    Abstract In this paper we describe the main morphological characteristics that distinguish the full-grown larva of Athripsodes taounate taounate, an Iberic-Maghrebian endemic. The conspecificity of the larva and adult was confirmed by DNA analysis. Morphological features that discriminate it from the described Iberian-Maghrebian species of Athripsodes are given.

New country record, Trinidad and Tobago, as well as chromatic and morphological variations are reported to Gnomidolon incisum Napp Martins, 1985 (Hexoplonini), and Compsibidion vandenberghei Heffern, Santos-Silva Nascimento, 2018 (Neoibidionini, Neoibidionina). Additionally, a key to species of Corimbion Martins, 1970 are provided, and the two new species of Neocompsa Martins, 1965, and the new species of Heterachthes Newman, 1840 are included in a previous key.

    Abstract The tropical Andes Mountains exhibit high levels of endemism and spatial turnover in the distribution of species. The lizard genus Stenocercus Dumril Bibron, 1837, contains 76 species and most of them occur in the tropical Andes, reaching elevations up to 4,000 m. We describe four new species of Stenocercus based on the examination of newly collected material from the Amazonian slopes of the Peruvian Andes. Stenocercus asenlignus sp. nov. inhabits the premontane forest of northern and central Peru, departments of Amazonas, San Martn and Hunuco, at elevations between 1,500 and 2,036 m, in the basins of the Mayo, Huayabamba, and Huallaga rivers. Stenocercus leybachi sp. nov. inhabits the premontane forest of the upper Huallaga River, Hunuco department in central Peru, at elevations between 824 and 1,270 m. Stenocercus qalaywasi sp. nov. was collected in a small village at the headwaters of the Mantaro River, Junn department in central Peru, at an elevation of 2,587 m. Finally, S. nigrocaudatus sp. nov. inhabits the montane forest from extreme northern Peru, Cajamarca department, at elevations of 1,700 and 1,892 m. These species are characterized by having granular scales on the posterior surface of the thighs, relatively short tail, caudals spinose, two caudal whorls per autotomic segment, and the ability to change coloration from green to brown or gray; they differ from other species of Stenocercus in scutellation features and color pattern.

    Abstract The larva of Stenophylax fissus (McLachlan 1875) is redescribed, illustrated, and compared with morphologically similar Limnephilidae larvae from the Iberian Peninsula. In addition, the sequence of the mtCOI gene in the barcode region of one individual is reported. Likewise, the barcode and the conspecificity between two larvae and one female of the non-sequenced species S. crossotus are given. In addition, the high genetic diversity among S. sequax populations is highlighted, suggesting a complex of cryptic species. Finally, a diagnostic matrix of the known larvae of Stenophylax species from the Iberian Peninsula is provided.

    Abstract Background: Genetic causes are increasingly recognized in patients with focal segmental glomerulosclerosis (FSGS), but it remains unclear which patients should undergo genetic study. Our objective was to determine the frequency and distribution of genetic variants in steroid-resistant nephrotic syndrome FSGS (SRNS-FSGS) and in FSGS of undetermined cause (FSGS-UC).
    Methods: We performed targeted exome sequencing of 84 genes associated with glomerulopathy in patients with adult-onset SRNS-FSGS or FSGS-UC after ruling out secondary causes.
    Results: Seventy-six patients met the study criteria; 24 presented with SRNS-FSGS and 52 with FSGS-UC. We detected FSGS-related disease-causing variants in 27/76 patients (35.5%). There were no differences between genetic and non-genetic causes in age, proteinuria, glomerular filtration rate, serum albumin, body mass index, hypertension, diabetes or family history. Hematuria was more prevalent among patients with genetic causes. We found 19 pathogenic variants in
    Conclusions: FSGS-related variants were detected in a substantial number of patients with SRNS-FSGS or FSGS-UC, regardless of age of onset of disease or the patient's family history. In our experience, genetic testing should be performed in routine clinical practice for the diagnosis of this group of patients.

    Abstract We evaluated the role of liver biopsy in the management of chronic hepatitis C in HIV-infected persons. Patients included had abnormal alanine transaminase (ALT) levels, detectable HCV RNA, and an interpretable liver biopsy. Demographic, epidemiologic, clinical, laboratory, histological, and therapeutic data were recorded in all patients. We also registered the clinical diagnosis of cirrhosis (previous to biopsy) and whether the biopsy result deferred the decision of initiating therapy. During the 33-month duration of the study, 112 patients were included. The degree of fibrosis in liver biopsies was none or mild (F0 or F1) in 47 patients (42%) and was significant or severe in 65 (58%). Seventeen patients (15%) had histological cirrhosis. By logistic regression analysis, only portal hypertension (odds ratio [95% confidence interval], 5.3 [1.05-25.9], P = 0.04) was independently associated with significant fibrosis. Overall, cirrhosis was predicted before biopsy in 29 patients (26%) by the caregiving physician, but only 8 of these were confirmed histologically. The clinical prediction of cirrhosis before biopsy had a sensitivity of 47%, a specificity of 78%, a positive predictive value of 28%, and a negative predictive value of 89%. Histological findings changed the decision to initiate HCV therapy in 19 patients (17%) because of little or no fibrosis. Liver biopsy is a useful tool in the management of HCV-HIV-coinfected persons. In addition to allowing grading and staging of the disease far better than any other method or combination of methods, it is important for making management decisions for patients coinfected with HCV and HIV.

    Abstract Background: Malnutrition is frequent in peritoneal dialysis (PD) patients, but the contribution of gastrointestinal (GI) dysfunction has not been well established.
    Methods: We studied GI function in 49 stable PD patients to ascertain its relationship with malnutrition. After an overload fat diet, fecal fat, sugar, starch and nitrogen, intestinal protein permeability (alpha(1)-antitrypsin fecal clearance [C-alpha(1)-AT]), fecal chymotrypsin (CT), GI hormones and gastrin, pepsinogen I and II, cholecystokinin (CCK), gastrin releasing peptide (GRP), and neuropeptide Y (NPY) were measured. Vasoactive intestinal polypeptide (VIP), substance P (SP), and tumor necrosis factor (TNF-alpha) and biochemical nutritional markers were evaluated.
    Results: All patients showed high fecal sugar. Elevated fecal nitrogen was found in 21 patients, 6 with high C-alpha(1)-AT. High fecal starch levels appeared in 21, fat in 20, and low fecal CT in 39 patients. These determinations showed inverse relation with nutritional markers. Increased fecal C-alpha(1)-AT values were associated with lower serum albumin. Fecal CT values showed a negative linear correlation with serum albumin and were inversely associated with retinol-binding protein, normalized protein nitrogen appearance, and serum iron. High plasma levels of pancreatic stimulating hormones were found: gastrin, CCK, and VIP. These levels were higher in patients with a worse pancreatic exocrine function. Higher values of other GI hormones, gastrin, pepsinogen I and II, CCK, GRP, and TNF-alpha. Normal concentrations of NPY, VIP, and PS were observed.
    Conclusion: GI abnormalities (malabsorption, maldigestion, pancreatic dysfunction, and protein losing enteropathy) are present in an important number of PD patients. These features are negatively associated to nutrition.

    Abstract Local people's involvement in the management of conservation initiatives is central to ongoing debates on the relative merits of distinct biodiversity conservation models. Since different governance models provide distinct opportunities for local people to participate in the management of protected areas, their knowledge of these governance models and motivation to collaborate will vary. This paper analyses cognisance and participation in (1) government-imposed biosphere reserves and (2) community conservation areas, in which ecotourism projects take place. Qualitative and quantitative data (n = 205) were gathered in two indigenous communities in Yucatan Peninsula, Mexico. Whereas local cognisance of community-driven conservation initiatives is not always greater than cognisance of government-imposed Biosphere Reserves, local participation is always greater. Cognisance of and participation in conservation initiatives depend on the management approach, extent of external resource support, and a community's social organization. Gender and land holding status influence access to information about conservation initiatives, since men with land rights had more access than other people. More participatory mechanisms for decision-making and direct communication strategies between managers and local people are required to improve communities’ involvement in conservation.
