    Abstract La République de Turquie a tourné le dos au Moyen-Orient pendant la plus grande partie du xxe siècle. Ce repli stratégique et culturel, acté dans les années 1920 pour consolider lÉtat naissant après leffondrement de lEmpire ottoman et refonder une nation turque débarrassée des influences orientales, a perduré jusquà la fin de la guerre froide. Le monde arabe et iranien était devenu lOrient dune Turquie qui se voulait fermement ancrée dans la modernité occidentale. Le contraste est aujourdhui saisissant : la Turquie en plein renouveau, progressivement libérée des tabous du kémalisme, réinvestit rapidement le Moyen-Orient, devenu terrain dexpansion économique et dexpérimentation diplomatique. Elle se positionne comme une puissance régionale à part entière, sur le mode du soft power. Le Moyen-Orient est même parfois présenté comme lalternative à une perspective européenne en berne. Mais les « printemps arabes » posent un sérieux défi au renouveau de linfluence turque dans la région. Modèle naturel pour les futures démocraties arabes, ou acteur impérial qui défend au plus près ses intérêts de puissance : quel sera le rôle de la Turquie dans un contexte de profonde instabilité régionale ? La diplomatie de lAKP, le parti dorigine islamiste qui dirige le pays depuis 2002, subit ici un test majeur, entre recherche déquilibre et exercice de responsabilité

    Abstract Background and purpose: The retrograde femoral approach is an established technique for implantation of nails for leg lengthening and correction and in cases of distal femoral fractures. The purpose of this study was to determine the 10-year outcome of this technique by analyzing the clinical long-term effects and radiological status of the knee after leg lengthening via a retrograde femoral approach.
    Patients and methods: This retrospective single-center study included 13 patients (median age at surgery 17 [range 15-20] years) who underwent unilateral, retrograde, femoral lengthening with a motorized nail. Outcome measurements were graded variables of the SF-36, ISKD score, and Lysholm score. MRI of both knees was performed in all patients. MRI was evaluated for the presence of degenerative changes and compared with the healthy contralateral knee. Cartilage condition was graded according to the International Cartilage Repair Society (ICRS) scoring system.
    Results: All patients were pain-free and had a full range of motion 10 (range 10.0-12.2) years after surgery. All postoperative knees showed fibrosis of Hoffa's fat pad and moderate to severe cartilage defects (ICRS Grade 2-4) of the trochlear groove (nail entry site). 6 out of 13 operated knees exhibited retropatellar cartilage defects.
    Conclusion: Our study showed that patients were pain-free, but cartilage defects at the entry point and arthrofibrosis at Hoffa's fat pad were observed without causing clinical impairment.

    Abstract Objective: Whether the day of the week on which the child presents affects timely diagnosis and risk of diabetic ketoacidosis (DKA) in children with new-onset type 1 diabetes (T1D) is not known.
    Research design and methods: We used data of 30,717 children with new-onset T1D during the last 10 years from the German Prospective Diabetes Registry. We determined the odds ratios of T1D diagnosis and DKA on a weekday, public holiday, and school vacation.
    Results: Compared with workdays, the odds ratios of being diagnosed with T1D were lower on weekends (0.39 [95% CI, 0.38-0.41]), public holidays (0.57 [0.53-0.63]), and school vacations (0.83 [0.80-0.85]). The odds of DKA diagnosis were also reduced on weekends (0.55 [0.52-0.59]), public holidays (0.73 [0.63-0.84]), and school vacations (0.85 [0.80-0.90]). Results did not change during the coronavirus 2019 pandemic.
    Conclusions: New-onset T1D and DKA in children are more often diagnosed during weekdays than weekends and holidays.

    Abstract This study assesses whether combining information about diseases and sequelae supports learning about disease risks and influences related health behavior (vaccination).
    Objective: To test whether extending knowledge about the risks of primary diseases (e.g., influenza) with causally linked secondary diseases (sequelae such as sepsis) can decrease vaccine hesitancy in older adults, who are especially vulnerable to primary and secondary diseases.
    Method: In a preregistered longitudinal online experiment, 585 German participants > 60 years of age were randomly assigned to a 3 (time: before and after leaflet presentation, 3-month follow-up; within) × 3 (educational leaflet type: sepsis leaflet, traditional vaccination leaflet, and control leaflet; between) mixed-measurements design. The assessed outcomes were knowledge about influenza, pneumococci, and sepsis; risk perceptions; and immediate and long-term vaccination intention and behavior for pneumococcal and influenza vaccinations.
    Results: The sepsis leaflet immediately increased the knowledge about influenza (effect size, η² = .080), pneumococci (η² = .071) and sepsis (η² = .113), risk perceptions (η² = .007), and intentions for both vaccinations (both η² = .015). Behavior during the follow-up did not differ between the conditions. Additional mediation analysis showed that increased knowledge immediately after the experiment predicted increased risk perceptions and intentions 3 months later (
    Conclusion: Because immediate increases in knowledge and risk perceptions did not change behavior in the long term, extended knowledge interventions might be more effective in locations where positive intention can directly turn into action, such as doctors' clinics.     Abstract iKNOW is the first evidence-based digital tool to support personalized counseling for women in Germany with a hereditary cancer risk. The counseling tool is designed for carriers of pathogenic gBRCA (germline breast cancer gene) variants that increase the lifetime risk of breast and ovarian cancer. Carriers of pathogenic variants are confronted with complex, individualized risk information, and physicians must be able to convey this information in a comprehensible way to enable preference-sensitive health decisions. In this paper, we elaborate on the clinical, regulatory, and practical premises of personalized counseling in Germany. By operationalizing these premises, we formulate 5 design principles that, we suggest, are specific enough to develop a digital tool (eg, iKNOW), yet wide-ranging enough to inform the development of counseling tools for personalized medicine more generally: (1) digital counseling tools should implement the current standard of care (eg, based on guidelines); (2) digital counseling tools should help to both standardize and personalize the counseling process (eg, by enabling the preference-sensitive selection of counseling contents from a common information base); (3) digital counseling tools should make complex information easy to access both cognitively (eg, by using evidenced-based risk communication formats) and technically (eg, by means of responsive design for various devices); (4) digital counseling tools should respect the counselee's data privacy rights (eg, through strict pseudonymization and opt-in consent); and (5) digital counseling tools should be systematically and iteratively evaluated with the users in mind (eg, using formative prototype testing to ensure a user-centric design and a summative multicenter, randomized controlled trial). On the basis of these paradigmatic design principles, we hope that iKNOW can serve as a blueprint for the development of more digital innovations to support personalized counseling approaches in cancer medicine.
