    Abstract In order to analyze the competitive behavior of online retailers in complex market under the environment of diversified customers, a duopoly competition model considering the service sensitivity and cognitive differences is established by using the Hotelling model. The equilibrium price, demand and revenue of static game and dynamic game are solved, and the changing trend is studied by numerical simulation. Results have shown that online retailers' profits are affected by the service sensitivity and cognitive differences, not only related to the price directly, but also relevant to unit service cost, brand cognition and competitors' pricing strategy highly. The high service providers with internet brand cognition have a significant price advantage, which means high service is always accompanied by high prices. Stray price effects exist in online retailers, blind to improve the quality of the cognitive service may do more harm than good for the low quality of service provider. Online retailers have the first-mover advantages. The previous decision maker's equilibrium profit is obviously higher than the latter, as well as the price. The dynamic equilibrium is also significantly better than the static equilibrium on profits and price. Online retailers have sufficient intrinsic motivation to grab the opportunity. There is a theoretical threshold for the service level of online retail, and merchants will not provide services beyond the threshold. Theoretical references can be provided on the pricing competition and service mode selection for online retailers in the context of big data.

    Abstract Dysferlin protein deficiency can cause neuromuscular dysfunction, resulting in autosomal recessive dysferlinopathy, which is caused by DYSF gene mutation. Dysferlin proteins belongs to the Ferlin1-like protein family and are associated with muscle membrane repair and regeneration. In China, pathogenic mutations of the protein often result in two clinical phenotypes of Miyoshi muscular or limb band muscular dystrophy type 2B. It is clinically characterized by progressive muscle weakness and elevated serum creatine kinase. The data of the child were collected, blood samples of the child and his family members were collected, and whole exome sequencing (WES) was performed. The recombinant expression vector was constructed, the function of the mutation was verified by minigene, and the pathogenicity of the mutation was further analyzed by combining with biological information analysis. The patient initially presented with asymptomatic elevation of serum creatine kinase(CK). Then progressive lower limb weakness, mainly distal limb weakness. Large amounts of scattered necrosis, myogenic lesions, and complete deletion of dysferlin protein were observed under muscle biopsy, which further improved genetic detection. Whole exome sequencing showed compound mutations (c.1397 + 1_1397 + 3del and c.1375dup p.M459Nfs*15) in DYSF gene. c.1375dup p.M459Nfs*15 have been reported. The other mutation is the deletion of c.1397 + 1_1397 + 3 in Intron15, which is an intron mutation that may affect splicing and the pathogenesis is still unknown. Minigene splicing assay verified that c.1397 + 1_1397 + 3del resulted in exon15 skipping and produced a premature termination codon. We report a novel pathogenic mutation in DYSF gene with Miyoshi myopathy and demonstrate this variant causes skipping of exon15 by minigene splicing assay. We point out the need of conducting functional analysis to verify the pathogenicity of intronic mutation. The finding enriches the mutation spectrum of DYSF gene and laid a foundation for future studies on the correlation between genotype and phenotype.

    Abstract BACKGROUND The aim of this study was to assess clinical outcomes of arthroscopic surgery for gluteal muscle contracture (GMC) with at least 2 years of follow-up in a large sample. MATERIAL AND METHODS A total of 665 patients who underwent arthroscopic release procedure at our institution between March 2014 and December 2018 and met the inclusion criteria were included in this study. All the patients were operated on by the same surgeon and the surgeon released the contracture band from anterior to posterior starting from the anterior edge of the ITB, the contracture band of the gluteus maximus, and vastus medialis tensor. After exclusion, 544 patients (218 males and 326 females) were finally included. Clinical outcomes at 2 years of postoperative follow-up were evaluated by a combination of several methods, including a new criterion we proposed for postoperative assessment, which included the cross-leg test, difficulty in squatting with legs together, and abnormal gait, the visual analogue scale (VAS), the modified Harris hip score (mHHS), and the satisfaction of patients. RESULTS The average follow-up time was 48.2±14.9 months. Based on the new criteria, 513 (94.3%) patients were evaluated as good, 25 (4.6%) as fair, and 6 (1.1%) as poor. The mHHS increased from 72.1±6.0 (range, 56.0-81.3) to 97.3±2.2 (range, 92.3-100.0) (P<0.001). The VAS decreased from 3.3±1.1 (range, 0-5) to 0.062±0.13 (range, 0-1) (P<0.001). For subjective satisfaction, 526 (96.7%) patients were satisfied, 14 (2.6%) patients were neutral, and 4 (0.7%) patients were dissatisfied. CONCLUSIONS In a large sample, favorable and durable clinical outcomes were observed after the arthroscopic procedure. Clinical symptoms and mHHS showed significant improvement with a minimum 2-year follow-up.

    Abstract In robot-assisted surgical systems, surgical instrument segmentation is a critical task that provides important information for surgeons to make informed decisions and ensure surgical safety. However, current mainstream models often lack precise segmentation edges and suffer from an excess of parameters, rendering their deployment challenging. To address these issues, this article proposes a lightweight semantic segmentation model based on edge refinement and efficient self-attention. The proposed model utilizes a lightweight densely connected network for feature extraction, which is able to extract high-quality semantic information with fewer parameters. The decoder combines a feature pyramid module with an efficient criss-cross self-attention module. This fusion integrates multi-scale data, strengthens focus on surgical instrument details, and enhances edge segmentation accuracy. To train and evaluate the proposed model, the authors developed a private dataset of endoscopic surgical instruments. It containing 1,406 images for training, 469 images for validation and 469 images for testing. The proposed model performs well on this dataset with only 466 K parameters, achieving a mean Intersection over Union (mIoU) of 97.11%. In addition, the model was trained on public datasets Kvasir-instrument and Endovis2017. Excellent results of 93.24% and 95.83% were achieved on the indicator mIoU, respectively. The superiority and effectiveness of the method are proved. Experimental results show that the proposed model has lower parameters and higher accuracy than other state-of-the-art models. The proposed model thus lays the foundation for further research in the field of surgical instrument segmentation.
