    Abstract In this work, we provide an up-to-date summary of the available molecular- and cell-related mechanisms by which alpha1-antitrypsin (AAT) protein could be of benefit in treating COVID-19 patients. As well, we demonstrate the current status in terms of the ongoing clinical trials using AAT in COVID-19 patients. Finally, we touch on the potential role gene therapy and stem cell-based gene therapy could have in such emerging and serious condition caused by the SARS-CoV-2 virus.

    Abstract Introduction: The placement of central venous catheters (CVC) is a frequent procedure in intensive care. It is not devoid of complications, the diagnosis of which relied for a long time on the chest X-ray. Currently, ultrasound appears to be an interesting alternative.
    Aim: To report the impact of the use of ultrasound on the time to exclusion of mechanical complications after CVC placement.
    Methods: This is a prospective, multicenter, comparative, double-blind study. Were included the patients in whom the placement of a CVC was decided. After placement, a chest X-ray was ordered and an ultrasound was performed to look for signs of misplacement and pneumothorax. The two examinations were interpreted by two different doctors. The primary endpoint between the ultrasound group and the RTX group was the time "T1" represented by the time required to exclude complications.
    Results: 30 patients were included in our study. The mean ultrasound T1time was significantly lower than the mean radiological T1time (p=0.000). Only one case of pneumothorax was observed. It was first detected by ultrasound. For the 29 other patients, exclusion of pneumothorax was confirmed by ultrasound and chest X-ray. No misplacement type complications detected. This was confirmed by ultrasound and radiological exclusions.
    Conclusion: Ultrasound is a faster tool than RTX in excluding mechanical complications after CVC placement. It guarantees a non-irradiating examination as efficient as chest X-ray for intensive care patients.

    Abstract The present study investigated the cytogenetic and testicular damage induced by the antiepileptic drug, sodium valporate (SVP) in albino rats and the effect of saffron aqueous extracts. Treating rats with SVP caused a significant increase in the chromosomal aberrations either structural or numerical and decreased the mitotic index. Besides, animals administered SVP showed DNA damage appeared in the single strand breaks (comet assay). Testis of SVP-treated rats showed many histopathological changes. A significant decrease in seminiferous tubules and their epithelial heights diameters and inhibition of spermatogenesis was recorded. In addition, the number of sperm head abnormalities was increased. Biochemical results revealed an increase in malondialdhyde (MDA) which is lipid peroxidation marker and a significant decrease in the level of serum antioxidant enzyme, catalase (CAT) and reducing antioxidant power (RAP). Animals given SVP and saffron showed an improvement in chromosomal aberrations, mitotic index, DNA damage and testicular alterations caused by SVP. Moreover, MDA decreased and CAT and RAP increased. It is concluded from the present results that the ameliorative effects of saffron extract against SVP-induced cytogenetic and testicular damage in albino rats may be due to the presence of one or more antioxidant components of saffron.

    Abstract L'hémorragie grave du péripartum demeure une des causes principales de mortalité maternelle. L'objectif de notre étude était de décrire le profil épidémiologique des patientes qui ont été prises en charge en milieu de réanimation suite à une hémorragie grave du péripartum et de rechercher d'éventuels facteurs de risque de mortalité. Notre étude est rétrospective descriptive et analytique. Nous avons inclus tous les cas d'hémorragie du péripartum ayant séjourné en unité de réanimation obstétricale du centre de maternité et de néonatologie de Tunis (CMNT) au cours de la période allant de janvier 2010 à Décembre 2013. Nous avons recueilli les paramètres démographiques, obstétricaux, ceux relatifs à la prise en charge chirurgicale et réanimatoire, les scores de gravité SAPS obstétrical et APACHEII, ainsi que la morbi-mortalité. Au total nous avons colligé 322 cas sur quatre ans. La répartition annuelle des patientes ainsi que les caractéristiques démographiques et obstétricales étaient comparables dans leur globalité sur les quatre années. Les pratiques thérapeutiques étaient également comparables. Le taux global de mortalité par hémorragie dans notre unité était à 4,7%, avec un taux annuel de mortalité stable. L'analyse des facteurs de risque de mortalité par hémorragie en milieu de réanimation a montré une association statistiquement significative entre la survenue du décès et les facteurs suivants: recours aux catécholamines, survenue de sepsis, èdème pulmonaire aigu, coagulation intravasculaire disséminée, insuffisance rénale aiguë avec recours à l'hémodialyse, SDRA ou TRALI, atteinte neurologique grave, défaillance multiviscérale et arrêt cardiaque récupéré.

    Abstract A biochemical characterization and a geographic variability of Thymus capitatus L. essential oils were undertaken. Chromatographic analysis using gas chromatography-flame ionization detector and gas chromatography-mass spectrometry of essential oils extracted from thyme aerial parts from four Tunisian regions (Kef, Bizerte, Boukornine and Grombalia) showed the predominance of carvacrol (75.79 ± 2.07%) in Boukornine region, thyme essential oils being carvacrol chemotype. Obtained results of antimicrobial activities showed that thyme essential oil has important antibacterial activity which was strongly related to the organoleptic quality of the oil which appeared strongly region-dependent. The inhibitory effect of thyme essential oils on the bacteria and fungi development showed that Bacillus cereus and Fusarium solani were the most sensitive bacteria and fungi, respectively. From these results, we have concluded that thyme essential oil could be used to cure mycotic infections and may have role as pharmaceutical and preservative agents.

    Title translation Early postpartum discharge: outcomes and risk factors of readmission.
    Abstract The objective of this study is to evaluate the practice of early postpartum discharge by analyzing maternal readmission rates and identifying readmission risk factors. This is a prospective and analytical study of 1206 patients discharged from hospital on postpartum day 1. For each patient we collected the epidemiological data, the course of pregnancy and childbirth. We identified the causes of readmission and their evolution. Cesarean delivery rate was 42%. Maternal readmission rate was 0.99%. The average length of stay in hospital after readmission was 26 hours. Intestinal transit disorders were the most frequent reason for consultation (50% of cases) followed by fever (25% of cases). The readmission risk factors identified in our study were: cesarean section (p = 0.004), emergency cesarean section (p = 0.016) anemia (P < 0.001) and thrombopenia (p = 0.003). Early postpartum discharge seems a safe option for the mother and their newborn children subject to the ability to clearly communicate health information to the patient and to the compliance with selection criteria.

    Abstract Objective: To elucidate for the first time the phenological stage effect on polyphenol, flavonoid and proanthocyanidin contents of pennyroyal (Mentha pulegium) as well as their antioxidant capacities and insecticidal potentials against Tribolium castaneum and Lasioderma serricorne. Methods: Crude methanol extracts from pennyroyal (Mentha pulegium) aerial parts were evaluated for their antiradical, reducing power activities and repellent potentials against selected coleopteran insects during different phenological stages. Results: Phenolic contents of pennyroyal aerial parts and their antioxidant effects were significantly dependent on maturity stage of the plant. The maximum of phenol production was reached during the full flowering stage. Apigenin was the major phenol in Mentha pulegium with an amount of 6.01 mg/g of dry vegetable matter during this period. Nevertheless, at fructification stage, the highest antioxidant activity was not related to high phenolic content. The repulsive effect of pennyroyal extract showed that Lasioderma serricorne was more susceptible to pennyroyal extract than Tribolium castaneum with corresponding median repellent dose values of 0.124 and 0.006 mg/mL respectively. Moreover, great differences in insect repulsion depending on extract concentrations, exposure time and developmental stage was observed. For the lowest concentration (0.125 mg/mL), the repellent effect against Tribolium reached its maximum (90%) during the fructification stage (after 24 h of exposure) while this repellent effect was 80% during the vegetative stage. Conclusions: Due to the strong insecticidal potential of pennyroyal extracts, this study highlights the therapeutic properties of this plant and encourages its use as a safer, environmental-friendly and efficient insecticide in food industry.

    Abstract Cette étude nous permettra d'évaluer la pratique d'une sortie précoce en post-partum en analysant le taux de réadmission maternelle et en essayant d'identifier les facteurs de risque de ré hospitalisation. Il s'agit d'une étude prospective et analytique à propos de 1206 patientes sorties de l'hôpital à J1 du post-partum. Pour chaque patiente, nous avons noté les données épidémiologiques, le déroulement de la grossesse et de l'accouchement. Nous avons identifié les causes des ré hospitalisations ainsi que leur évolution. Le taux de césariennes était de 42%. Le taux de réadmissions maternelles était de 0,99%. La durée moyenne du séjour lors de la ré hospitalisation était de 26 heures. Les troubles du transit ont été le motif de consultation le plus fréquent (50% des cas) suivis par la fièvre (25% des cas). Les facteurs de risque de ré hospitalisation, identifiés dans notre étude étaient: la césarienne (p=0,004), la césarienne en urgence (p=0,016), l'anémie (p=0,001) et la thrombopénie (p=0,003). La sortie précoce en post-partum semble une option sure pour la maman et le nouveau-né sous réserve d'une information claire de la patiente et du respect des critères de sélection.
