    Abstract Introducción: El uréter retrocavo es una anomalía congénita rara que requiere manejo quirúrgico.
    Objetivo: Reportar los resultados en el manejo laparoscópico para uréter retrocavo y hacer una revisión bibliográfica.
    Método: Estudio retrospectivo, descriptivo, transversal, en pacientes con diagnóstico de uréter retrocavo que fueron sometidos a plastia laparoscópica de uréter retrocavo.
    Resultados: Cuatro pacientes sometidos a plastia laparoscópica por diagnóstico de uréter retrocavo, 1 hombre y 3 mujeres, con una media de edad de 40.7 años. La media del tiempo operatorio fue de 138.2 minutos. La pérdida de sangre cuantificada transoperatoria media fue de 23.7 ml. Ningún paciente presentó complicaciones transoperatorias ni posoperatorias. Los pacientes permanecieron hospitalizados hasta su egreso una media de 2.5 días. Todos los pacientes presentaron mejoría de su sintomatología.
    Conclusiones: El uréter retrocavo es una condición rara y aún no existe un consenso sobre su diagnóstico, tratamiento y seguimiento. El manejo quirúrgico laparoscópico es una opción para el tratamiento de estos pacientes.
    Introduction: The retrocave ureter is a rare congenital anomaly that requires surgical management.
    Objective: Report the results in laparoscopic management for retrocave ureter and literature review.
    Method: Retrospective, descriptive, cross-sectional study in patients with a diagnosis of retrocave ureter who were sometimes a laparoscopic retrocave ureter.
    Results: Four patients sometimes underwent laparoscopic plasty for diagnosis of retrocave ureter, 1 man and 3 women, with a mean age of 40.7 years. The mean operative time was 138.2 minutes. The mean intraoperative quantified blood loss was 23.7 ml. No patient presented transoperative or postoperative complications. The patients remained hospitalized until their progress with an average of 2.5 days. All patients visualize improvement in their symptoms.
    Conclusion: The retrocave ureter is a rare condition and there is still no consensus on its diagnosis, treatment and follow-up. Laparoscopic surgical management is an option for the treatment of these patients.

    Abstract Esta investigación tiene como objetivo establecer la incidencia de la participación ciudadana en la construcción de la política pública de juventud durante la Administración de Luis Eduardo Garzón a partir de tres aspectos centrales: 1) La identificación de la apuesta política hacia la participación del sector juvenil y el enfoque que orientó la formulación de la política, 2) la caracterización de la participación ciudadana durante el proceso de construcción, 3) la incidencia de la participación ciudadana establecida a partir de la relación entre las nociones y lecturas de los actores involucrados en el proceso, y el modelo adoptado durante la construcción de la política. La hipótesis que orienta la investigación plantea que durante la administración de Luis Eduardo Garzón se avanza normativa y conceptualmente en ubicar la participación ciudadana como un elemento clave para la construcción de las políticas sociales. Sin embargo, para el caso de la política pública de juventud, estos avances no garantizan que la participación del sector juvenil incida significativamente en el proceso de construcción. Esta investigación se define bajo un enfoque cualitativo y un estudio de casos (localidades de Fontibón y de Antonio Nariño), a partir de los cuales se aborda el tema por estudiar.

    Abstract Immune checkpoint inhibitors (ICIs) are a group of drugs that have improved outcomes for patients with various cancers. Generally considered safe and well tolerated, these drugs are occasionally linked to immune-mediated or immune-related adverse events. Among these, autoimmune neurological events are rare, displaying varying incidence rates across different studies. Peripheral neuropathy, although one of the more common neurological immune-related events, is at times underestimated. This case report highlights an adult patient diagnosed with metastatic intrahepatic cholangiocarcinoma. Initially, the patient underwent chemoimmunotherapy with gemcitabine, cisplatin, and durvalumab for eight cycles, achieving partial response without significant toxicity. Following this, the patient continued with maintenance monotherapy with durvalumab every 28 days. After completing six cycles of maintenance therapy, the patient suddenly experienced paresthesia and hypoesthesia in four limbs, accompanied by apraxia in the hands that was more pronounced on the right side. Additionally, the patient reported neuropathic pain in the right arm and encountered limitations in certain instrumental activities of daily living. Diagnostic studies, including laboratory and electrophysiological studies, combined with the clinical presentation, identified immune-related peripheral polyneuropathy. Durvalumab was suspended and prednisolone therapy was initiated, resulting in a rapid resolution of all neuropathic symptoms. In addition to the clinical case, this article reviews the literature on immunotherapy-associated peripheral neuropathy.

    Abstract Metformin is the preferred medication for the initial management of type 2 diabetes mellitus (T2DM). Although its use is widely recommended, caution should be exercised when prescribing it to populations susceptible to systemic hypoperfusion conditions, as it can lead to accumulation in the body and metabolic disturbances that may result in metformin-associated lactic acidosis. This severe complication is often underdiagnosed. To promote a better understanding of this topic, the present review focuses on the analysis of the clinical, pathophysiological, diagnostic, and management aspects of metformin-associated lactic acidosis, with particular attention to management through renal replacement therapies. The analysis will be based on the experience of a series of cases of metformin-associated lactic acidosis treated at a hospital clinical center in Chile.

    Abstract Pitt-Hopkins syndrome (PTHS) is a neurodevelopmental disorder caused by haploinsufficiency of transcription factor 4 (TCF4). In this work, we focused on the cerebral cortex and investigated in detail the progenitor cell dynamics and the outcome of neurogenesis in a PTHS mouse model. Labeling and quantification of progenitors and newly generated neurons at various time points during embryonic development revealed alterations affecting the dynamic of cortical progenitors since the earliest stages of cortex formation in PTHS mice. Consequently, establishment of neuronal populations and layering of the cortex were found to be altered in heterozygotes subjects at birth. Interestingly, defective layering process of pyramidal neurons was partially rescued by reintroducing TCF4 expression using focal in utero electroporation in the cerebral cortex. Coincidentally with a defective dorsal neurogenesis, we found that ventral generation of interneurons was also defective in this model, which may lead to an excitation/inhibition imbalance in PTHS. Overall, sex-dependent differences were detected with more marked effects evidenced in males compared with females. All of this contributes to expand our understanding of PTHS, paralleling the advances of research in autism spectrum disorder and further validating the PTHS mouse model as an important tool to advance preclinical studies.

    Abstract During the operation of the landfills, leachate should be managed with caution to avoid possible negative environmental impacts. Considering this, the present study aims to evaluate the relationship between different variables in the leachate composition and elucidate the transformation processes through which this effluent passes during the landfill's period of operation. The study was conducted with eight sanitary landfills from the state of Minas Gerais, in southeastern Brazil, and used descriptive statistical analysis, principal component analysis (PCA), correlation analysis, and calculation of the leachate pollution index (LPI).     Abstract Degenerative Cervical Myelopathy (DCM) is the most common cause of spinal cord impairment in elderly populations. It describes a spectrum of disorders that cause progressive spinal cord compression, neurological impairment, loss of bladder and bowel functions, and gastrointestinal dysfunction. The gut microbiota has been recognized as an environmental factor that can modulate both the function of the central nervous system and the immune response through the microbiota-gut-brain axis. Changes in gut microbiota composition or microbiota-producing factors have been linked to the progression and development of several pathologies. However, little is known about the potential role of the gut microbiota in the pathobiology of DCM. Here, DCM was induced in C57BL/6 mice by implanting an aromatic polyether material underneath the C5-6 laminae. The extent of DCM-induced changes in microbiota composition was assessed by 16S rRNA sequencing of the fecal samples. The immune cell composition was assessed using flow cytometry. To date, several bacterial members have been identified using BLAST against the largest collection of metagenome-derived genomes from the mouse gut. In both, female and males DCM caused gut dysbiosis compared to the sham group. However, dysbiosis was more pronounced in males than in females, and several bacterial members of the families Lachnospiraceae and Muribaculaceae were significantly altered in the DCM group. These changes were also associated with altered microbe-derived metabolic changes in propionate-, butyrate-, and lactate-producing bacterial members. Our results demonstrate that DCM causes dynamic changes over time in the gut microbiota, reducing the abundance of butyrate-producing bacteria, and lactate-producing bacteria to a lesser extent. Genome-scale metabolic modeling using gapseq successfully identified pyruvate-to-butanoate and pyruvate-to-propionate reactions involving genes such as Buk and ACH1, respectively. These results provide a better understanding of the sex-specific molecular effects of changes in the gut microbiota on DCM pathobiology.

    Abstract Resumo A Revista Brasileira de Saúde Ocupacional (RBSO) é um periódico considerado referência pela comunidade da área de segurança e saúde do trabalhador, sendo veículo dedicado à publicação de artigos científicos que contribuam para a reflexão e análise científica de questões atuais e relevantes relacionadas às relações entre trabalho e saúde. Em 2023, a RBSO completa 50 anos de existência. Esta nota editorial dedica-se a relatar o evento comemorativo, descrevendo os principais aspectos abordados nas apresentações e nos debates ocorridos, com vistas a registrar este importante momento, que pode ser considerado um marco para celebrar o passado e construir o futuro da revista. Tal evento foi uma oportunidade crucial, não somente para celebrar o meio século de existência da RBSO, mas também para propiciar o encontro de uma comunidade dedicada à publicação científica em saúde do trabalhador, que proporcionou reflexões, propostas e recomendações voltadas ao fortalecimento da revista e do campo. A presença e o envolvimento da equipe editorial, bem como dos convidados, foram fundamentais para o sucesso desse encontro, durante o qual a RBSO reafirmou o compromisso com a busca contínua por aprimoramento editorial, a partir das experiências e ideias debatidas.
