    Abstract Objectives: Visiting a dentist can be an unpleasant experience for many people. Especially clinical procedures for provision of fixed dental prostheses (FDP) can be burdensome. The aim of this study was to assess the impact of media entertainment via flat screens on the ceiling on patient experiences during dental treatment for FDP.
    Methods: In this randomized controlled clinical trial (RCT), a sample of 145 patients (mean age: 42.7 years, 55.2% female) undergoing treatment for FDP was recruited and randomly assigned to intervention group (n = 69) with media entertainment or control group without media (n = 76). Perceived burdens were assessed with the 25-item Burdens in Prosthetic Dentistry Questionnaire (BiPD-Q). Total and dimension scores can range from 0 to 100 with higher scores representing higher burdens. Impact of media entertainment on perceived burdens was assessed using t-test and multivariate linear regression analysis. Effect sizes (ES) were calculated.
    Results: Perceived burdens were in general quite low indicated by a mean BiPD-Q total score of 24.4 points, with highest scores for the domain preparation (28.9) and lowest for global treatment aspects (19.8). Media entertainment had a significant effect on overall perceived burdens with lower scores in the intervention group (20.0) than in the control group (29.2; ES: 0.54; p = 0.002). Highest impact was observed for the domains global treatment aspects (ES: 0.61; p < 0.001) and impression (ES: 0.55; p = 0.001), and lowest for anesthesia (ES: 0.27; p = 0.103).
    Conclusion: Media entertainment on flat screens during dental treatments decreases perceived burdens and may provide patients a less unpleasant treatment experience.
    Clinical significance: Long and invasive treatments as performed for providing fixed dental prostheses may induce substantial burdens to the patients. Media entertainment on a flat screen TV on the ceiling has a significant attenuating effect on patients and significantly reduces perceived burdens, and subsequently improves process-related quality of care in dentistry.

    Abstract Introduction: The African Federation of Emergency Medicine (AFEM) recommends the use of emergency point-of-care ultrasound (ePOCUS) as a core skill for health care practitioners in Africa. The study explored the use of ePOCUS by health care practitioners among AFEM members who work across Africa.
    Methods: An anonymous online survey was distributed to individual members of AFEM and affiliated organisations. The questionnaire was tested by the AFEM Scientific Committee for potential content modifications prior to distribution. Summary statistics are presented.
    Results: Of the 220 participants that were analysed, 148 (67.3%) were using ePOCUS. The mean age was 36 years; 146 (66%) were male; and 198 (90%) obtained their primary medical qualification in Africa. In total, 168 (76%) were doctors, and most participants (
    Conclusion: Access barriers to ePOCUS training, mentorship, equipment and consumables are still relevant in Africa. The low credentialing rate and the potential discordance between local burden of disease and ePOCUS training requires further investigation.

    Abstract Guillain-Barre syndrome (GBS) is an autoimmune disease of demyelination and inflammation of peripheral nerves. Current treatments are limited to plasma exchange and intravenous immunoglobulins. Cargo-free nanoparticles (NPs) have been evaluated here for their therapeutic benefit on the disease course of experimental autoimmune neuritis (EAN), mimicking the human GBS. NPs prepared from poly-lactic co-glycolic acid (PLGA) with variable size and surface charge (i.e., 500 nm vs. 130 nm, polyvinyl alcohol (PVA) vs. sodium cholate), were intravenously administered in before- or early-onset treatment schedules in a rat EAN model. NP treatment mitigated distinctly the clinical severity of EAN as compared to the P2-peptide control group (P2) in all treatments and reduced the trafficking of inflammatory monocytes at inflammatory loci and diverted them towards the spleen. Therapeutic treatment with NPs reduced the expression of proinflammatory markers (CD68 (P2: 34.8 ± 6.6 vs. NP: 11.9 ± 2.3), IL-1β (P2: 18.3 ± 0.8 vs. NP: 5.8 ± 2.2), TNF-α (P2: 23.5 ± 3.7 vs. NP: 8.3 ± 1.7) and elevated the expression levels of anti-inflammatory markers CD163 (P2: 19.7 ± 3.0 vs. NP: 41.1 ± 6.5; all for NP-PVA of 130 nm; relative to healthy control). These results highlight the therapeutic potential of such cargo-free NPs in treating EAN, which would be easily translatable into clinical use due to their well-known low-toxicity profile.

    Abstract Introduction: The African Federation of Emergency Medicine (AFEM) recommends the use of emergency point-of-care ultrasound (ePOCUS) as a core skill for health care practitioners in Africa. The study explored the use of ePOCUS by health care practitioners among AFEM members who work across Africa. Methods: An anonymous online survey was distributed to individual members of AFEM and affiliated organisations. The questionnaire was tested by the AFEM Scientific Committee for potential content modifications prior to distribution. Summary statistics are presented. Results: Of the 220 participants that were analysed, 148 (67.3%) were using ePOCUS. The mean age was 36 years; 146 (66%) were male; and 198 (90%) obtained their primary medical qualification in Africa. In total, 168 (76%) were doctors, and most participants (n = 204, 93%) have worked in Africa during the last 5 years. Reasons for not using ePOCUS mainly related to lack of training and problems with ultrasound machines or consumables. Most ePOCUS users (116/148, 78%) attended courses with hands-on training, but only 65 (44%) participants were credentialed (by 18 different organizations). The median score for self-perceived level of ePOCUS skills was 75 in credentialed users versus 50 in those that were not credentialed. Ultrasound in trauma was the most frequently used module (n = 141, 99%), followed by focused cardiac assessment (n = 128, 90%) and thoracic (including lung) assessment (n = 128, 90.1%). The FASH-module (Focused Assessment with Sonography for HIV/TB) was the least used (n = 69, 49%). Conclusion: Access barriers to ePOCUS training, mentorship, equipment and consumables are still relevant in Africa. The low credentialing rate and the potential discordance between local burden of disease and ePOCUS training requires further investigation.

    Abstract Objectives: The gut-liver axis is discussed to play an important role in hepatic cirrhosis. Decompensated liver cirrhosis is associated with portal hypertension, which can lead to a variety of complications. Transjugular intrahepatic portosystemic shunt (TIPS) is an established treatment option for the complications of portal hypertension. In this study we focused on the effect of TIPS on intestinal microbial composition in cirrhotic patients.
    Methods: Thirty patients with liver cirrhosis were compared to 18 healthy adults. Seventeen patients with cirrhosis and portal hypertension received a TIPS. Clinical characteristics, including age, sex, and liver function measured with a Child-Pugh score and model for end-stage liver disease score, were obtained. Intestinal microbial composition was assessed via 16S rRNA gene amplicon sequencing from stool probes before and after TIPS.
    Results: TIPS led to a reduction of hepatic venous pressure gradient. However, TIPS did not cause a shift in the intestinal bacterial communities. Independent from the application of TIPS, antibiotic therapy was associated with a significant difference in the intestinal bacterial microbiota and also a reduced α-diversity. In addition, a significant difference was observed in the intestinal bacterial composition between patients with liver cirrhosis and healthy controls.
    Conclusion: The presence of liver cirrhosis and the use of antibiotic therapy, but not the application of TIPS, were associated with a significant shift of the intestinal bacterial communities, showing a high impact on the microbiota of patients with liver cirrhosis.

    Abstract Most clinical studies in dentistry are conducted in university-based settings. However, whether findings can be generalised to private dental practices is not clear. Aims of this study were to determine patients' criteria for choosing a dentist, and to assess whether criteria differ between patients in university dental clinic and private dental practices. In this cross-sectional study, a convenience sample of 300 subjects involving dental patients of a university dental clinic and private dental practices, and subjects from general population (each n = 100) were included. Criteria for choosing a dentist were assessed with a 31-item questionnaire, covering socio-demographic characteristics, qualifications, psychosocial skills, practice structure, practice offers and accessibility, with a 6-point ordinal rating scale ranging from "very crucial"-(5) to "not crucial at all"-(0). A total of 189 subjects (63%) provided completed questionnaires for analyses. Dentists' psychosocial skills (mean 4.4) were rated as most important when choosing a dentist, whereas socio-demographic characteristics were of almost no significance (mean 1.0). Significant differences between settings were observed only for practice offers and accessibility with highest ratings in university setting. However, after controlling for potential confounders, ratings differed only slightly and not statistically significantly. In conclusion, dentists' psychosocial skills appear to be most important criteria for choosing a dentist. Setting-related differences in criteria seem to be likely but obviously result from differences in socio-demographic characteristics.

    Abstract Objectives: Orofacial appearance is increasingly recognized as an important dental patient-reported outcome making instrument development and refinement efforts to measure the outcome better necessary. The aim of this study was to derive a one-item version of the Orofacial Esthetic Scale (OES).
    Materials and methods: OES data were collected from a consecutive sample of a total of 2113 adult English- or Spanish-speaking dental patients from HealthPartners dental clinic in Minnesota. Participants with missing data were excluded and analysis were performed using data from 2012 participants. Orofacial appearance was assessed with the English and the Spanish language version of the OES. Linear regression analysis was performed, with the OES item 8 ("Overall, how do you feel about the appearance of your face, your mouth, and your teeth?") as the predictor variable and the OES summary score as the criterion variable, to calculate the adjusted coefficients of determination (R

    Conclusion: The OES item 8 can be used for the one-item OES (OES-1). It is a psychometrically sound instrument for measuring orofacial appearance.
    Clinical relevance: Due to its easy application and sufficient psychometric properties, the OES-1 can be used effectively as an alternative to longer OES instruments in all areas of dental practice and research.

    Abstract Inflammatory bowel disease (IBD) is a chronic inflammatory disease of the gastrointestinal tract with increasing incidence worldwide. Although a deeper understanding of the underlying mechanisms of IBD has led to new therapeutic approaches, treatment options are still limited. Severe adverse events in conventional drug therapy and poor drug targeting are the main cause of early therapy failure. Nanoparticle-based targeting approaches can selectively deliver drugs to the site of inflammation and reduce the risk of side effects by decreasing systemic availability. Here, we developed a nanoparticulate platform for the delivery of the anti-TNF-α antibody adalimumab (ADA) by covalent crosslinking to the particle surface. ADA binding to nanoparticles improved the stability of ADA against proteolytic degradation in vitro and led to a significantly better therapeutic outcome in a murine colitis model. Moreover, immobilization of ADA reduced systemic exposure, which can lead to enhanced therapeutic safety. Thus, nanoparticle protein decoration constitutes a platform through which epithelial delivery of any biological of interest to the inflamed gut and hence a local treatment can be achieved.
