    Abstract The Turkana people inhabit arid regions of east Africa-where temperatures are high and water is scarce-and they practice subsistence pastoralism, such that their diet is primarily composed of animal products. Working with Turkana communities, we sequenced 367 genomes and identified 8 regions putatively involved in adaptation to water stress and pastoralism. One of these regions includes a putative enhancer for STC1-a kidney-expressed gene involved in the response to dehydration and the metabolism of purine-rich foods such as red meat. We show that STC1 is induced by antidiuretic hormone in humans, is associated with urea levels in the Turkana themselves, and is under strong selection in this population (s∼0.041). This work highlights that partnerships with subsistence-level groups can lead to new models of human physiology with biomedical relevance.

    Abstract A case of pacemaker syncope associated with cardiorespiratory arrest is reported. This patient was seen three times in emergency departments before finally being admitted for evaluation. Ventricular tachycardia due to pacemaker firing in the T-wave on electrocardiogram was observed. The patient was managed with cardioversion, temporary pacing and replacement of the generator and wire. The dislodgement of the pacemaker wire was confirmed at operation. Patients with pacemaker implants who present with unexplained syncopal attacks should be admitted to the hospital for monitoring, diagnostic work-up, and treatment.

    Abstract A large number of diseases may present with respiratory distress. In adults, upper airway obstruction (UAO) is relatively rare. Consequently, UAO may initially be overlooked in the differential diagnosis of the dyspneic patient. Because it may progress rapidly, delays or errors in diagnosis can be critical. During an eight-month period in one emergency department, seven adult patients with potentially life-threatening diseases of the upper airway were seen. To reacquaint physicians with the syndrome of mechanical obstruction of large airways, several illustrative cases are presented and the syndrome is discussed.
