    Abstract Context: Oral cancer is the major cause of mortality and morbidity worldwide. There are many factors that influence the tumor microenvironment that promotes tumorigenesis. Hypoxia is one of the factors that affects the process of angiogenesis by inducing proangiogenic factors to maintain the blood supply which in turn enhances the aggressiveness of the tumor and prognosis of solid tumors such as oral squamous cell carcinoma.
    Aim and objective: The aim of the study was to compare the expression of hypoxia-inducible factor 1α (HIF-1α) and hypoxia-inducible factor 2α (HIF-2α) in various histological grades of oral squamous cell carcinoma immunohistochemically.
    Methodology: Immunohistochemical evaluation of HIF-1α and HIF-2α was done in 90 samples of oral squamous cell carcinoma which were graded histologically into 30 samples each of well, moderately and poorly differentiated squamous cell carcinoma. Statistical evaluation: Statistical analysis was done to study the prognostic significance of the biomarkers.
    Results: All the cases showed positivity for expression of HIF-1α and HIF-2α. The number of positive staining in both markers reduced as the tumor severity increased from well to poorly differentiated. The expression of MIL of HIF-2α was higher than HIF 1α and HIF 2α expression was mostly seen in cytoplasmic in well-differentiated and nuclear in both moderately and poorly differentiated OSCC suggestive that HIF-2α is a more specific marker to hypoxia.
    Conclusion: Hypoxia is an essential factor that triggers other angiogenic switch and inflammatory factors which facilitates the process of tumorigenesis. This is also important for predicting the treatment outcome and prognosis of the patients. HIF-2α is a more sensitive marker that appears to be correlated and could perhaps serve as a good surrogate marker of hypoxia.

    Abstract We report two cases who attended the genitourinary medicine clinic for the treatment of epididymo-orchitis. They did not respond to treatment and subsequent ultrasound sonography (USS) of testis confirmed microlithiasis and excluded any inflammatory process or carcinoma in situ. Because of its association with testicular cancer, these patients should be followed up by USS at least once a year on a long-term basis after diagnosis. Patients should be encouraged for testicular self-examination.

    Abstract A boy aged 15 years infected a girl of 13 years with Chlamydia and genital warts. The boy has been engaged in sexual activities for the last four years. There was no dispute that the boy had had sex with the girl. He was prosecuted but acquitted by the Court on the ground of 'defence of sleepwalking'. This is the first case where sexually transmitted infections like Chlamydia and genital warts have been transmitted sexually through sleepwalking. This case also raises the issues of underage sex and issues of confidentiality.

    Abstract The definition of the age of young offenders was changed by an Act of Parliament (The Crime and Disorder Act 1998), which was implemented by the Home Office on 1 April 2000. This Act brought down the upper-age limit of young offenders from 20 to 17. Our objective was to investigate the sexually transmitted infections (STIs) among this redefined group of young offenders. Among various types of STIs, we observed that a significant number of young prisoners had complaints of testicular lumps (35%), which were not reported in the past. We tried to find out the reason for this common complaint and believe that this was due to extra vigilance, and testicular self-examination in conjunction with sex and relationship programmes which ran alongside other programmes developed as a joint venture by Prisoner Learning and Skills Unit, Prison Health Policy Unit and Sex Education Forum.

    Abstract The presence of Peyronie's disease can serve as a potential defence against an accusation of rape. However, it can also facilitate an act of sexual intercourse establishing commission of a rape where it does not act as a defence. We describe two cases of Peyronie's disease where two defendants charged with rape pleaded not guilty on the ground of the presence of Peyronie's disease and they argued that they could not have committed the acts because of the severe bending of the penis. One of them was acquitted, whereas the other was found guilty and was imprisoned for six years for the offence of rape. The reason for the difference is development of the condition at a particular site in the penis with different implications. In one defendant, the fibrosis developed at the ventral site producing a curvature downwards, and in the other the fibrosis developed in the dorsal aspect producing a curvature upwards, which facilitated penile penetration.
