    Abstract This prospective study sought to determine the effect of a post-discharge telephone call on 30-day readmission rates at a short-term inpatient psychiatric facility. The patient population consisted of English-speaking adults in the age group of 18-65 years with a high school education or higher and met involuntary commitment criteria. Participants received a telephone call 72 hours after discharge using a standard script. Three hundred and forty-two patients completed the study. A Pearson Chi-Square test was used to analyze the data. We used an alpha level of 0.05 for all statistical tests. Total readmissions were reduced by 3.4% (p = 0.004) during the time of the intervention when compared to patients receiving standard care the year prior. During the intervention period, the average readmission rate was 9.9% (95%CI 5.9-12.1%). We conclude that systematic telephone follow-up has the potential to reduce 30-day readmission rates for inpatient psychiatric facilities.

    Abstract Social media is a mainstay in American culture, but it's impact on mental health continues to evolve. Social media has revolutionized the way people share information and communicate. It is particularly popular among adolescents and young adults ages 18-24. There has been increasing concern that the internet and social media may be influencing suicidal behavior and self-harm in this population. Publications focusing on the impact of social media use on suicidal/self-injurious behavior are beginning to emerge in the literature. This clinical vignette chronicles the events of a suicide attempt documented on the social media application Snapchat. It facilitates a discussion about the role social media played in influencing this behavior.

    Abstract Opioid addiction in the United States currently presents a national crisis despite availability of different treatments. Prior findings suggest that both repetitive transcranial magnetic stimulation (rTMS) and subanesthetic dose of ketamine are efficacious in patients with opioid use disorders (OUD) when administered in isolation. However, their therapeutic value may be undermined by varying clinical responses that tend to dissipate with treatment cessation. There has been no study to date that has used a combination of both for OUD, and there are still many unanswered questions with respect to both. TIMBER (Trauma Interventions using Mindfulness Based Extinction and Reconsolidation of memories) therapy attempts to alter the expression of emotionally charged memories such as traumatic memories, and has been successfully tried in chronic post-traumatic stress disorder (PTSD) and in combination with memory-altering pharmacotherapy like ketamine infusion. By a combination of extinction and reconsolidation of memory approaches, TIMBER works to not over-flood and/or retraumatize as is seen in other treatment approaches. TIMBER involves a balanced combination of both the memory extinction and memory reconsolidation approaches (rather than extinction-only approaches) which explains its superior efficacy in PTSD and benefit in substance use disorders.

    Abstract Huntington's disease (HD) is a rare neurodegenerative disease of the central nervous system characterized by choreatic movements, behavioral disturbances, and neuropsychiatric sequelae. The disease is inherited in an autosomal dominant fashion by an increased number of CAG repeats on the short arm of chromosome 4p16.3 in the Huntingtin gene. Huntington's disease demonstrates the genetic principle of anticipation, where the larger the number of CAG repeats the earlier the signs and symptoms of the disease appear in subsequent generations. The symptoms often consist of behavioral disturbances and learning disturbances. The disease is suspected based on signs and symptoms and confirmed by genetic testing. There is no cure for the disease, and there is a high rate of neuropsychiatric symptoms including depression, and aggressive behavior. A significant risk of suicide in this population exists given the severity and unrelenting nature of the disease. Most patients will have multiple hospitalizations during the course of the illness. A consultant psychiatrist may be asked to evaluate and make recommendations for the treatment of acute agitation in HD patients. This can be a challenging task given the limited number of studies and the complex nature of agitation in the hospital setting. The aim of this review is to look at the currently available data for the treatment of acute agitation in patients with Huntington's disease.

    Abstract A patient presented to the emergency department with his brother due to recent onset of paranoid behavior that escalated over the past month. The patient endorsed paranoid delusions of people watching him and following him in a black truck. The patient admitted to being unable to sleep for the past two weeks and to having hypervigilant behavior whenever he leaves the house. Due to the patient's presentation, the differential diagnosis included schizophrenia, substance-induced psychotic disorder, psychotic disorder due to another medical condition, bipolar disorder, and major depressive disorder with mood incongruent psychotic features. Upon interview, the patient stated he was using marijuana to decrease his self-reported anxiety and other social stressors since the age of 13 years. Over the past month, the patient says he has been "dabbing," which is a highly concentrated form of cannabis mainly consumed by experienced users. These "dabs" have an extremely high tetrahydrocannabinol (THC) content (up to 80%), which is the main psychoactive component in cannabis products. Since the patient began using this potent form of cannabis, he has had increasing difficulty functioning at work and worsening symptoms of psychosis. After eliciting this information and noting the patient had a positive urine drug screen for cannabis, a diagnosis of cannabis-induced psychosis along with severe cannabis use disorder was made. There is a trend towards increasing THC concentrations in cannabis products. This case highlights the importance of being aware of these highly potent cannabis products and their potential harms. Patients should approach these products with caution, as they are not only more dangerous to manufacture but also have the potential to induce psychosis in susceptible populations.

    Abstract Ciprofloxacin is a broad-spectrum antibiotic that is generally well-tolerated and widely used in clinical practice to treat various forms of infection. There have been several case reports detailing the development of psychosis and other neuropsychiatric symptoms secondary to use of this antibiotic. This case report details the history of an acute paranoid psychosis that developed in a patient treated with ciprofloxacin for epididymitis.

    Abstract Paraphilic disorder in patients with autism spectrum disorder (ASD) can be extremely disturbing to both the patient and caretakers. It can interfere with these patient's ability to develop social skills, which are essential for adaptation, and function within society. This case report details the history of an 18-year-old male patient previously diagnosed with Asperger's syndrome, who also exhibited symptoms consistent with paraphilic disorder. A review of the literature was then conducted to determine if there is any documented association of paraphilic disorder, or abnormal sexual behavior in patients with ASD. Several case reports involving patients with ASD, and co-morbid paraphilic disorder have been described in the literature. ASD appears to be associated with paraphilic disorder.

    Abstract The diagnosis of cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) in adults can be challenging. Initially, this disease can mimic embolic cerebral infarction, multiple sclerosis, and other neurological diseases on imaging studies. CADASIL is the most common hereditary cerebral angiopathy which is inherited in an autosomal dominant fashion. There is a wide variety of clinical presentations including a migraine headache, mood disturbances, cognitive dysfunction, and recurrent subcortical cerebral infarctions. This case details the hospital course and diagnosis of a 41-year-old male who initially presented with symptoms consistent with his previous diagnosis of a hemiplegic migraine who was later found to have biopsy-positive CADASIL after the symptoms failed to remit.
