    Abstract Purpose: This study examined the health-related quality of life among children who have undergone surgery for tetralogy of Fallot (TOF) compared to healthy children.
    Design: Cross-sectional study.
    Methods: The study was carried out at Children's Heart Hospital and Primary Healthcare Centers in Sulaymaniyah, Iraq, between July 1, 2022, and November 1, 2022. The study involved 400 participants, comprised of 200 parents of healthy children and 200 parents with children with repaired TOF. Data collection involved utilizing a 5-point Likert Scale questionnaire administered through direct face-to-face interviews with the parents of the children.
    Findings: Children with surgically repaired TOF had markedly lower mean scores (
    Conclusions: Repaired TOF children exhibited notable deficiencies across all dimensions of QoL compared to healthy children. Moreover, SES emerged as a significant determinant influencing the QoL outcomes of repaired TOF children.
    Clinical evidence: Despite undergoing corrective surgery for TOF, the QoL in children with repaired TOF continues to be lower than that of their healthy counterparts. Notably, the economic and educational status of the family significantly impacts the way these children perceive and experience their QoL. This finding underscores the critical significance of factoring in socioeconomic elements when addressing the well-being of this particular group of children.

    Abstract Vitamin D (Vit D) is essential in maintaining calcium homeostasis and other body processes. It has been widely studied how Vit D affects cell cycle pathways and how it affects the development and prevention of breast cancer (BC). This study aimed to determine Vit D insufficiency linkage to the development of BC. In this case-control study, 130 women (65 BC patients and 65 healthy controls) aged 20-60 years who visited Shar Hospital Breast Center in Sulaimaniyah, Iraq, from December 2021 to May 2022 were included. Patients were selected after their diagnosis had been verified by breast ultrasound, mammography, and core biopsy. The ELISA test was used to measure the concentrations of serum Vit D and expressed in ng/ml. The results showed that the BC patients had considerably lower serum Vit D levels that were <20 ng/L in 66.1% (n=43) and 43.1% (n=28) in healthy controls. Compared to the control group (20.2±8.7), the mean Vit D level in BC patients was lower (17.8±8.6). A logistic regression test revealed a substantial increase in the risk of BC for low-level Vit D concentrations below 20 ng/L (OR 2.59, 95% CI 1.24-5.38; P=0.009). Vit D is still a significant risk factor for boosting the likelihood of developing BC after age and body mass index (BMI) adjustments (AOR 2.30, 95% CI 1.1-4.86; p=0.03 and AOR 3.67, 95% CI 1.55-8.7; p=0.002 for BC patients and controls, respectively). According to the outcomes of our investigations, we concluded that Vit D insufficiency raises the risk of BC among women in Sulaimaniyah, Iraq.

    Abstract Objectives: The study was aimed at using speckle tracking echocardiography as a novel technique to diagnose right ventricular failure (RVF) in children with total correction of tetralogy of Fallot (TOF) through surgery.
    Methods: A quasi-experimental study was performed at the Children's Heart Hospital of Sulaimani for 9 months. A total of 150 children with completely repaired TOF were enrolled to investigate RVF. Conventional echocardiographic data were recorded, including right ventricular (RV) ejection fraction (EF), tricuspid annular plane systolic excursion (TAPSE), myocardial performance index (MPI), and RV end-systolic and diastolic volume (RVESV and RVEDV). Additionally, speckle tracking was performed for the regional and longitudinal strain and strain rate in four-chamber apical view. RVF diagnosis was determined on the basis of electrocardiography measurement of P-wave dispersion, T-wave dispersion, and QRS duration.
    Results: Children with repaired TOF who were diagnosed with RVF through conventional echocardiography exhibited abnormalities with respect to children with normal RV function, including a TAPSE of 1.3 ± 0.11 cm, RVEF of 35.5 ± 6.72, RVESV of 69.8 ± 15.13 ml, RVEDV of 110.1 ± 14.13 ml, MPI of 0.60 ± 0.12, and Pmax of 52.4 ± 14.08. The use of speckle tracking in RVF diagnosis revealed a relatively lower longitudinal strain and strain rate (-12.1 ± 2.3 and -0.9 ± 0.3, respectively) in the children with RVF. Moreover, longitudinal right ventricular strain was positively correlated with TAPSE (r = 0.656) and EF (r = 0.675), and negatively correlated with RVEDV (r = -0.684), RVESV (r = -0.718), MPI (r = -0.735), and Pmax (r = -0.767).
    Conclusions: The application of speckle tracking with the longitudinal RV strain and strain rate to estimate RV function in children with repaired TOF is a new advanced method that, compared with conventional echo, significantly improves the diagnosis of regional myocardial deformations and cardiac muscle motion velocity.

    Abstract This epidemiological study was conducted to compare the climatic conditions of four different districts of Khyber Pakhtunkhwa, Pakistan, in terms of the prevalence of parasitic infection Paramphistomum in cows and buffaloes. A total of 2400/cows and buffaloes (200/per month and 50/per district) fecal samples were collected. The samples were examined for Paramphistomum egg contamination by direct microscopic examination and sedimentation methods. The infectivity ratio was 17.5% in cows and 17% in buffaloes; the higher rate of infection was recorded in autumn with 41% in cows and 36% in buffaloes. Whereas the lowest ratio was 6.5% in cows and 7% in buffaloes recorded during spring. It was also noted that a high rate of prevalence was recorded in older animals (higher than 2 years of age) than younger (less than 2 years of age), while gender-wise, it was observed that male animals were more affected than females. Prevalence was higher in cows in Mardan district (19.30%) followed by Nowshera (19.10%) and Swabi (17.80%) districts while lowest in Charsadda district (13.80%) where in buffaloes the higher prevalence was recorded in Swabi (18.80%) followed by Mardan (18.60%) then Charsadda (16.80%) while lowest in Nowshera (13.80%).

    Abstract The global spread of the COVID-19 pandemic, its threat to people’s health and its indirect impacts through the subsequent measures taken by governments to control the disease have led to uncertainty and huge disruption in the way people used to live with a negative impact on mental health and wellbeing. The current study estimate prevalence of the common mental health disorders among a sample of the adult population in the Kurdistan Region during the pandemic. Through an online survey using DASS-21 to measure depression, anxiety, and stress data of 548 adult participants were collected during April 2020 and analyzed. The sample was mainly from Sulaimani (89%), mean age was 37.9 (SD 13.5) and male/female ratio was 1. Prevalence of any severity levels of depression, anxiety, and stress was 45%, 47%, and 18% respectively. In regression analysis, notably female sex was a significant independent factor for higher levels of depression (coefficient 1.89, p<0.05), anxiety (coefficient 2.19, p<0.001), and stress (coefficient 1.52, p<0.05). Postgraduate education and other occupations were also significantly associated with depression (coefficient 3.81, p<0.05; coefficient 2.39, p<0.05 respectively), anxiety (coefficient 2.92, p<0.05; coefficient 4.1, p<0.001 respectively) and stress (coefficient 3.83, p<0.001; coefficient 4.1, p<0.001 respectively). The study indicates high levels of common mental health disorders during the pandemic and calls for public health measures to promote the mental health and resilience of the population.

    Abstract Antibacterial resistance among uro-pathogens has become a global issue for bacterial infection treatment. The antibacterial activity of various plant extracts and silver nanoparticles (AgNPs) derived from medicinal plant extract was evaluated against different drug-resistant uro-pathogens isolated from clinical patients by Agar Well Diffusion and Minimum Inhibitory Concentration (MIC) assays. A total of 100 urine samples were collected from patients admitted to the hospital followed by the identification of Pseudomonas aeruginosa, Escherichia coli, Staphylococcus aureus, Proteus mirabilis, Enterobacter aerogenes, and Klebsiella pneumonia through colony morphology, gram staining, and biochemical characterization. All isolates were highly resistant to all the tested drugs except Amikacin, Gentamicin, and Imipenem. Organic extracts of clove have potent antibacterial activity against all drug-resistant isolates. Interestingly P. aeruginosa was sensitive to all inorganic extracts. These results revealed that the organic extracts of medicinal plants showed more consistent antimicrobial activity compared to their aqueous fraction. Furthermore, the green synthesis of AgNPs derived from clove extract showed strong activity against all tested pathogens followed by green synthesis of AgNPs derived from garlic, ginger, kalwonji, turmeric, and thymus extract respectively compared to medicinal plant extract. Moreover, MIC results showed that AgNPs derived from medicinal plant extract indicate higher efficiency in inhibiting bacterial growth than the medicinal plant extract. The current finding shows that AgNPs derived from plant extract can be recommended as a good alternative for treating the resistant uro-pathogens strains as compared to synthetic drugs and will be considered an attractive candidate for future biological applications.

    Abstract Congenital anomalies or birth defects can be acquired during the fetal stages of development or from the genetic makeup of the parents. Congenital anomalies are important causes of infant and childhood illness and disability. Little is known about incidence and types of these anomalies in Iraqi Kurdistan. Therefore, this study was undertaken to estimate the incidence and types of congenital anomalies in Sulaimaniyah city. The study was carried out on the hospital's records of all newborns registered as having a congenital anomaly. The records of 586 neonates with congenital anomalies were analyzed from a total of 178,954 live broths that occurred during 4 years in the city. The data was obtained from the statistics section of maternal and a child unit of the Preventive Health Department. The overall incidence of all types of congenital anomalies over the four years was 3.3/1000 live births. There was a statistically significant difference in incidence between males and females over the four years, male to female risk ratio 1.2 (95% CI 1.02-1.42, P= 0.03). The commonest congenital anomalies affected the cardiovascular system accounting for 24% followed by those of the nervous system with 16%. Down syndrome accounted for 14% of all anomalies and cleft lip/palate for 11%. Types of anomalies were statistically associated with low birth weight and maternal age. The study indicates that the incidence of congenital anomalies is not high in the region; however, more extensive studies are required to give a more realistic incidence.

    Abstract Organic acids are specific groups of chemicals that contain COOH in their structures. They have properties of weakly acidic nature and dissolves completely into protons when present in aqueous solutions. In biological systems, organic acids are involved in various activities such as important intermediates in various metabolic cycles, antimicrobial compounds, and prediction of maturity indices. Due to these properties, organic acids are important phytochemicals that have a wide range of commercial applications like animal feeds utilization, improve plant quality, bactericidal and bacteriostatic potential, as well as in the pharmaceutical industry for numerous preparations. In pharmacokinetics, organic acids are primarily involved in the absorption of pH-dependent drugs. Different techniques have been employed for the extraction, purification, and quantification of organic acids. Among them, microwave-assisted extractions (MAE) and ultrasound-assisted extractions (UAE) are the chiefly used techniques where MAE is one of the most valuable methods for isolating different compounds from plant materials for industrial and analytical purposes. Moreover, bioactive chemicals such as natural organic acid from plants and other sources are important and referred as "lifespan essential"since they havea beneficial effect on healthy aging and Alzheimer's disease (AD). Therefore, dietary organic acids and avoiding risk factors, will reduce the risk of neurodegenerative diseases such as AD and increase a healthy lifespan. Thus, the present study aimed to understand the phytochemistry, biological activities, industrial applications, possible interaction with other drugs/foods supplements,techniques of extraction, purification and fractionation, techniques of identification and quantification, the effect of food processing,and molecular mechanisms of organic acid to attenuate AD.
