    Abstract We report the case of a woman in her 50s who presented headaches, blurred vision, diplopia and loss of peripheral vision. She was treated for normal tension glaucoma based on optic nerve cupping prior to the development of diplopia. Records demonstrated visual field constriction over 4 months despite well-controlled intraocular pressures. Examination revealed decreased visual acuity and visual field constriction. The multifocal electroretinogram was abnormal. After a thorough review of her medical and family history, a concern for cancer-associated retinopathy developed. Blood samples were positive for antiretinal antibodies against 23 kDA and 46 kDA proteins. Cancer screening was recommended as the aetiology for retinopathy was unknown and revealed a left breast lump. Following lumpectomy with adjuvant chemoradiation, her visual acuity normalised and visual field defects completely resolved. This case serves to provide an example that distant systemic symptoms may be a manifestation of the underlying malignancy and the importance of clinical suspicion with prompt evaluation.

    Abstract Purpose: Chronic pain is a common feature of hypermobile Ehlers-Danlos Syndrome (hEDS), yet how patients assess and communicate their pain remains poorly understood. The objective of the present study was to explore the use of numeric pain assessment in individuals with hEDS, from a patient-centered perspective.
    Materials and methods: Our analysis is based on in-depth qualitative interviews. The interviews were conducted over the phone. Our participants were patients living with hEDS (
    Results: Three primary themes emerged from these data, namely, (1) confusion around the quantification of multidimensional pain, (2) the subjectivity of pain experience, and (3) a strategic use of assessments for practical purposes beyond the accurate representation of pain.
    These results demonstrate the need for caution in relying exclusively on numeric pain assessment instruments. We conclude with a brief proposal for a clinical communication strategy that may help to address the limitations of numeric pain assessment that were identified in our interviews.

    Abstract Approaches for raising calves vary across commercial dairy farms and relate to behavioral opportunities and animal welfare. The objectives of this study were to evaluate how US dairy producers and calf managers perceive 1) welfare implications of varying management practices (including social housing and milk allowance) and behaviors in dairy calves, and 2) aspects of the human-animal relationship in calf rearing and relationships with on-farm management and personal calf handling habits. Survey questions were primarily quantitative (e.g., Likert scales) and addressed how common calf management practices and observed calf behaviors were related to aspects of welfare, including calf health and comfort. We additionally posed questions addressing respondent habits, management protocols, and perceptions related to human-animal interaction. Responses from 93 dairy producers and calf managers were collected via digital surveys. Social housing was viewed as being generally positive for both calf comfort and health, although this view was stronger with respect to calf comfort. Respondents from farms using social housing (56%) had more positive perceptions of social housing, viewed social play as being associated with better calf comfort and health, and considered access to other calves and "freedom to express natural behavior" as being more important for calves, compared with respondents from farms not providing social housing. Providing greater milk allowances (>7.6 L/d) was viewed as being good for both calf comfort and health, although respondents from farms providing these milk allowances (59%) had more positive perceptions than those who provided lesser allowances. Abnormal oral behaviors were viewed as being associated with both poor calf comfort and health. The welfare importance of various resources which may reduce abnormal oral behaviors (including hay and brushes) was perceived more ambiguously, although respondents from farms providing these resources, compared with those who do not, generally viewed them as more preferred by calves. We observed a positive relationship between how respondents perceived the human-animal bond (i.e., that calves enjoy contact with humans) and stated personal behavior related to calf contact (frequency of contacting calves to scratch or pet them). Respondent demographics were not related to perceptions of the human-animal relationship, but respondents identifying as female described more frequent positive calf interactions. Described aspects of human-animal interactions were not related to implementation of social housing on-farm. Job satisfaction was positively related to perception of the human-animal relationship. Overall, these results suggest that most calf management personnel place a high value on calf welfare, although farms implementing social housing appear to place a greater value on subjective calf well-being and individual perceptions of animal welfare may depend on practical experience.

    Abstract Background: The Ehlers-Danlos Syndromes (EDS) are a group of connective tissue disorders that are hereditary in nature and characterized by joint hypermobility and tissue fragility. The complex nature of this unique patient population requires multidisciplinary care, but appropriate centers for such care do not exist in large portions of the country. Need for more integrated services has been identified in Chicagoland, or Chicago and its suburbs. In order to explore and begin to address barriers to seeking appropriate care facing EDS patients in this region, we developed an online survey which we circulated through EDS social media groups for Chicagoland patients.
    Results: Three hundred and nine unique respondents participated. We found that there exists a strong medical need for and interest in the development of a center in the region, and participants reported that, if made available to them, they would make extensive and regular use of such a facility.
    Conclusions: We conclude that the establishment of a collaborative medical center specializing in the diagnosis and treatment of EDS, Hypermobility Spectrum Disorder, and related disorders in the Chicagoland area would greatly benefit patients by providing comprehensive care, alleviate the burden on overworked healthcare providers, and contribute to the sustainability of medical facilities.

    Abstract Antimicrobial use (AMU) in the food chain is a potential driver of antimicrobial resistance. Despite Australia's strong regulation of AMU limited to veterinary prescriptions, a proportion of empirical antimicrobial treatments are administered by dairy farmers to manage common cattle health problems. This cross-sectional survey identified key influences on AMU by dairy cattle farmers within New South Wales, Australia, to detect opportunities for antimicrobial stewardship (AMS) engagement. The study identified existing relationships, resources and attitudes of the dairy farmers that could be optimised for on-farm AMS strategies. Farmers were most highly influenced by veterinary advice and clinical signs of the animal followed by the withholding period and the potential for antimicrobial resistance development. Farmers' high confidence regarding their own knowledge of antimicrobials (>90%), their high regard for veterinary advice (>90%) and high rate of veterinary health care plan use (69%) provides a strong framework to build the profile and practice of AMS on dairy farms. Positive engagement by dairy farmers (survey response of 20%), was achieved by working with the NSW Food Authority. Despite respondents reporting low reliance on formal (government and commercial) organisations for information about AMU, their engagement demonstrates an opportunity for groups with unparalleled access to dairy farmers to drive AMS. An association between frequent use of veterinary advice and respondents keeping ceftiofur on-farm requires further investigation. Quantitative and qualitative analysis of on-farm resources, decision-making, and practices is required to understand how practices relate to veterinary advice and accepted standards of appropriate AMU on dairy farms.

    Abstract Bayesian networks are increasingly used to quantify the uncertainty of subjective and stochastic concepts such as trust. In this article, we propose a data-driven approach to estimate Bayesian parameters in the domain of wearable medical devices. Our approach extracts the probability of a trust factor being in a specific state directly from the devices (e.g. sensor quality). The strength of the relationship between related factors is defined by expert knowledge and incorporated into the model. We use propagation rules from requirements engineering to estimate how much each trust factor contributes to the related intermediate nodes in the network and ultimately compute the trust score. The trust score is a relative measure of trustworthiness when different devices are evaluated in the same test conditions and using the same Bayesian structure. To evaluate our approach, we developed Bayesian networks for the trust quantification of similar wearable devices from two manufacturers under identical test conditions and noise levels. The results demonstrated the learnability and generalizability of our approach.

    Abstract Background: Patients with hypermobile Ehlers-Danlos Syndrome (hEDS) often make use of complementary and alternative medical (CAM) techniques to manage their chronic pain and other symptoms. Nevertheless, how they use CAM, which techniques they favor, and how CAM use affects their allopathic care remain unclear. The purpose of this qualitative study was to understand patients' personal experiences with CAM and its role in their symptom management.
    Materials and methods: Thirty individuals living with hEDS completed a brief online survey related to their CAM use. Thereafter, in-depth interviews were conducted with 24 of the survey respondents, qualitatively investigating their experiences with CAM. Data were analyzed using thematic analysis.
    Results: Participants described massage therapy (
    Conclusion: Participants made extensive use of CAM therapies. They described both critical benefits as well as harms from the use of these non-conventional modalities. These results underscore the importance of clinicians maintaining communicative and compassionate relationships with their patients, and of an openness to the discussion and use of CAM treatments.
