory disturbances 39% and orgasmic disorders 27%. These disturbances seemed to be more common in patients treated with protease inhibitors. Several relevant questions related to sexual dysfunction in these patients are addressed in this review, including the possible pathogenic mechanisms involved. Despite the inconsistent results among the studies, the data that support a direct or indirect role of HAART in the generation of these disturbances seem to exceed the data that do not support it. As a conclusion, antiretroviral therapy, particularly protease inhibitors, seems to be to some extent directly or indirectly related to sexual dysfunction through different mechanisms.
