    Abstract The South China Sea is the largest marginal sea of southeastern Asia, lying presently under the influences of the Western Pacific Warm Pool and Asian monsoon systems. Sediment cores from this area provide high-resolution records for interpreting millennial- to centennial-scales paleoclimatic changes expressed in the western Pacific. Here we present results of high-resolution paleoceanographic data including planktic foraminifer fauna sea surface temperature (SST) anddepth of thermocline (DOT) estimates along with foraminifer stable isotopes, alkenone SST estimates analyzed froma core takenfrom the southern South China Sea (SCS) near Wan-An Shallow (IMAGES III 1997 cruise core MD97-2151). The intervals of the record presented here cover the past two glacial Terminations (centering at ab. 12,000 and 128,000 yrs B.P.). Our analyses of SST estimates by using planktic foraminifer transfer functions with paralleling measurements of alkenone SST methods all show events of rapid cooling reversals occurrring during the Termination I concurrent with the Younger Dryas (ab. 13-11 kyr B.P.), and Heinrich events reported previously from GISP2 ice core and North Atlantic core studies. Our reconstructions indicate also that theTermination I inthe southern SCS is characterized by a change of monsoon wind systems, with probably much stronger winter monsoon winds in the glacial period. We also found that during the Termination II, there was no such climatic reversal analogous to the Younger Dryas. During oxygen isotope stage 5, our estimates of SST and DOT, and abundances of deep-dwelling planktic foraminifer species all show large-amplitude variations, indicating an instability of monsoon climate during the interglacial period. Our studies also highlight the climatic teleconnections shown by the linkage of the SCS and other regional records for examples from the East China Sea and Chinese loess.

    Abstract Purpose: To investigate the incidence rate and risk factors for anisometropia among young schoolchildren.
    Methods: A population-based cohort study, the Myopia Investigation Study in Taipei, was conducted in primary schools in Taipei City. Children were recruited for biannual comprehensive eye examinations over 2 years. Cycloplegic autorefraction and slit lamp examinations were performed biannually. Data on demographic information, parental history, lifestyle and near-work activities were collected using parent-administered questionnaires at the first and final visits. Anisometropia was defined as ≥1 D difference in the spherical equivalent (SE) refractive error.
    Results: A total of 7035 8-year-old children completed the 2-year follow-up evaluations. The average annual incidence of anisometropia was 3.8%. Multivariable logistic regression analysis revealed that baseline SE (odds ratio [OR]: 0.87 95% CI: 0.80-0.95) and female sex (OR: 1.24, 95% confidence interval [CI]: 1.02-1.50) were significantly associated with incident anisometropia. Among lifestyle risk factors, spending <1 h per day in after-school outdoor activities on weekdays (OR: 1.38, 95% CI: 1.08-1.76) and performing near work at a distance <30 cm (OR: 1.33, 95% CI: 1.08-1.64) were significantly associated with an increased risk of incident anisometropia. In the multiple linear regression analysis, the inter-eye difference in SE increased significantly in children performing near work at distances <30 cm (adjusted β = 0.03; p = 0.02).
    Conclusions: This study indicated the annual incidence of anisometropia in Taiwanese schoolchildren. Less time spent outdoors and shorter eye-to-object distances during near work increased the risk of incident anisometropia.

    Abstract Study design: Systematic review and meta-analysis.
    Objectives: Postoperative ileus (POI) can negatively impact patient recovery and surgical outcomes after spine surgery. Emerging studies have focused on the risk factors for POI after spine surgery. This study aimed to review the available literature on risk factors associated with POI following elective spine surgery.
    Methods: Electronic databases were searched to identify relevant studies. Meta-analysis was performed using random-effect model. Risk factors for POI were summarized using pooled odds ratio (OR) with 95% confidence intervals (CI).
    Results: Twelve studies were included in the present review. Meta-analysis demonstrated males exhibited a higher risk of POI than females odds ratio (OR, 1.76; 95% CI, 1.54-2.01). Patients with anemia had a higher risk of POI than those without anemia (OR, 1.48; 95% CI, 1.04-2.11). Patients with liver disease (OR, 3.3; 95% CI, 1.2-9.08) had a higher risk of POI. The presence of perioperative fluid and electrolyte imbalances was a predictor of POI (OR, 3.24; 95% CI, 2.62-4.02). Spine surgery involving more than 3 levels had a higher risk of POI compared to that with 1-2 levels (OR, 1.82; 95% CI, 1.03-3.23).
    Conclusions: Male sex and the presence of anemia and liver disease were significant patient factors associated with POI. Perioperative fluid and electrolyte imbalance and multilevel spine surgery significantly increased the risk of POI. In addition, through this comprehensive review, we identified several perioperative risk factors associated with the development of POI after spine surgery.

    Abstract Ocular globe injury is a severe ophthalmic emergency that requires immediate attention in the emergency department. In this case report, we present a 35-year-old male who suffered a penetrating ocular injury and globe rupture caused by a nail puncture. The patient presented with severe pain and visual loss and was treated with tetanus vaccination, empirical antibiotics, and pain control, followed by an urgent orbital computed tomography (CT) scan and consultation with an ophthalmologist. The CT scan revealed a retained nail in the ocular space, and an urgent operation was performed to repair the eyeball rupture, remove the intraocular foreign body, and perform an anterior vitrectomy. The patient was discharged 6 days after the operation with a visual acuity of 20/400 and an ocular trauma score of 34. This case highlights the importance of initial emergency physician decision-making and the need for a thorough history-taking and examination when encountering penetrating ocular injuries.
