    Abstract Background: Long-term maternal outcomes of subsequent pregnancies (SSPs) in patients with peripartum cardiomyopathy (PPCM) have not been analyzed.
    Objectives: The goal of this study was to evaluate the long-term survival of SSPs in women with PPCM.
    Methods: We conducted a retrospective review of 137 PPCMs in the registry. The clinical and echocardiographic findings were compared between the recovery group (RG) and nonrecovery group (NRG), defined as left ventricular ejection fraction ≥50% and <50% after an index of pregnancy, respectively.
    Results: Forty-five patients with SSPs were included with a mean age of 27.0 ± 6.1 years, 80% were of African American descent, and 75.6% from a low socioeconomic background. Thirty (66.7%) women were in the RG. Overall, SSPs were associated with a decrease in mean left ventricular ejection fraction from 45.1% ± 13.7% to 41.2% ± 14.5% (P = 0.009). At 5 years, adverse outcomes were significantly higher in the NRG compared with the RG (53.3% vs 20%; P = 0.04), driven by relapse PPCM (53.3% vs 20.0%; P = 0.03). Five-year all-cause mortality was 13.33% in the NRG compared with 3.33% in the RG (P = 0.25). At a median follow-up of 8 years, adverse outcomes and all-cause mortality rates were similar in the NRG and RG (53.3% vs 33.3% [P = 0.20] and 20% vs 20%, respectively).
    Conclusions: Subsequent pregnancies in women with PPCM are associated with adverse events. The normalization of left ventricular function does not guarantee a favorable outcome in the SSPs.

    Abstract Rapid growth in population and industrialization causes the worldwide crisis for potable water, which is emphasizing on researchers to find an alternative way. Solar still is a solar energy-based device facing the problem of low productivity. The motive of the present investigation is to improve the productivity of triangular pyramid solar still by incorporating triangular cross-sectional aluminium hollow-fins and wool cloth wick-fins. In two sets of experiments, the performance of the triangular pyramid solar still with hollow-fins, wick-fins, and without fins has been compared for the case of partially and fully submerged fins. The total distillate outputs per day (including nocturnal duration) from stills for the case of partially submerged fins were: 2.5674 L/m² (12.36% higher) with hollow-fins, 2.5481 L/m² (11.51% higher) with wick-fins, and 2.2850 L/m² for without fins. For the case of partially submerged fins, the productivity was observed higher for the still with wick-fins during morning hours (9:00 h to 13:00 h), whereas for the still with hollow-fins during afternoon hours (14:00 h to 17:00 h). For the case of fully submerged fins, total distillate outputs per day (including nocturnal duration) from stills were: 2.2529 L/m² (9.00% higher) with hollow-fins, 2.3042 L/m² (11.49% higher) with wick-fins, and 2.0668 L/m² for without fins. For the still with hollow-fins and wick-fins, efficiency was improved by 13.22% and 15.70% in the case of partially submerged fins, and 17.36% and 29.34% in the case of fully submerged fins. The enhancement in the efficiency of the still with wick-fins and hollow-fins was observed higher for the case of fully submerged fins than the case of partially submerged fins.

    Abstract Background and objective: Colonoscopy is a common procedure performed by colorectal surgeons for screening, diagnosis, and surveillance of various colorectal diseases. Existing literature has conflicting data on quality outcomes of colonoscopies performed in the afternoon and the morning schedules and only includes colonoscopies performed by gastroenterologists. We sought to analyze procedural outcomes between morning and afternoon colonoscopies performed by colorectal surgeons.
    Data sources and main outcome measures: A retrospective chart review of colonoscopies performed by colorectal surgeons at a tertiary care center from October 2018 through July 2020 was performed. Complete colonoscopies with documented times were included. Patients with colonic resection and incomplete colonoscopy were excluded. Main outcome measures adenoma and polyp detection rates and colonoscopy time variables were compared between morning and afternoon colonoscopies.
    Results: A total of 781 patients were analyzed. Colonoscopies were evenly distributed during shifts (49% morning and 51% afternoon). The overall polyp and adenoma detection rates were 46% and 29%, respectively. There were no significant differences in adenoma and polyp detection rates and colonoscopy duration between morning and afternoon colonoscopies. Multivariate analysis demonstrated that history of prior polypectomy was an independent predictor of adenoma detection rate (OR: 2.17, 95% CI 1.33-3.54, p = 0.002) and was associated with significantly increased colonoscopy times in afternoon shift.
    Conclusion: There were no differences in quality outcomes of adenoma and polyp detection rates between morning and afternoon colonoscopies performed by colorectal surgeons. In addition to known predictors, cecal intubation time and history of polypectomy were also independent predictors of adenoma detection rate. Patients with prior polypectomy had increased colonoscopy times in afternoon shift. Since colorectal surgeons perform higher proportion of diagnostic and surveillance colonoscopies, these patients may be better suited for colonoscopies in morning shift.

    Abstract Background: In patients with Parkinson's disease (PD), oral dosing of extended-release carbidopa-levodopa (Rytary, IPX066 [ER CD-LD]) achieves peak levodopa plasma concentrations within 1 hour and maintains them for 4-6 hours.
    Aims: To compare the onset and duration of ER CD-LD benefit with those of immediate-release carbidopa-levodopa (IR CD-LD) in PD patients with motor fluctuations, using crossover data, and to evaluate which threshold values of improvement in finger-tapping and Unified Parkinson's Disease Rating Scale (UPDRS) motor scores yield results most similar to those for trained raters' "on"/"off" assessments.
    Methods: Patients underwent serial "on"/"off" rating and provided serial finger-tapping and UPDRS motor scores after receiving, in an "off" state, their usual morning IR dose or an ER dose designed to produce a similar levodopa peak concentration. Predefined improvement thresholds for analysis were 10%, 15%, and 20% increases in finger-tapping score and 2.5, 5, 7, and 11-point decreases in UPDRS motor score. Serial plasma samples were assayed for levodopa.
    Results: Among 27 patients, mean time to onset of an "on" state was similar for ER compared with IR CD-LD (0.83 vs 0.81 hour), but mean duration was significantly longer for ER CD-LD than for IR CD-LD (5.56 vs 2.69 hours;
    Conclusion: For ER CD-LD, observer assessments of "on" state were corroborated by sustained treatment effects. Correlations among "on"-state duration, finger-tapping score, and UPDRS motor score may suggest clinically relevant thresholds for acute assessment of treatment benefit.

    Abstract (1) Background: The amount of time nurses spend with their patients is essential to improving the quality of patient care. Studies have shown that nurses spend a considerable amount of time on a variety of activities––which are often not taken into account while estimating nurse-to-patient care time allocation––that could potentially be eliminated, combined or delegated with greater productivity. The current study aimed to calculate standard time for each activity category by quantifying the amount of time required by nurses to complete an activity category and determine the adjustment time that can be given during work, as well as determine factors that can be altered to improve the efficiency of nursing care on inpatient general wards of a governmental hospital. (2) Method: A time and motion study was conducted over two weeks using 1-to-1 continuous observations of nurses as they performed their duties on inpatient general wards, while observers recorded each single activity, and specifically the time and movements required to complete those activities. (3) Result: There was 5100 min of observations over 10 working days. Nurses spent 69% (330 min) of time during their 8 h morning shift on direct patient care, (19.4%) ward/room activities (18%), documentation (14%), indirect patient care (12%) and professional communication (5%). Around 94 min of activities seem to be wasted and can be potentially detrimental to nurses’ overall productivity and threaten patient care quality. The standard number of hours that represents the best estimate of a general ward nurse regarding the optimal speed at which the staff nurse can provide care related activities was computed and proposed. (4) Conclusions: The findings obtained from time–motion studies can help in developing more efficient and productive nursing work for more optimal care of patients.

    Abstract The study was conducted to evaluate the ameliorating potential of Cassia absus L. (CA) seed-powder against cadmium-induced alterations in zebrafish (Danio rerio L.). Fish were divided into five groups (C1: received normal feed, C2: received normal feed along with CA powder, C3: normal feed along with cadmium exposure (1 ppm), C4: Vitamin Eenriched feed along with cadmium exposure and T1: Feeding of powder of CA (50% of total feed) along with cadmium exposure comprising of eighteen fish in each group. The fish of the treatment group were offered with powder of CA (10 mg /fish in the morning for 21 days along with exposure to cadmium chloride. Whole fish was used to evaluate oxidative stress markers and histopathological evaluation of organs. After 21 days of exposure to cadmium chloride to fish, a significant reduction in catalase activity, non-significant reduction in reduced glutathione level (GSH) along with a significantly higher level of malondialdehyde was observed. The treatment of CA seed-powder partially ameliorated alterations in oxidative stress markers. Cadmium chloride exposure for 21 days led to pathological changes in the liver, kidney, gills and brain of zebrafish. The microscopic changes in the organs of fish treated with CA powder were mild as compared to the toxicity group. Phytochemical analysis of the methanolic and n-butanol fraction of CA seed revealed the presence of various flavonoids. It can be concluded that Cassia absus seeds have the ability to control the toxic potential of Cadmium and it may be due to phytochemicals like flavonoids present in the seed powder.

    Abstract Background: Prehypertension is associated with high blood pressure and higher cardiovascular risk, and it should be suggested to prehypertensive patients that they modify their lifestyle in order to help avoid these problems. Methodology: The present study was the cross-sectional longitudinal study conducted on prehypertensive subjects of lower socio-economic group residing in Ahmedabad City. The participants were explained regarding modification in form of avoidance of entire amount of salt, morning exercise in form of brisk walking for half an hour at least three times a week and abstinence of smoking and alcohol in subjects who are habituated. Patients were instructed to attend OPD for every one month in the morning for 3 months as follow up. Results: In our study, pre HTN was detected in 72 (40%) of male subjects and 70 (41.1%) of female subjects. Prehypertension was found to be more prevalent in males (54.6%) as compared to females (45.4%). Overall 81.15% of subjects who were prehypertensive were reverted to normotension within 3 months with life style modification. Conclusion: It is concluded from our study that preHTN is quite prevalent affecting > 40% of urban population of lower socioeconomic class. Weight reduction, abstinence from smoking, regular exercise and salt reduction are highly effective, easy to follow and cheaper measures to control pre HTN and prevent its progression to hypertensive levels thereby also reducing cardiovascular and other complications of HTN.     Abstract Background: Atrial fibrillation (AF) ablation is a complex procedure, generally requiring at least one overnight hospital stay. We investigated the safety and feasibility of early mobilization and same-day discharge following streamlined peri-ablation management for AF.
    Methods: From 2014, we offered same-day discharge to selected patients who underwent uncomplicated AF ablation on the morning lists, with ultrasound-guided femoral access, uninterrupted warfarin or minimal interruption in novel oral anticoagulants, and reversal of intraprocedural heparin with protamine. Patients were discharged 6-8 h postprocedure and offered access to a dedicated nurse helpline.
    Results: Of 1599 AF ablation cases performed from April 2014 to March 2017, 811 (50.7%) were performed on the morning lists and 169/811 (20.8%) were discharged on the same day. Excluding 26 research cases, 1/143 (0.7%) had transient right phrenic nerve palsy and five (3.5%) cases experienced minor problems that did not preclude same-day discharge; three (2.1%) needed rehospitalization postdischarge: one for pericarditic chest pain and two for nausea/vomiting. Compared to 642 overnight cases, day-case procedures were shorter, more likely to be redos, to be performed under sedation rather than general anesthesia, and less likely to involve linear lesions and electrical cardioversion.     Conclusions: Selective same-day discharge after AF ablation is safe and feasible using a streamlined peri-procedural care protocol. Wider adoption can potentially reduce health-care costs while improving patient experience.

    Abstract Background: Diagnosis followed by effective treatment of tuberculosis (TB) reduces transmission and saves lives in persons living with HIV (PLHIV). Sputum smear microscopy is widely used for diagnosis, despite limited sensitivity in PLHIV. Evidence is needed to determine the optimal diagnostic approach for these patients.
    Methods: From May 2011 through June 2012, we recruited PLHIV from 15 HIV treatment centers in western Kenya. We collected up to three sputum specimens for Ziehl-Neelsen (ZN) and fluorescence microscopy (FM), GeneXpert MTB/RIF (Xpert), and culture, regardless of symptoms. We calculated the incremental yield of each test, stratifying results by CD4 cell count and specimen type; data were analyzed to account for complex sampling.
    Results: From 778 enrolled patients, we identified 88 (11.3%) laboratory-confirmed TB cases. Of the 74 cases who submitted 2 specimens for microscopy and Xpert testing, ZN microscopy identified 25 (33.6%); Xpert identified those plus an additional 18 (incremental yield = 24.4%). Xpert testing of spot specimens identified 48 (57.0%) of 84 cases; whereas Xpert testing of morning specimens identified 50 (66.0%) of 76 cases. Two Xpert tests detected 22/24 (92.0%) TB cases with CD4 counts <100 cells/μL and 30/45 (67.0%) of cases with CD4 counts ≥100 cells/μl.
    Conclusions: In PLHIV, Xpert substantially increased diagnostic yield compared to smear microscopy and had the highest yield when used to test morning specimens and specimens from PLHIV with CD4 count <100 cells/μL. TB programs unable to replace smear microscopy with Xpert for all symptomatic PLHIV should consider targeted replacement and using morning specimens.
