    Abstract Species of the genus Cryptotendipes Beck et Beck in Florida are reviewed. Previous Florida records of C. casuarius (Townes) are shown to be misidentifications of C. emorsus (Townes). One species new for the state, C. pseudotener (Goetghebuer), is recorded and a new species, C. rutteri Epler, is described from pharate pupae and associated larval exuviae. Thus, a total of three species are known from the state. Keys are provided for adult males, pupae and larvae of these three species.

    Abstract A new species of Dicrotendipes is described in all life stages from Florida. Adults of this new species are nearly identical to D. modestus (Say); pupae are similar to D. modestus, D. neomodestus (Malloch) and D. tritomus (Kieffer); while the larvae are unique and were keyed by Epler (1992, 1995, 2001) as Dicrotendipes sp. A. The taxonomic status of D. modestus and D. pulsus (Walker) is discussed.

    Abstract Background: Pica is common in pregnancy and is often felt to be benign. The following case of severe pica presenting without anemia is unusual in its presentation, laboratory findings, and treatment.
    Case: A 31-year-old multiparous woman at 37 0/7 weeks of gestation presented with esophagitis and gastritis secondary to laundry detergent consumption. She had borderline anemia (hemoglobin of 11 g/dL and hematocrit of 37%, mean corpuscular volume 80%) but was severely iron-deficient (serum ferritin 7 micrograms/dL). Parenteral iron infusion was associated with dramatic resolution of her cravings within 36 hours of treatment.
    Conclusion: Pica may be related to deficient iron stores in the absence of anemia and can result in serious morbidity. Parenteral iron may be associated with rapid pica resolution in symptomatic pregnant patients.

    Abstract Objectives: Quaternary care centres have an imperative to serve as hospitals of last resort and must also meet professional quality targets. We developed a high-risk committee (HRC) to evaluate cases meeting pre-defined predicted risk cut-offs as a part of an overall quality improvement drive.
    Methods: We describe the structure, outcomes and effects of the Penn HRC. Using propensity-matching, we investigated whether the committee modifies or screens risk. We used multivariable analysis to examine the impact of unmeasured variables on clinical outcomes in this cohort.
    Results: Institutional predicted and observed mortality had already been in decline prior to HRC institution in 2017, due to a multi-faceted quality improvement initiative. Between 2017 and 2020, the HRC discussed 205 patients with a median predicted risk of mortality of 10.6% (range 0.4-66%). Coronary artery bypass grafting was the most commonly presented operation. A total of 155 patients underwent operation (risk 10.3%), 12 had surgery deferred for optimization (risk 6%), 50 had surgery declined (risk 11.7%) and 12 patients had a deferred decision for further investigation. Overall 30-day survival was 86% for the entire cohort and 89% for operated patients. A matched analysis of similar patients prior to and following the HRC showed that the HRC did not directly modify outcomes. Most patients had better than expected survival (observed:expected mortality < 1). Predicted risk did not predict 30-day mortality among this high-risk cohort.
    Conclusions: HRCs serve as an important element in quality improvement by encouraging a thoughtful approach and channelling the collective experience of a group of senior surgeons. It may improve patient selection by identifying a cohort with extremely poor survival, while allowing safe operation with acceptable outcomes among a group with very high operative risk.

    Abstract Purpose: In-person visits with a trained therapist have been standard care for patients initiating continuous positive airway pressure (CPAP). These visits provide an opportunity for hands-on training and an in-person assessment of mask fit. However, to improve access, many health systems are shifting to remote CPAP initiation with equipment mailed to patients. While there are potential benefits of a mailed approach, relative patient outcomes are unclear. Specifically, many have concerns that a lack of in-person training may contribute to reduced CPAP adherence. To inform this knowledge gap, we aimed to compare treatment usage after in-person or mailed CPAP initiation.
    Methods: Our medical center shifted from in-person to mailed CPAP dispensation in March 2020 during the COVID-19 pandemic. We assembled a cohort of patients with newly diagnosed obstructive sleep apnea (OSA) who initiated CPAP in the months before (n = 433) and after (n = 186) this shift. We compared 90-day adherence between groups.
    Results: Mean nightly PAP usage was modest in both groups (in-person 145.2, mailed 140.6 min/night). We did not detect between-group differences in either unadjusted or adjusted analyses (adjusted difference - 0.2 min/night, 95% - 27.0 to + 26.5).
    Conclusions: Mail-based systems of CPAP initiation may be able to improve access without reducing CPAP usage. Future work should consider the impact of mailed CPAP on patient-reported outcomes and the impact of different remote setup strategies.

    Abstract For any modern army it is very important to continuously maintain a high degree of operational (combat) readiness (availability) in order to maximize the effectiveness of the use of technical systems. Since determination and prediction of technical states and failures of technical systems in engineering, especially in armament, are difficult due to the impossibility of continuous condition monitoring with appropriate measuring equipment there is a need for a maintenance model that would be most helpful in taking timely action maintenance. In this paper, the subject of research is a model of maintenance of the M-84 tank weapoons systems. IntroductionThe M-84 tank is one of the most promising technical systems in the Serbian Army. Its use and modifications are foreseen in the next ten years. The M-84 is characterized by good tactical and technical characteristics. It has a powerful 125 mm cannon, coupled 7.62 mm machine gun and 12.7 mm anti-aircraft machine gun. The M-84 tank has an automatic battery charger and a fire control system. The fire control system enables fast target tracking and stabilization of the cannon barrel, which is a prerequisite for timely and favorable effect on the target. There are certain ambiguities in the existing model of maintenance of tank weapons. Technical diagnostics Technical diagnostics, as a part of the process of condition-based maintenance, should determine technical conditions of components or technical systems with certain accuracy at a point in time.     Abstract Objective: Patients with profound cardiogenic shock may require venoarterial (VA) extracorporeal membrane oxygenation (ECMO) for circulatory support most commonly via the femoral vessels. The rate of cardiac recovery in this population remains low, possibly because peripheral VA-ECMO increases ventricular afterload. Whether direct ventricular unloading in peripheral VA-ECMO enhances cardiac recovery is unknown, but is being more frequently utilized. A randomized trial is warranted to evaluate the clinical effectiveness of percutaneous left ventricle venting to enhance cardiac recovery in the setting of VA-ECMO.
    Methods: We describe the rationale, design, and initial testing of a randomized controlled trial of VA-ECMO with and without percutaneous left ventricle venting using a percutaneous micro-axial ventricular assist device.
    Results: This is an ongoing prospective randomized controlled trial in adult patients with primary cardiac failure presenting in cardiogenic shock requiring peripheral VA-ECMO, designed to test the safety and effectiveness of percutaneous left ventricle venting in improving the rate of cardiac recovery.
    Conclusions: The results of this nonindustry-sponsored trial will provide critical information on whether left ventricle unloading in peripheral VA-ECMO is safe and effective.
