    Abstract Objective: Blood transfusion therapy (BTT) is widely used in trauma patients. However, the adverse effects of BTT in pediatric trauma patients with traumatic brain injury (TBI) were poorly studied. The objective of this study is to evaluate the effect of BTT on mortality in children with severe TBI.
    Methods: In this retrospective cohort analysis, we analyzed 2012 and 2016 Kids' Inpatient Databases and used a weighted sample to obtain national outcome estimates. We included children aged 1 month to 21 years with TBI who were mechanically ventilated, considered severe TBI; we then compared the demographics, comorbidities, and mortality rates of those patients who had undergone BTT to those who did not. Statistical analysis was performed using the chi-squared test and regression models. In addition, in a correlative propensity score matched analysis, cases (BTT) were matched 1:1 with controls (non-BTT) based on age, gender, hospital region, income quartiles, race, and All Patients Refined Diagnosis Related Groups (APRDRG) severity of illness scores to minimize the effect of confounding variables between the groups.
    Results: Out of 87,980 children with a diagnosis of TBI, 17,199 (19.5%) with severe TBI were included in the analysis. BTT was documented in 3184 (18.5%) children. Among BTT group, the mortality was higher compared to non-BTT group [31.6% (29.7-33.5%) vs. 14.4 (13.7-15.1%), (OR 2.2, 95% CI 1.9-2.6; p<0.05)]. In the BTT group, infants and adolescents, white race, APRDRG severity of illness, cardiac arrest, platelet, and coagulation factor transfusions were associated with higher mortality. In a propensity-matched analysis, BTT associated with a higher risk of mortality (32.1% [30.1-34.2] vs. 17.4% [15.8-19.1], p<0.05; OR: 2.2, 95% CI: 1.9-2.6).
    Conclusion: In children with severe TBI, blood transfusion therapy is associated with higher mortality.

    Abstract Independent components within low-dimensional representations are essential inputs in several downstream tasks, and provide explanations over the observed data. Video-based disentangled factors of variation provide low-dimensional representations that can be identified and used to feed task-specific models. We introduce MTC-VAE, a self-supervised motion-transfer VAE model to disentangle motion and content from videos. Unlike previous work on video content-motion disentanglement, we adopt a chunk-wise modeling approach and take advantage of the motion information contained in spatiotemporal neighborhoods. Our model yields independent per-chunk representations that preserve temporal consistency. Hence, we reconstruct whole videos in a single forward-pass. We extend the ELBO's log-likelihood term and include a Blind Reenactment Loss as an inductive bias to leverage motion disentanglement, under the assumption that swapping motion features yields reenactment between two videos. We evaluate our model with recently-proposed disentanglement metrics and show that it outperforms a variety of methods for video motion-content disentanglement. Experiments on video reenactment show the effectiveness of our disentanglement in the input space where our model outperforms the baselines in reconstruction quality and motion alignment.

    Abstract Cryopyrin-associated periodic syndrome is a rare and probably underdiagnosed disease. It presents with various systemic manifestations, including ophthalmological, making its diagnosis a challenge for the clinician. We present the case of a 4-year-old girl for which the identification of papilledema in the ophthalmological examination was the key sign for the diagnosis of cryopyrin-associated periodic syndrome. Our aim is to raise awareness of this syndrome with serious implications for affected patients, so that it is taken into account more frequently as a differential diagnosis, allowing an early diagnosis.

    Abstract Treatment with recombinant growth hormone, or somatropin, increases linear growth and is effective in improving final height in children with isolated growth hormone deficiency (IGHD), however, the available information of these results in the Latin population is scarce.
    Objective: To evaluate the effect of somatropin on growth velocity and final height in Mexican children with IGHD, as well as to determine the factors associated with final height.
    Patients and method: A retrospective study was conducted in 50 children with isolated and severe growth hormone deficiency treated with somatro pin. Auxological characteristics were assessed before somatropin and at final height. Only patients with severe GH deficiency, with a peak GH value < 5 μg/L, were included.
    Results: 40% (n = 20) were girls. The basal height Z-score was -2.6 ± 0.4 vs final height Z-score 1.6 ± 0.7, with a Z-score of height in crease of 1 ± 0.6. Ninety-two percent (n = 46) reached their genetic potential; somatropin dose was 33 ± 0.5 μg/kg/d, with a treatment duration of 4.16 ± 1.5 years. The highest growth velocity was observed during the first year. In the multivariate analysis, the association between final height, mid-parental target height (r = 0.30; p = 0.03, β = 0.7; p = 0.001), and somatropin dose (r = 0.63; p = 0.001, β = 0.30; p = 0.028) was observed.
    Conclusions: Somatropin treatment allows normalization of linear growth and the achievement of genetic height potential in most Mexican children with IGHD. Final height is associated with mid-parental height and somatropin dose, highlighting the importance of genetic potential and the dose-response effect of somatropin in establishing height prognosis.

    Abstract Ethnopharmacological relevance: Eryngium cymosum F. Delaroche was detected as a traditional remedy against type 2 diabetes consumed by patients of Tlanchinol in the state of Hidalgo, Mexico.
    Aim of the study: Assessing the hypoglycemic effect and safety of the traditional extract of E. cymosum and relating it to key glucose-lowering mechanisms both in fasting and postprandial state.
    Materials and methods: The aqueous extract of E. cymosum was subjected to HPLC analysis to identify its main components. Hyperglycaemic STZ-NA Wistar rats were administered with the extract to evaluate its effect on blood glucose levels and a possible dose-dependence. Afterward, it was evaluated in both pyruvate and maltose tolerance tests in STZ-NA rats to characterize its effect on gluconeogenesis and carbohydrate breakdown, two of the main mechanisms responsible for fasting and postprandial hyperglycaemia in type 2 diabetes patients. In addition, the inhibitory capacity of the extract was evaluated on key enzymes involved in gluconeogenesis and a-glucosidases. Moreover, insulin concentrations were measured in normoglycemic rats in both conditions to establish a link between the hypoglycaemic effect of the extract with insulin release and functioning.
    Results: Caffeic acid (1), chlorogenic acid (2), and rosmarinic acid (3) were identified as the main constituents of the aqueous extract of E. cymosum, which exerted a hypoglycaemic effect in hyperglycaemic STZ-NA rats. It has a significant antihyperglycemic effect in the pyruvate tolerance test, and it was able to reduce the postprandial hyperglycaemia in maltose tolerance tests significantly. Moreover, it effectively reduced the activity of both gluconeogenic enzymes reaching almost 100% of inhibition, while it presented a modest 32% inhibition of aglucosidases. On the other hand, the extract decreased insulin levels after its oral administration in healthy rats in both nutritional states, without affecting normoglycemia in normal curves and reducing the postprandial peak in glucose load curves.
    Conclusions: The traditional consumed form of aerial parts of E. cymosum is safe and regulated glucose levels both in fasting and in postprandial state.

    Abstract Antecedentes: La pancreatitis aguda leve es una inflamación local del páncreas sin complicaciones locales ni falla orgánica. Su manejo consiste en tres elementos básicos: hidratación, analgesia y ayuno. La realimentación se inicia cuando el paciente no tiene dolor y refiere apetito, pero el momento exacto para iniciarla no está previamente documentado.
    Objetivo: Determinar la tolerancia a la alimentación oral inmediata (8 horas posterior al inicio del manejo) en comparación con la alimentación temprana (48 horas) en los pacientes con pancreatitis aguda biliar leve.
    Método: Se incluyeron pacientes con pancreatitis aguda biliar leve y se aleatorizaron en dos grupos: A) alimentación temprana a las 48 horas (30 pacientes) y B) alimentación inmediata a las 8 horas de inicio del manejo (29 pacientes). Fueron evaluados por un tercer observador ciego (no involucrado en el estudio) para documentar síntomas como náusea, vómito, reactivación del dolor abdominal, síndrome de respuesta inflamatoria sistémica, tolerancia alimentaria y estancia hospitalaria.
    Resultados: No se encontraron diferencias significativas entre los grupos en cuanto a datos clínicos y bioquímicos. La estancia hospitalaria disminuyó significativamente en el grupo B (5.4 vs. 7.8 días; p < 0.003).
    Conclusión: La alimentación inmediata en las primeras 8 horas del manejo de los pacientes con pancreatitis aguda biliar leve es bien tolerada y segura, y reduce la estancia hospitalaria.
    Background: Mild acute pancreatitis is defined as pancreatic edema, without organic failure and without local complications. Its management consists in three basic elements: hydration, analgesia and fasting. Start refeeding when the patient haven´t pain and referring hungry, but exact time for this is not previously documented.
    Objective: To determine the tolerance to immediate oral feeding (8 hours after the start of management) compared to early feeding (48 hours) in patients with mild acute biliary pancreatitis.
    Method: Included all patient with mild acute biliary pancreatitis, and they were randomized in to two groups: A) early oral feeding (n = 30) and B) immediate oral feeding (n = 29). They were evaluated by a third blind observer (not involved in the study) for documented symptoms like nausea, vomiting, reactivation of abdominal pain, systemic inflammatory response syndrome, food tolerance and hospital stay.
    Results: There were no significant differences between both groups in the clinical data or in the biochemical studies. The hospital stay was significantly less in the group B (5.4 vs. 7.8 days; p < 0.003).
    Conclusion: Immediate oral feeding is well tolerated and secure in patients with mild acute biliary pancreatitis.

    Abstract Objective: To determine the average value of the diameter of the common bile duct by age group in adult patients without bile duct pathology.
    Method: A cross-sectional, observational, descriptive and prospective study in adults without bile duct pathology who underwent abdominal ultrasound. The internal diameter of the common bile duct was measured after questioning. In the descriptive analysis, absolute frequencies, percentages, mean and standard deviation were used in the inferential Student's t test and Pearson's correlation.
    Results: 608 patients without bile duct pathology were included, the mean diameter of the common bile duct was 4.04 ± 1.11 mm; with a minimum of 1.9 mm and a maximum of 8.8 mm. The mean for the population under 30 years was 3.8 ± 0.87, for the 30-39 years group it was 4.27 ± 0.89, for the 40-49 years group it was 4.39 ± 0.83, for the 50-59 years 4.86 ± 1, for the 60-69 years group it was 5.4 ± 0.95, and for those over 70 years it was 6.9 ± 0.8.
    Conclusions: The diameter of the common bile duct in adult patients without biliary pathology is greater at older age.

    Abstract In this study, the characteristic growth of Cladobotryum virescens on nine culture media was analyzed. The growing behavior of this fungus was dependent on the culture medium. In vitro analysis showed that oat agar was better than other media tested with the highest conidia production. The antifungal activity against Fusarium chlamydosporum and Alternaria brassicicola was evaluated by the Dual Culture method. C. virescens displayed high activity against both pathogens acting through antibiosis and mycoparasitism. This effect was increased by a higher competitiveness of the strain for the substrate. Furthermore, the crude ethyl acetate extract of the culture broth was tested in vitro against Botrytis cinerea and Septoria tritici, as well as the hemibiotrophic oomycete Phytophthora infestans using a microtiter plate assay at different concentrations. The extract showed excellent inhibition even below 5 ppm. According to these results, we concluded that C. virescens can be considered as a potential biological control agent in agriculture. To the best of our knowledge, this is the first study to investigate C. virescens as a biocontrol agent for different diseases caused by five relevant pathogens that affect cereals and vegetables.
