    Abstract Although a number of comprehensive texts pertaining to perioperative medicine are available, this remains the only one that focuses specifically on the patient undergoing orthopedic surgery, now in a revised and expanded second edition. Beginning with chapters covering preoperative evaluations and general principles and practices of perioperative medicine, the book then considers anesthesiologic management in orthopedic surgery and the role of postoperative pain management. This is followed by a section on medical management in specific clinical settings, discussing patients with connective tissue disease, cardiac disease, chronic pulmonary and renal diseases, diabetes and neurological diseases, among others. A fourth section covers specific perioperative problems in orthopedic surgery, such as care of the elderly patient, venous thromboembolism, infection, nutrition, compartment syndrome and bone health. Finally, the role of allied services, quality improvement and ethics are highlighted. Selected case studies are included to illustrate real-world perioperative issues and management strategies in orthopedic surgery, now moved to their respective chapters for ease of use and reference. The shaded key points and chapter objectives boxes at the beginning of each chapter, and the summary points at the end of each chapter, have been retained and updated within a full-color interior.     Abstract Whether a reaction to events in the clinical arena, a consequence of technological innovation, or the legitimization of the marked ethos in health care, the field of medical ethics has become a complex domain in our time. Indeed, the ubiquity of ethical dilemmas in the provision of health care is well documented with more than 99% of primary care physicians reporting ethical problems arising in the conduct of their practices (1-3). Although commentary relating to this domain has most often originated in the primary care setting, hospital-based medicine and its ethics committees are another fertile source, as is medical research. Yet when one examines subspecialty medicine, a remarkable dearth of analysis and discourse pertaining to medical ethics emerges. This discussion is an attempt to address this deficit, at least as it pertains to one subspecialty: rheumatology. Leading with a brief overview of medical ethics

    Abstract As a result of advances in surgery, anesthesiology, and perioperative care, patients of more advanced age and medical complexity are now considered suitable candidates for surgical intervention. Rheumatologists, though frequently called upon to advise their patient in the surgical domain, may not be fully cognizant of the principles underlying medical consultation and therapy in perioperative setting. This paper provides an approach for guiding such medical care with a particular focus on the concerns pertinent to patient suffering from a chronic rheumatic disease.
