    Abstract Background: The mechanical tactile stimulation, such as plastic pins and airflow-driven membrane, induces cortical activity. The cortical activity depends on the mechanical tactile stimulation pattern. Therefore, the stimulation pattern of mechanical tactile stimuli intervention may influence its effect on the somatosensory function. However, the effect of the mechanical tactile stimulation input pattern on the somatosensory function has not yet been investigated at the behavioral level. The present study aimed to clarify the effects of mechanical tactile stimuli intervention with different stimulation patterns on the ability to discriminate moving directions.
    Results: Twenty healthy adults participated in the experiment. Three conditions were used for mechanical tactile stimuli intervention: (1) the whole stimulus surface was stimulated, (2) the stimulus moved within the stimulus surface, and (3) a no-stimulus condition. The effects of mechanical tactile stimuli intervention on tactile discrimination were evaluated using a simple reaction task and a choice reaction task to discriminate the movement direction. Reaction time, correct rate, and rate correct score were calculated to measure task performance. We examined the effects of mechanical tactile stimuli intervention on the ability to discriminate the moving direction for a certain period under three intervention conditions. The results showed that the mean reaction time during the simple reaction task did not differ significantly before and after the intervention under all intervention conditions. Similarly, we compared the data obtained before and after the intervention during the choice reaction task. Our results revealed that the mean reaction time and correct rate did not differ significantly under vertical and horizontal conditions. However, the rate correct score showed a significant improvement after the horizontal moving tactile stimulation intervention under both vertical and horizontal conditions.
    Conclusions: Our results showed that the effect of mechanical tactile stimuli intervention on mechanical tactile stimulation moving direction discrimination function depended on the input pattern of mechanical tactile stimuli intervention. Our results suggest the potential therapeutic benefits of sustained tactile stimulation intervention. This study revealed that it is possible to change behavioral levels via mechanical tactile stimuli intervention as well as the potential of mechanical tactile stimuli intervention in the field of rehabilitation.

    Abstract This paper investigates the super-resolution of velocity fields in two-dimensional flows from the viewpoint of rotational equivariance. Super-resolution refers to techniques that enhance the resolution of an image from low to high resolution, and it has recently been applied in fluid mechanics. Rotational equivariance of super-resolution models is defined as the property by which the super-resolved velocity field is rotated according to a rotation of the input, leading to inferences that are covariant with the orientation of fluid systems. In physics, covariance is often related to symmetries. To better understand the connection with symmetries, the notion of rotational consistency of datasets is introduced within the framework of supervised learning, which is defined as the invariance of pairs of low- and high-resolution velocity fields with respect to rotation. This consistency is sufficient and necessary for super-resolution models to learn rotational equivariance from large datasets. Such a large dataset is not required when rotational equivariance is imposed on super-resolution models through the use of prior knowledge in the form of equivariant kernel patterns. Nonetheless, even if a fluid system has rotational symmetry, this symmetry may not carry over to a velocity dataset, which is not rotationally consistent. This inconsistency can arise when the rotation does not commute with the generation of low-resolution velocity fields. These theoretical assertions are supported by the results of numerical experiments, where two existing convolutional neural networks (CNNs) are converted into rotationally equivariant CNNs and the inferences of these CNNs are compared after the supervised training.

    Abstract Aim: This study investigated whether contrast extravasation on computed tomography (CT) angiography in patients with traumatic brain injury (TBI) is associated with death or surgical procedures.
    Methods: Patients over 18 years old, directly brought in by ambulance with an isolated head injury and confirmed to have acute intracranial hemorrhage on a CT scan upon admission between 2010 and 2020, were included. The primary outcome was mortality, and the secondary outcome was neurosurgical procedures performed from admission to discharge from the intensive care unit. Multivariable logistic regression analyses were performed to evaluate the association between these outcomes and contrast extravasation.
    Results: The analysis included 188 patients with a median age of 65 years, 123 men (65.4%), 34 deaths (18.1%), and 91 surgeries (48.4%). Among the 66 patients with contrast extravasation, 22 (33.3%) died and 47 (71.2%) required surgery. Among the 122 patients with no contrast extravasation, 12 (9.8%) died, and 44 (36.1%) required surgery. The presence or absence of extravascular leakage was associated with death (odds ratio, 3.6 [95% CI: 1.2-12.2]) and surgery (odds ratio, 7.6 [95% CI: 2.5-22.7]).
    Conclusion: Contrast extravasation was associated with mortality and performance of surgery in patients with an isolated head injury.

    Abstract Background: Information is scarce regarding the clinical implications of aortic valve replacement (AVR) for patients suffering from moderate mixed aortic valve disease (MAVD), characterized by a combination of moderate aortic stenosis (AS) and regurgitation (AR). The objective of this retrospective study was to explore the clinical effects of AVR in individuals with moderate MAVD.
    Methods: We examined the clinical data from patients with moderate MAVD and preserved left ventricular ejection fraction, who had undergone echocardiography in the period spanning from 2010 to 2018. Moderate AS was defined as aortic valve area index of 0.60-0.85 cm
    Results: Among 88 patients (mean age, 74.4 ± 6.8 years; 48.9%, men), 44 (50.0%) required AVR during a median follow-up period of 3.3 years (interquartile range, 0.5-4.9). Mean values of specific aortic valve variables are as follows: aortic valve area index, 0.64 ± 0.04 cm
    Conclusions: Approximately half of the patients diagnosed with moderate MAVD eventually necessitated AVR throughout the period of observation, leading to positive clinical results. Vigilant tracking of these patients and watchful monitoring for signs requiring AVR during this time frame are essential.

    Abstract Percutaneous biliary intervention is widely accepted as an effective and safe treatment for various types of bile duct diseases. We present the case of a 44-year-old woman who developed bile leakage after a living-donor liver transplantation for locally advanced cholangiocarcinoma. A percutaneous drainage tube was placed in the segment 8 bile duct via the blind end of the jejunum. However, the bile leakage was unchanged. Bile leakage from the right posterior hepatic duct was suspected. Using a dual lumen microcatheter, a percutaneous drainage tube was placed in the segment 7 bile duct via the blind end of the jejunum, which reduced the bile leakage. These results suggest that a dual lumen microcatheter is a valuable tool for navigating the biliary tree during difficult percutaneous biliary interventions.

    Abstract Objectives: To find the optimal treatment duration with antibiotics for community-acquired pneumonia (CAP) in adults.
    Design: Systematic review and duration-effect meta-analysis.
    Data sources: MEDLINE, Embase and CENTRAL through 25 August 2021.
    Eligibility criteria: All randomised controlled trials comparing the same antibiotics used at the same daily dosage but for different durations for CAP in adults. Both outpatients and inpatients were included but not those admitted to intensive care units. We imposed no date, language or publication status restriction.
    Data extraction and synthesis: Data extraction by two independent reviewers. We conducted a random-effects, one-stage duration-effect meta-analysis with restricted cubic splines. We tested the non-inferiority with the prespecified non-inferiority margin of 10% examined against 10 days . The primary outcome was clinical improvement on day 15 (range 7-45 days).
    Secondary outcomes: all-cause mortality, serious adverse events and clinical improvement on day 30 (15-60 days).
    Results: We included nine trials (2399 patients with a mean (SD) age of 61.2 (22.1); 39% women). The duration-effect curve was monotonic with longer duration leading to a lower probability of improvement, and shorter treatment duration (3-9 days) was likely to be non-inferior to 10-day treatment. Harmful outcome curves indicated no association. The weighted average percentage of the primary outcome in the 10-day treatment arms was 68%. Using that average, the absolute clinical improvement rates of the following durations were: 3-day treatment 75% (95% CI: 68% to 81%), 5-day treatment 72% (95% CI: 66% to 78%) and 7-day treatment 69% (95% CI: 61% to 76%).
    Conclusions: Shorter treatment duration (3-5 days) probably offers the optimal balance between efficacy and treatment burden for treating CAP in adults if they achieved clinical stability. However, the small number of included studies and the overall moderate-to-high risk of bias may compromise the certainty of the results. Further research on the shorter duration range is required.

    Abstract Aims: Data on B-type natriuretic peptide (BNP) levels and adverse outcomes in patients with moderate mixed aortic valve disease (MAVD), defined as moderate aortic stenosis (AS) and regurgitation (AR), are scarce. Therefore, this study investigated the impact of BNP on the clinical outcomes in such patients.
    Methods and results: Clinical data from 81 patients (mean age, 74.1 ± 6.8 years; 50.6%, men) treated for moderate MAVD and left ventricular ejection fraction (LVEF) ≥ 50% during 2010-2018 were retrospectively analysed. Specific echocardiographic data of the study patients were LVEF of 57.8 ± 5.0%, aortic valve index of 0.64 ± 0.04 cm
    Conclusions: Patients with moderate MAVD are at higher risk of unfavourable clinical outcomes, and age and BNP are independently related to the occurrence of adverse events. High BNP levels may reflect extravalvular cardiac damage in patients with moderate MAVD.

    Abstract Our recent studies have shown that noxious stimuli in the colorectum enhance colorectal motility via the brain and spinal defecation centers in male rats. In female rats, however, noxious stimuli have no effect on colorectal motility. The purpose of this study was to determine whether sex hormones are major contributing factors for sex-dependent differences in neural components of the spinal defecation center. Colorectal motility was measured using an in vivo method under ketamine and α-chloralose anesthesia in rats. Capsaicin was administered into the colorectal lumen as noxious stimuli. Orchiectomy in male rats had no effect on the capsaicin-induced response of colorectal motility. However, in ovariectomized female rats, capsaicin administration enhanced colorectal motility, though intact female animals did not show enhanced motility. When estradiol was administered by using a sustained-release preparation in ovariectomized female rats, capsaicin administration did not enhance colorectal motility unless a GABA

    Abstract Dendritic cells (DCs) present foreign antigens to T cells via the major histocompatibility complex (MHC), thereby inducing acquired immune responses. ATP accumulates at sites of inflammation or in tumor tissues, which triggers local inflammatory responses. However, it remains to be clarified how ATP modulates the functions of DCs. In this study, we investigated the effects of extracellular ATP on mouse bone marrow-derived dendritic cells (BMDCs) as well as the potential for subsequent T cell activation. We found that high concentrations of ATP (1 mM) upregulated the cell surface expression levels of MHC-I, MHC-II, and co-stimulatory molecules CD80 and CD86 but not those of co-inhibitory molecules PD-L1 and PD-L2 in BMDCs. Increased surface expression of MHC-I, MHC-II, CD80, and CD86 was inhibited by a pan-P2 receptor antagonist. In addition, the upregulation of MHC-I and MHC-II expression was inhibited by an adenosine P1 receptor antagonist and by inhibitors of CD39 and CD73, which metabolize ATP to adenosine. These results suggest that adenosine is required for the ATP-induced upregulation of MHC-I and MHC-II. In the mixed leukocyte reaction assay, ATP-stimulated BMDCs activated CD4 and CD8T cells and induced interferon-γ (IFN-γ) production by these T cells. Collectively, these results suggest that high concentrations of extracellular ATP upregulate the expression of antigen-presenting and co-stimulatory molecules but not that of co-inhibitory molecules in BMDCs. Cooperative stimulation of ATP and its metabolite adenosine was required for the upregulation of MHC-I and MHC-II. These ATP-stimulated BMDCs induced the activation of IFN-γ-producing T cells upon antigen presentation.
