    Abstract Intussusception in adults represents 1% of bowel obstructions and up to 0.02% of all hospital admissions. Amongst these, colo-colic intussusception of the descending colon forms the rarest of causes due to the fixed nature of the descending colon. Most of adult intussusceptions follow a lead point and are commonly due to colonic malignancy which may get missed on pre-operative evaluation. Surgery is usually warranted as these patients are usually symptomatic and at risk of vascular compromise, leading to perforations and obscure malignancies. We present a case of laparoscopic limited hemicolectomy and primary anastomosis in a middle-aged male who presented with colo-colic intussusception, which appeared to be following a malignant mass on imaging and lipoma on colonoscopic biopsy done twice. Keeping in mind the possibility of a malignant lead point, no attempt was made to reduce the intussusception and a vessel first approach with 5 cm margin on either side was performed.

    Abstract Background: Post-burn contracture is a common sequele occurring after burn. Upper limb contracture like axilla is also occurring more commonly because it is most mobile part of body and likely to involve in burn. Post-burn contracture of axilla leads to functional deficit as it limits the movement of upper extremity mainly abduction and extension. Patients with axillary contracture are unable to utilize his upper limb because he is unable to take the hand at required site. Aims and Objectives: The objective of this study was to study various operative strategies for patients presenting with post-burn axillary contracture. Materials and Methods: Twenty patients presenting with post-burn axillary contractures were taken in our study. Results: In our study of 20 patients of post-burn axillary contracture, five patients had Grade I (25%), Grade II 05 (25%), Grade III 01 (5%), and Grade IV 09 (45%) contractures. Regarding surgical procedure, Z-plasty was done in 8 patients (40%), release with skin grafting 10 patients (50%), and local flap done in 2 patients (10%). In our study, the mean degree of range of motion achieved after release of axillary contracture was 120°. In our study, 2 patients (10%) developed re-contracture, 1 patient (5%) had a graft failure while 85% of the patients had no complications. Conclusion: In the present study of 20 cases of management of post-burn axillary contracture, it is reasonable to conclude that the prevention of contracture is of utmost importance. The choice of surgical procedure for reconstruction of post-burn axillary contractures can be made according to the pattern of scar contracture and the state of the surrounding skin. The choice of a flap should have priority over the skin graft due to the superior functional and esthetic results of flaps. The results were satisfactory in our study in most cases with regard to quality of life, coverage, and range of motion.

    Abstract Paediatric acute liver failure (PALF) is often characterised by its rapidity of onset and potential for significant morbidity and even mortality. Patients often develop multiorgan dysfunction/failure, including severe acute kidney injury (AKI). Whilst the management of PALF focuses on complications of hepatic dysfunction, the associated kidney impairment can significantly affect patient outcomes. Severe AKI requiring continuous kidney replacement therapy (CKRT) is a common complication of both PALF and liver transplantation. In both scenarios, the need for CKRT is a poor prognostic indicator. In adults, AKI has been shown to complicate ALF in 25-50% of cases. In PALF, the incidence of AKI is often higher compared to other critically ill paediatric ICU populations, with reports of up to 40% in some observational studies. Furthermore, those presenting with AKI regularly have a more severe grade of PALF at presentation. Observational studies in the paediatric population corroborate this, though data are not as robust-mainly reflecting single-centre cohorts. Perioperative benefits of CKRT include helping to clear water-soluble toxins such as ammonia, balancing electrolytes, preventing fluid overload, and managing raised intracranial pressure. As liver transplantation often takes 6-10 h, it is proposed that these benefits could be extended to the intraoperative period, avoiding any hiatus. Intraoperative CKRT (IoCKRT) has been shown to be practicable, safe and may help sicker recipients tolerate the operation with outcomes analogous with less ill patients not requiring IoCKRT. Here, we provide a comprehensive guide describing the rationale, practicalities, and current evidence base surrounding IoCKRT during transplantation in the paediatric population.

    Abstract Retinal vasculitis (RV) refers to an entity in which the retinal vasculature is inflamed, frequently with indications of inflammation elsewhere in the eye. Non-infectious RV can be idiopathic or associated with systemic disease, ocular conditions, and malignancy. It can also be classified based on the vessel affected: artery, vein, or both. Due to the lack of strong evidence-based treatment trials and algorithms for RV, physicians must often rely on their experience, which creates great variability in treating this entity. This article provides an overview of various treatment modalities used in the management of non-infectious RV, with a focus on immunomodulatory therapies. We outline a potential stepwise approach of starting with steroids to control the acute inflammation and subsequently changing to immunomodulatory therapy (IMT) for long-term treatment.
