    Abstract Introduction: Preterm identification of cervical dilation in pregnant women leads to the application of emergency cervical cerclage with an expectation of achieving term delivery. However, this is not always feasible. Short- and long-term neonatal complications post-preterm birth pose a significant challenge. It is crucial to anticipate potential complications and understand the possibilities of postpartum development as they can be encountered. We aimed to evaluate the effect of the degree of cervical dilatation before ultrasound and physical examination-indicated cerclage in singleton pregnancies presenting with premature cervical dilatation with bulging fetal membranes (rescue cerclage) on subsequent neonatal outcomes.
    Materials and methods: In this retrospective clinical study, over a 10-year period between January 2009 and January 2019, 72 singleton pregnancies undergoing rescue cerclage were included and divided into two groups according to pre-cerclage cervical dilatation: Group 1 (n = 33) and Group 2 (n = 39) with cervical dilatation ≤3 cm and >3 cm, respectively. Latency period for pregnancy prolongation, gestational age at delivery, birth weight, and neonatal morbidity and mortality were compared across the groups. Logistic regression was used to delineate the independent effect of cervical dilatation at cerclage placement on neonatal mortality.
    Results: Group 2 had a higher delivery rate at ≤28 weeks' gestation (p = 0.007) and lower birth weight (p = 0.002) compared to Group 1, with an increased mean latency period in Group 2 (90 ± 55 days versus 52 ± 54 days, p = 0.005). The newborn intensive care unit (NICU) requirement, respiratory distress syndrome (RDS), neonatal jaundice and sepsis, and retinopathy of prematurity (ROP) were more frequent in Group 2. Neonatal mortality rate was higher (52.6% versus 24.2%, p = 0.015) and intact survival was lower (23.1% versus 48.4%, p = 0.013) in Group 2, whereas rates of cerebral palsy (8% and 9%, respectively) were similar between the groups (p = 0.64).
    Conclusion: Advanced cervical dilatation (>3 cm) during physical examination-indicated cerclage in singleton pregnancies is associated with earlier delivery, leading to increased neonatal morbidity and mortality when compared with pregnancies having lesser degrees of cervical dilatation at cerclage. However, short-term poor neurological outcomes seem comparable.

    Abstract Dyes are frequently used in industries such as textile, leather, paper and printing, to water sources causes harmful effects on the environment and human health. Therefore, it is crucial to effectively remove colored contaminants from water in order to protect the environment and public health, maintain biodiversity and preserve the esthetic aspects of water resources. In this study, wood chips obtained from Pinus brutia (PB) tree grown in many parts of the world were turned into biochar and then modified and used for the removal of malachite green, a cationic dye. For this purpose, biochar (PBB) was made by collecting PB wood and turning it into chips (PB). Later, PBB was modified to gain nano-magnetic properties. The structure of the obtained PBB and nM-PBB adsorbents was characterized by FT-IR. pH (2–9), temperature (25 °C–55 °C), time change (15 min-240 min), adsorbent amount change (0.05 g–0.45g) and MG concentration (25 mg/L–250 mg/L) were investigated in MG removal of PBB and nM-PBB. The process was found to be pseudo-second-order and spontaneous endothermic reaction. PBB and nM-PBB were found to be suitable for Langmuir isotherm in MG removal (qₘₐₓ=13.004 mg/g for PBB, qₘₐₓ=18.215 mg/g for nM-PBB).

    Abstract Background: Septic arthritis of the shoulder is a rare but devastating condition that may lead to joint destruction. There are few studies and limited outcome data on shoulder arthroplasty for infected native shoulders with end-stage glenohumeral arthritis (GHA). Hence, this study aimed to demonstrate the clinical outcomes of two-stage implantation in reverse shoulder arthroplasty (RSA) using an antibiotic spacer in the first stage for this challenging condition.
    Methods: We conducted a retrospective study on two-stage implantation in RSA in infected shoulders. Patients were diagnosed with end-stage GHA due to primary shoulder sepsis or infection following non-arthroplasty shoulder surgery. Laboratory data, range of motion (ROM), and functional scores including American Shoulder and Elbow Surgeons score, Constant score, and Disabilities of the Arm, Shoulder and Hand score were assessed prior to spacer placement and at the latest follow-up. Furthermore, intraoperative and postoperative complications were recorded.
    Results: In this study, 10 patients with a mean age of 54.8 ± 15.8 years (range, 30-77 years) were included. The mean follow-up period was 37.3 ± 9.1 months (range, 25-56 months). All postoperative ROM measurements and functional scores were improved significantly. Although no reinfection was observed, a total of 5 complications including 2 hematomas, 1 intraoperative humeral fracture, 1 humeral stem loosening, and 1 anterior deltoid dysfunction were observed in 4 patients after a follow-up period of at least 2 years after RSA.
    Conclusions: Two-stage implantation in RSA is an effective method for improving the function and controlling the infection in postinfectious end-stage GHA in native shoulders.

    Abstract Objective: There is no consensus about the optimal frequency of patient visits during the rehabilitation program after arthroscopic rotator cuff repair (ARCR). This study aimed to investigate the short- and long-term effects of high-frequency (HF) and low-frequency (LF) visits of the patients in the first 12 weeks of rehabilitation after ARCR.
    Methods: This was a quasi-randomized study with 2 parallel groups. Forty-seven patients with ARCR were included in 2 different patient visit frequency protocols (HF = 23, LF = 24) in 12 weeks of postoperative rehabilitation. Patients in the HF group visited the clinic twice a week, whereas patients in the LF group visited once every 2 weeks for the first 6 weeks and once a week for the following 6 weeks. Both groups performed the same exercise protocol. Outcome measurements were pain and range of motion measured at baseline; at the 3rd, 5th, 8th, 12th, and 24th weeks; and at 1-year follow-up. Shoulder function was assessed at the 12th and 24th weeks and at 1-year follow-up with an American Shoulder and Elbow Surgeons score.
    Results: There was a significant group × time interaction in pain intensity during the activity between the groups. The activity pain intensity was higher in the LF group (4.2 points) at 8 weeks post surgery than in the HF group (2.7 points) (mean difference: 1.5 points, P < .05), whereas it was similar in both groups at other time periods. The interaction term was not significant between the groups for pain intensity during rest and night through the 1-year follow-up. No group × time interactions were observed in shoulder range of motion and American Shoulder and Elbow Surgeons score over the postoperative period.
    Conclusion: Both rehabilitation programs at different visit frequencies after ARCR showed similar clinical results in the long term. A supervised, controlled rehabilitation program with LF visits in the first 12 weeks after surgery can be sufficient to achieve optimal clinical results and reduce rehabilitation-related costs after ARCR.
    Impact: This study highlights that LF treatment protocols under the supervision of the therapist can be adopted after the arthroscopic rotator cuff repair to achieve successful results while decreasing the treatment costs. Physical therapists should plan the treatment sessions efficiently for the compliance of the patients to the exercise treatment.
    Lay summary: If you are a patient with arthroscopic rotator cuff repair, a supervised, controlled rehabilitation program with low-frequency visits in the first 12 weeks after surgery could help you achieve the best outcome and help lower the costs of rehabilitation. A total of 3 visits in the first 6 weeks might be sufficient (once every 2 weeks). More frequent visits (1-2 visits a week) should happen 6 to 12 weeks after the surgery.

    Abstract Introduction: Idiopathic pulmonary hypertension(IPAH) is a rare disease that causes severe morbidity and mortality despite advances in treatment management. Evaluating the prognosis of the disease is critical in determining therapeutic approaches. We aimed to evaluate the prognostic significance of C-reactive protein/albumin ratio (CAR) and uric acid, which is an easily applicable and inexpensive parameter in patients with IPAH.
    Methods: Seventy-two IPAH patients and 99 consecutive non-IPAH patients as a control group were enrolled in the study retrospectively. Right heart catheterization(RHC), echocardiography, and laboratory parameters of the two groups and those who died and survived among the IPAH patients were compared.
    Results: IPAH and control group were compared at the first stage and CAR (1.98(0.28-10.74), 0.75(0.22-4.7),respectively;p < 0.01) and uric acid (0.33(0.19-0.87), 0.3(0.11-0.48) mmol/L, respectively; p = 0.03) values were significantly higher in the pulmonary hypertension group compared to the control group. Compared with the surviving IPAH patients, CAR (4.60(1.39-10.74),1.54(0.28-6.74),respectively;p < 0.001) and uric acid levels (0.458(0.26-0.87), 0.315(0.19-0.56) mmol/L, respectively; p < 0.001) were significantly higher in the group of patients who died. In the multivariate Cox regression models uric acid(p < 0.001) and CAR(p < 0.001) were found to be associated with survival time. Receiver operating characteristic curves (ROC) analyses showed that > 1.54 CAR value (AUC = 0.81,Sens:85.7%,Spec:56.9%,p < 0.001) and > 5.85 mg/dL (>0.348 mmol/L) uric acid value (AUC = 0.864, Sens:85.7%, Spec:78.4%, p < 0.001) are strong predictors for mortality.
    Conclusion: In this study, we showed that simple markers such as CAR, which augment the inflammation marker feature of CRP, and uric acid can give prognostic information in PAH patients.

    Abstract Objective:The aim of our study was to determine the correlation between leukocyte platelet aggregates and thrombosis in patients with solid tumors who are susceptible to inflammation and thrombosis.Material and Method:In this study, we compared laboratory and clinical data of 28 metastatic gastrointestinal tumors, 33 metastatic lung tumors, 29 operated tumor patients, for a total of 90 patients with a control group consisting of 12 healthy volunteers followed at Namik Kemal University Oncology clinic in between 2013 -2014.Results:According to the results, monocyte platelet aggregates level of metastatic gastrointestinal tumors and operated tumors were found to be significantly higher compared to the control group (p: 0.05, 0.029, respectively. No significant difference was observed between the groups in the levels of thrombin-antithrombin III complex and neutrophil lymphocyte ratio. When all groups were analyzed to assess the hypercoagulopathy, deep vein thrombosis frequency were identified as 22% (18 patients). The rate was 17% in operated tumors, and was 21% in metastatic lung tumors. The incidence of deep vein thrombosis in metastatic gastrointestinal tumors was found to be 35%. There was no significant difference between patients with and without deep vein thrombosis about age, monocyte platelet aggregates, granulocyte platelet aggregates, thrombin-antithrombin III complex and high-sensitivity C-reactive protein levels.Conclusion:In conclusion, to the best of our knowledge for the first time we found leukocyte platelet aggregate values increases in cancer patients of different stages and different tissue origin. To confirm the results we have achieved, and to demonstrate the potential impact of this information in estimation of thrombotic events in cancer patients more studies conducted in larger groups of patients are needed.

    Abstract This study aims to explore the pharmacodynamics of chelidonine, the active ingredient in greater celandine (Chelidonium majus L.), on in vitro rat bladder and trachea tissue, and evaluate its cell protective effects on primary lung and kidney cell lines. The study was carried out via repeated applications of acetylcholine, atropine, verapamil and oxybutynin, alongside Ca++ in a calcium-free environment, on urinary bladder tissue, and repeated applications of acetylcholine, atropine, carbachol and mecamylamine on trachea tissue. At the same time, cell viability and catalase and superoxide dismutase activity was measured on primary cell lines obtained from lung and kidney tissue samples. The study has shown that chelidonine has a relaxant effect on bladder and trachea tissues, and it may be mentioned that this effect is produced via muscarinic receptors. In addition, chelidonin caused a statistically insignificant increase in cell viability in primary lung and kidney cell lines at increasing doses (1 and 4 µg/mL), but this increase remained at the control group level. In contrast, chelidonin caused a significant decrease in cell viability at the same cell lines at doses of 8 and 16 µg/mL. In conclusion, it is suggested that greater celandine, which is used in folk medicine, and its active ingredient chelidonine might have beneficial effects on asthma, urinary incontinence and other urinary tract and respiratory diseases among others.
