    Abstract The recent global energy crisis has shocked Bangladesh's power sectors, and experts recommend using alternative energy sources to conserve natural gas, fossil fuels, and electricity. Numerous investigations on the photovoltaic thermal (PVT) system have been carried out to get the source efficiently. As a result, a parametric evaluation of the PVT system's efficiency in Dhaka, Bangladesh, is investigated numerically using CNT nanofluid as a coolant.     Abstract Takayasu's arteritis is a chronic large vessel vasculitis that mainly affects the aorta and its major branches such as brachiocephalic, carotid, subclavian, vertebral, renal, coronary and pulmonary arteries. It most commonly occurs in female at child bearing age and female to male ratio is 8:2. Initial presentations of Takayasu's arteritis may be nonspecific like fever, malaise, weakness, fatigue, arthralgia, myalgia and weight loss but in advanced stage there may be features of vascular inflammation like segmental stenosis, occlusion, dilatation and/or aneurysm. It is a sub acute process over months to years and usually with good collateral formation. So, critical limb ischemia due to acute onset of vascular stenosis is very rare. Here we are reporting a case of a 50-year- old male patient who presented with digital gangrene due to critical limb ischemia. Though the age of onset was at 50 years and the patient lacks clinical features like constitutional symptoms, bruit, claudication, asymmetrical blood pressure, the patient was diagnosed as a case of Takayasus's arteritis. The patient dramatically improved with oral glucocorticoid treatment. Although rare, takayasu's arteritis can presents only with digital gangrene without having other clinical features. The physicians should be aware of this unusual but limb threatening presentation and more studies are needed to find out the exact mechanism of this presentation.

    Abstract This study is aimed to perform a numerical time-dependent investigation thermal conductivity effect of the annular cylinder within a vented cavity using CNT based-water nanofluid. For demonstrating the effect of thermal conductivity, four distinct hollow cylinder materials such as Ks = 0.5(Plastic tiles), Ks = 0.84(Clay tiles), Ks = 1.1(Concrete tiles), and Ks = 2(Slate tiles) are introduced together with a suitable variation of dimensionless time (0 ≤ τ ≤ 1). The governing equations of the model with associated boundary conditions is solved using finite element based Galerkin's weighted residual method. Different contour plots for thermal and flow field transformation and mean Nusselt number, mean fluid temperature, bulk convective field temperature, temperature gradient, pressure gradient, vortices, and fluid velocity magnitude are presented for qualitative and quantitative thermal performance analysis. With the decrease of solid thermal conductivity, 27.3% thermal transport enhancement is noted from the heated surface of the cylinder. However, a 16.3% increase in the bulk fluid temperature has been recorded with the increase in cylinder conductivity. The numerical outcomes from this investigation propose a better thermo-fluid efficiency compared to the existing methodology which can be suggestive to engineers and researchers for designing heat exchangers, heat pipes, and other thermal systems.

    Abstract Rotating cylinder movement in a cavity flow is an exciting field of study in heat transfer. Considerable research has been carried out on rotating cylinders under MHD mixed convection in various types of enclosures. However, considering partially heated square enclosure and magnetic field using CNT-water nanofluid is very limited. This study's goal is to assess the hydrothermal phenomena in a square enclosure with a rotating cylinder. Simulation has been conducted for different rotational speeds (Ω) and dimensionless times (τ) to observe the thermal and fluid flow behaviour. The Galerkin Residual based finite element method has been used to conduct numerical calculations. The results are shown as isotherms, streamlines, and average Nusselt number at the cylinder wall. Moreover, the drag force at the moving wall, and the fluid properties such as the root mean square (rms) of velocity, the temperature, the vorticity functions, and the average fluid temperature are also presented. The heat transfer rate, drag force, rms velocity, and temperature increase with the rise of rotational speed and dimensionless time rise. Maximum vorticity occurs at Ω = 8 and τ = 1. The maximum vorticity function increases 12 times with the increasing rotational speed. Higher rotational speed leads to increased average fluid temperature. The case of Ω = 8, τ = 1 shows the most temperature variance, while Ω = 1, τ = 0.1 has the least. Increasing rotational speed results in higher drag force on the cylinder's surface. At Ω = 4, the drag force is 2.8 times greater than at Ω = 2. Overall, the fluid flow and thermal performance boost up while the rotating speed of the cylinder is higher.

    Abstract Non-alcoholic fatty liver disease (NAFLD) is found in one-fourth of the world population and its prevalence is rising. Screening of NAFLD in general population with ultrasonography (USG) is not a cost- effective approach. Alternative methods for prediction of NAFLD are needed. We conducted this study to assess whether basal metabolic index (BMI), waist circumference (WC), waist height ratio (WHtR) is useful for predicting NAFLD. This study was done in a village of Comilla District, Bangladesh from July 2019 to December 2019. Two hundred and nineteen (219) subjects with NAFLD detected on USG and 100 healthy controls without NAFLD were included in this study. The diagnosis of NAFLD was made on the basis of USG by a radiologist. BMI, WC, WHtR were calculated. The area under the curve (AUC) in the receiver operating characteristic (ROC) was calculated to assess the diagnostic ability of BMI, WC, and WHtR for predicting NAFLD. Sensitivity and specificity with optimal cut-off point was calculated. Data were entered into SPSS16for statistical analysis. Mean age in NAFLD group was 45.11±13.50 years. The AUROC values were 0.832, 0.763 and 0.771 for BMI, WC and WHtR respectively. The Cut-off values were 24.4 for BMI, 75.60 for WC and 0.54 for WHtR. Sensitivity and specificity were 80.24%and 79.12% for BMI, 82.34% and 80.45% for WC, 79.56% and 78.98% for WHtR respectively. ROC for BMI, WC and WHtR were above the diagonal line with high sensitivity and specificity. BMI, waist circumference, waist height ratio is useful for predicting NAFLD in rural Bangladeshi population. We can use these simple anthropometric indices as a screening tool for detection of NAFLD in primary health care set up.

    Abstract Purpose: Two cases are described of necrotizing Sweet syndrome (nSS), a rare variant of acute febrile neutrophilic dermatosis that mimics necrotizing soft tissue infections.
    Observation: A 74-year-old female with myelodysplastic syndrome (MDS) presented with isolated periorbital nSS that closely mimicked necrotizing fasciitis (NF); she displayed pathergy to debridement, was exquisitely responsive to corticosteroids, and underwent successful first-stage reconstruction of the eyelid with full-thickness skin grafting. A second 40-year-old female patient with relapsed acute myelogenous leukemia (AML) presented with multifocal nSS most prominently involving the eyelid. Positive herpes zoster virus (HSV) PCR and bacterial superinfection complicated the diagnosis. She improved with chemotherapy for AML and corticosteroid therapy.
    Conclusion: nSS is rare and a high level of clinical suspicion as well as an understanding of its distinguishing features is necessary to avoid undue morbidity. Identification of pathergy, histopathology, microbiology, and clinical context are critical to avoid misdiagnosis of infection.

    Abstract Signalling transduction pathways (STPs) are commonly hijacked by many cancers for their growth and malignancy, but demystifying their underlying mechanisms is difficult. Here, we developed methodologies with a fully Bayesian approach in discovering novel driver bio-markers in aberrant STPs given high-throughput gene expression (GE) data. This project, namely 'PathTurbEr' (Pathway Perturbation Driver) uses the GE dataset derived from the lapatinib (an EGFR/HER dual inhibitor) sensitive and resistant samples from breast cancer cell lines (SKBR3). Differential expression analysis revealed 512 differentially expressed genes (DEGs) and their pathway enrichment revealed 13 highly perturbed singalling pathways in lapatinib resistance, including PI3K-AKT, Chemokine, Hippo and TGF-$\beta $ singalling pathways. Next, the aberration in TGF-$\beta $ STP was modelled as a causal Bayesian network (BN) using three MCMC sampling methods, i.e. Neighbourhood sampler (NS) and Hit-and-Run (HAR) sampler that potentially yield robust inference with lower chances of getting stuck at local optima and faster convergence compared to other state-of-art methods. Next, we examined the structural features of the optimal BN as a statistical process that generates the global structure using $p_1$-model, a special class of Exponential Random Graph Models (ERGMs), and MCMC methods for their hyper-parameter sampling. This step enabled key drivers identification that drive the aberration within the perturbed BN structure of STP, and yielded 34, 34 and 23 perturbation driver genes out of 80 constituent genes of three perturbed STP models of TGF-$\beta $ signalling inferred by NS, HAR and MH sampling methods, respectively. Functional-relevance and disease-relevance analyses suggested their significant associations with breast cancer progression/resistance.
