    Abstract Resumo: Este estudo objetiva identificar o tipo de apoio recebido por cuidadoras familiares de pessoas idosas na pandemia de COVID-19, assim como os fatores socioeconômicos de saúde e relativos ao cuidado associados. Utilizou-se dados do estudo CUIDA-COVID, questionário online realizado entre agosto e novembro de 2020 (n = 4.820). Foram construídos três modelos múltiplos, tendo como desfecho: apoio contratado, apoio familiar e ausência de apoio. As variáveis independentes relacionadas a aspectos socioeconômicos, de saúde e cuidado foram convertidas em categorias dicotômicas. O apoio familiar foi o mais comum (48,6%), seguido pelo ausência de apoio (38,3%) e apoio contratado (13,1%). Não foram observadas associações significativas entre raça/cor, situação de pobreza, problema crônico de coluna e estado de ânimo negativo com nenhum dos tipos de apoio. Foram fatores fortemente associados (RPajustada > 2,0) ao apoio contratado: a não diminuição acentuada da renda na pandemia; a não dedicação ao cuidado em tempo integral; a autoavaliação da saúde boa/excelente; e o alto grau de dependência da pessoa idosa. Para o apoio familiar, as principais associações foram: o não aumento acentuado do esforço dedicado ao cuidado na pandemia; a não dedicação ao cuidado em tempo integral; e a diminuição acentuada da renda na pandemia. Foram as características associadas ao ausência de apoio: viver com o idoso; dedicar-se ao cuidado em tempo integral; residir em domicílios com uma ou duas pessoas; e relatar um aumento acentuado do esforço dedicado ao cuidado na pandemia. No geral, as cuidadoras com apoio contratado tiveram as melhores condições socioeconômicas, de cuidado e saúde. A desassistência foi relacionada a piores condições de saúde e cuidado, enquanto o apoio familiar às piores condições socioeconômicas.

    Abstract This study aims to identify the type of support received by family caregivers of older people during the COVID-19 pandemic, as well as the associated socioeconomic health and care-related factors. Data from CUIDA-COVID study, an online questionnaire conducted from august to november 2020 (n = 4,820) were used. Three multiple models were built, with the following outcome: contracted support, family support, and lack of support. Independent variables related to socioeconomic, health, and care aspects were converted into dichotomous categories. Family support was the most common (48.6%), followed by lack of support (38.3%), and contracted support (13.1%). No significant associations were observed between race/skin color, poverty status, chronic back pain and negative mood with any type of support. Factors strongly associated (adjustedPR > 2.0) with contracted support were: the non-sharp decrease in income during the pandemic; non-dedication to full-time care; self-rated good/excellent health; and the high degree of dependence of the older individual. For family support, the main associations were: the non-marked increase in the effort to care during the pandemic, the non-dedication to full-time care and the sharp decrease in income during the pandemic. The characteristics associated with lack of support were: living with the older person; dedication to full-time care; living in households with 1 or 2 people; and report a sharp increase in effort to care during the pandemic. Overall, caregivers with contracted support had the best socioeconomic, care and health conditions. Lack of support was related to worse health and care conditions, while family support was related to worse socioeconomic conditions.

    Abstract A idade avançada é um dos fatores de risco para o desenvolvimento do câncer de mama. O objetivo deste estudo é apresentar a situação epidemiológica e o acesso ao diagnóstico do câncer de mama entre idosas nas regiões do Brasil. O estudo é descritivo e compreende o período entre 2000 a 2019. Analisam-se indicadores de mortalidade, morbidade e acesso ao diagnóstico. A qualidade da informação da mortalidade foi analisada pelo indicador ‘óbitos com causa básica mal definida’. Segundo os resultados, a maioria dos óbitos foi prematura. As regiões Sul e Sudeste tiveram as maiores taxas de mortalidade por câncer de mama. Nas regiões Norte e Nordeste, há maior subnotificação de óbitos devido à qualidade das informações sobre mortalidade. Conclui-se que as idosas devem ser consideradas nas ações e nos programas sobre câncer de mama e que se deve aumentar o acesso ao diagnóstico das idosas.

    Abstract The objective is to analyze the factors associated with the worsening of the self-rated health (SRH) of Brazilian women who live with elderly people with functional dependence (EFD) during the first wave of COVID-19. ConVid - Behavior Research was used as a data source. For the analysis, the group of women who lived with EFD was compared with those who lived with the elderly without any dependence. Hierarchical prevalence ratio (PR) models were estimated to test the associations between sociodemographic characteristics, changes in income, routine activities and health in the pandemic, with the outcome of worsening SRH. This worsening was more frequent in the group of women living with EFD. After adjusting for hierarchical factors, being black (PR=0.76; 95%CI 0.60-0.96) and having a per capita income lower than minimum wage (PR=0.78; 95%CI 0.64- 0.96) were shown to be protective factors for SRH worsening among EFD co-residents. Indisposition, emergence/worsening of back problems, affected sleep, poor SRH, feeling loneliness and difficulty in carrying out routine activities during the pandemic were positively associated factors. The study demonstrates that living with EFD was associated with a worsening in the health status of Brazilian women during the pandemic, especially among those of higher social status.

    Abstract A qualidade da informação sobre raça/cor é condição necessária para conhecer o impacto da desigualdade na mortalidade. O objetivo deste trabalho é analisar a tendência e a desigualdade na completude da raça/cor dos óbitos de idosos no Sistema de Informações sobre Mortalidade (SIM) entre 2000 e 2015 no Brasil. Analisa-se a completude dessa variável por diferentes abrangências geográficas, a raça/cor mais afetada pela má completude e a associação entre a excelência do preenchimento da raça/cor com a territorialidade e a condição socioeconômica dos municípios. Os dados dos óbitos de idosos provêm do SIM, e as informações de população, dos Censos e estimativas do Ministério da Saúde. Estima-se a variação percentual da proporção da incompletude. A variação percentual de pretos/pardos foi estimada entre 2000 e 2010 para o SIM e Censos. Estima-se regressão logística simples e ajustada (IC95%), tendo, como desfecho excelente, completude da raça/cor e, como resposta, variáveis territoriais e socioeconômicas. Encontrou-se acentuada melhora da qualidade do preenchimento no período, especialmente até 2006, sendo excelente a média nacional desde 2007. Nota-se desigualdade territorial em nível municipal. Municípios de IDH baixo/médio, com alta proporção de pobreza e desigualdade tiveram menor chance de ter excelente completude. O modelo ajustado mostra que a região e o tamanho do município são as características que explicam a excelente qualidade da variável raça/cor. Municípios do Nordeste e de pequeno porte têm menos chance de excelente completude. Conclui-se que raça/cor no SIM tem qualidade para ser utilizada nos estudos de desigualdade da mortalidade dos idosos, salvo exceções em nível municipal.

    Abstract The quality of information on race/color is a necessary condition for knowing the impact of inequality on mortality. This study aims to analyze the trend and inequality in completeness of race/color in death records of elderly in Brazil's Mortality Information System (SIM in Portuguese) from 2000 to 2015. The study analyzes the completeness of this variable according to different geographic areas, the race or color most affected by poor completeness of records, and the association between excellent completion of race/color and the municipalities' territoriality and socioeconomic status. Data on deaths of elderly individuals were obtained from the SIM and information on the population from censuses and Ministry of Health estimates. The study estimates the percent variation in the proportion of incompleteness. The percent variation of black and brown individuals was estimated from 2000 to 2010 from the SIM and censuses. Crude and adjusted logistic regression (95%CI) were used to estimate completeness of race/color as the outcome and territorial and socioeconomic characteristics as independent variables. We found a sharp improvement in quality of completion during the period, especially up to 2006, with an excellent average since 2007. The findings showed territorial inequality at the municipal level. Municipalities with low/medium HDI (with a high proportion of poverty and inequality) showed lower odds of excellent completeness. The adjusted model shows that the region and size of the municipality are characteristics that explain the excellent quality of the race/color variable. Municipalities in Northeast Brazil and small municipalities have the lowest odds of excellent completeness. In conclusion, race/color in the SIM has sufficient quality to be used in studies on inequality of mortality in the elderly, with exceptions at the municipal level.

    Abstract The high level of institutionalization of Brazilian social policies for the elderly stands out in Latin America. However, a problem underlying its consolidation is the lack of indicators for their monitoring and evaluation. This paper aims to propose indicators that are sensitive to the different dimensions of health policies carried out for this population. The study is divided in three stages of research. On the first it was outlined an overview of the main public health policies dedicated to the elderly and identified common guidelines and goals concerning health. On the second stage we selected social and health indicators to follow the thematic areas. The third stage consisted in calculating the indicators proposed for Brazil and its macroregions. Considering the preliminary analysis, we can conclude that although it is possible to calculate sensitive indicators to follow the health policies using data sources available in Brazil, limitations regarding the timeliness of the data, their disaggregability and reliability restrict their use in the improvement of public policies.

    Abstract The growth in longevity in Brazil has drawn attention to more useful population health measures to complement mortality. In this paper, we investigate socio-spatial differences in life expectancy and healthy life expectancy based on information from the Brazilian National Health Survey (PNS), 2013 and 2019. A three-stage cluster sampling with stratification of the primary sampling units and random selection in all stages was used in both PNS editions. Healthy life expectancy was estimated by Sullivan's method by sex, age, and Federated Units (UF). Severe limitations to at least one noncommunicable chronic disease (NCD) or poor self-rated health were used to define the unhealthy state. Inequality indicators and a Principal Component analysis were used to investigate socio-spatial inequalities. From 2013 to 2019, both life expectancy and healthy life expectancy increased. The analysis by UF show larger disparities in healthy life expectancy than in life expectancy, with healthy life expectancy at age 60 varying from 13.6 to 19.9 years, in 2013, and from 14.9 to 20.1, in 2019. Healthy life expectancy in the wealthiest quintile was 20% longer than for those living in the poorest quintile. Wide socio-spatial disparities were found with the worst indicators in the UF located in the North and Northeast regions, whether considering poverty concentration or health care utilization. The socio-spatial inequalities demonstrated the excess burden of poor health experienced by older adults living in the less developed UF. The development of strategies at subnational levels is essential not only to provide equal access to health care but also to reduce risk exposures and support prevention policies for adoption of health behaviors.

    Abstract Background: There are no validation studies on patient satisfaction surveys in Spanish that can evaluate a hospital pharmacy drive-thru service.
    Objective: To develop and apply a pharmacy drive-thru satisfaction survey in Spanish during the COVID-19 pandemic with an analysis of the instrument validation.
    Methods: This was a qualitative study for developing, validating, and measuring patient satisfaction who used the drive-thru pharmacy during the COVID-19 pandemic. Content validity was obtained by a two-round Delphi and patient interview for apparent validity. The questionnaire was administered to 110 patients. The researchers made an item reduction by inter-item and item-total correlation analysis, stability validation by a test-retest, a test of reliability by Cronbach's alpha, and extraction of factors by an exploratory factorial analysis. Likewise, confirmatory factor analysis was developed to obtain a structural equation model based on generating an instrument of two sub-models of latent factors (service and place) with ten observed variables (items).
    Results: A questionnaire was developed that relates six observable variables to the latent factor service and four observable variables to the latent factor place which are ten items based on a Likert scale from 1 to 5, obtaining a Cronbach's alpha = 0.901. The mean population satisfaction score was 4.523. The model presented a Root Mean Square Error of Approximation (RMSEA) of 0.026 (0.000-0.098), and standardized beta values greater than 0.2 according to the confirmatory factor analysis. Therefore, the goodness-of-fit of our model is consistent and the instrument of patient satisfaction with the use of drive-thru has been validated. Patient satisfaction had a mean of 4.9 points.
    Conclusions: This study developed and validated a reliable scale that evaluates satisfaction in a hospital pharmacy drive-thru service during COVID-19 pandemic that can be applied in other Spanish speaking countries. A great percentage of the patients that were evaluated had good satisfaction.
