    Abstract Severe traumatic brain injury (TBI), such as that suffered by patients with cerebral contusion, is a major cause of death and disability in young persons. Effective therapeutics to treat or mitigate the effects of severe TBI are lacking, in part because drug delivery to the injured brain remains a challenge. Promising therapeutics targeting secondary injury mechanisms may have poor pharmacokinetics/pharmacodynamics, unwanted side effects, or high hydrophobicity. To address these challenges, we have developed a multi-lamellar vesicle nanoparticle (MLV-NP) formulation with a narrow size distribution (243 nm in diameter, 0.09 polydispersity index) and the capability of encapsulating hydrophobic small molecule drugs for delivery to the injured brain. To demonstrate the utility of these particles, we produced dual-fluorescent labeled nanoparticles containing the organic dyes, coumarin 153 and rhodamine B, that were delivered intravenously to Sprague-Dawley rats and C57Bl6/J mice at 1, 1 and 4, 24, or 48 h after controlled cortical impact injury. Distribution of particles was measured at 5, 25, 48, or 49 h post-injury by fluorescence microscopy of coronal brain sections. In all cases of MLV administration, a 1.2- to 1.9-fold enhancement of ipsilateral fluorescence signal was observed compared to the contralateral cortex. Enhanced fluorescence was also observed in the injured hippocampal tissue in these animals. MLV-NPs administered at 1 h were observed intracellularly in the injured hemisphere at 48 h, suggesting the possibility of concentrated drug delivery to injured cells. These results suggest that MLV-NP delivery of therapeutic agents may be a viable strategy for treating cerebral contusion TBI.

    Abstract Renal extracorporeal shock wave lithotripsy (ESWL) is a noninvasive and effective method for the treatment of a variety of urinary tract calculi with significantly lower morbidity than both surgical procedures for stone removal (pyelolithotomy/ureterolithotomy) and percutaneous nephrostolithotomy. Though significant, severe complications are rare, ESWL therapy can result in a wide spectrum of tissue injury. This article serves to review the currently known complications of ESWL, which radiologists may encounter, providing an understanding of the potential effects of shock waves on body organs and the resultant functional and morphologic changes that may result.

    Abstract This article examines the current roles of hysterosalpingography and other imaging studies used in diagnostic assessment and treatment of infertility in women. The advantages and limitations of hysterosalpingography, transabdominal or transvaginal sonography, and MR imaging in detecting abnormalities of the uterus (congenital anomalies, intrauterine adhesions, myometrial and endometrial derangements) and tubal and peritubal disease are presented. The use of sonography to evaluate cyclical endometrial changes, to assess development of the follicle and corpus luteum, to determine whether ovulation has occurred, and to detect dysfunctional ovulatory dynamics is described, as is the role of this technique in improving the prognostic value of the postcoital test for assessing the quality of cervical mucus. With the development and availability of newer and improved methods for the treatment of infertility disorders in women, comprehensive and detailed evaluation to determine the precise underlying cause or causes of the infertility is important to allow formulation of the most appropriate therapeutic plan. With careful and thorough assessment, the cause of infertility can be identified in 90-95% of couples evaluated.

    Abstract This article serves to highlight the recent advances that have been made in the workup and differential diagnosis of erectile dysfunction, focusing particularly on the role of imaging in this diagnostic assessment. Intracavernosal injection of vasoactive agents, duplex sonography with pulsed Doppler analysis, penile arteriography, and dynamic infusion pharmacocavernosometry/cavernosography are among the techniques discussed. With the development and availability of nonprosthetic treatment options, identification of the specific underlying cause of a patient's impotence (as determined by these methods) allows the formulation and institution of a therapeutic plan based on the patient's underlying pathophysiology and appropriate for the individual's case. On the basis of our experience, we present a practical and cost-effective algorithm for the diagnostic evaluation of the impotent patient.
